Oak Street Health Opens New Center In Indianapolis

The center will expand access to holistic healthcare for adults on Medicare in the local community and brings new jobs.

INDIANAPOLIS, IN — Oak Street Health announced the opening of its fourth center in Indianapolis Wednesday, located in the Glendale neighborhood. Oak Street Health, a network of health centers providing a new model of primary care for Medicare patients, celebrated the grand opening of the new center Wednesday afternoon, which is located at 2240 East 53rd St, Suite B-1, Indianapolis, according to a release.

In a release sent to Patch, a spokesperson said the Glendale center will provide area residents access to a "more holistic healthcare option that delivers on Oak Street Health’s mission to provide more personal, equitable and accountable healthcare for all," including access to complimentary transportation to and from appointments, Medicare education classes to help patients understand their insurance benefits, and supplemental services in addition to primary care, and behavioral health to address their healthcare needs in one location.

The Glendale center will also provide social activities and classes for patients and community members so they can stay active and connected with peers, according to a release. Oak Street Health's local workforce can be attributed to the 83 percent of its physicians and 80 percent of staff living within five miles of their center.

Oak Street Health also offers a 24/7 patient support line as well as complex care teams and home nursing visits for patients who are in need, contributing to a 94 percent patient retention rate and a 92 Net Promoter Score, the release said, adding that through the network's preventive services and accessibility, Oak Street Health has decreased hospital admissions by 44 percent, and emergency room visits by 46 percent.
