ANDOVER, MA — An Andover doctor was recently reprimanded by the Massachusetts Board of Medicine.

Dr. William A. Mitchell's license was reprimanded July 13 after he agreed in a consent order that he failed to properly maintain medical records for three patients undergoing surgery, as well as having a patient placed under anesthesia before surgery was ready to proceed, according to a statement from the medical board.

The consent order prohibits Mitchell from performing surgeries and procedures on his own. Mitchell is a board-certified orthopedic surgeon and has been licensed to practice medicine in the state since 1984. He is also licensed in New York.

He has a private office in Andover and is affiliated with Newton-Wellesley Hospital.
