Fredericksburg Health Inspections: 15 Violations At 1 Facility

FREDERICKSBURG, VA — Several facilities in the Fredericksburg area recorded violations during recent inspections from the Rappahannock Area Health District. Inspectors conducted the most recent visits between Jan. 10 and Jan. 16.

In Virginia, all food service locations are supposed to be inspected at least once per year. Facilities with higher risk factors may be inspected as often as four times per year, according to the Virginia Department of Health.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected. Sometimes, the follow-up inspection only considers the most important "priority" violations.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

Depending on the results of the inspection, the health department can pursue several actions.

It can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or officials can temporarily shut down an establishment until certain issues are solved.

The Virginia Department of Health only recommends restaurants have their permits suspended when "the continued operation of the foodservice presents a substantial and imminent threat to the public health."

Here are the most recent violations as listed by the Rappahannock Area Health Department:

  • Bright Beginnings Childcare Center, 85 Cleremont Drive Fredericksburg, VA 22405
    • Date inspected: Jan. 10
    • Total violations: 6
      • No proof of training available on site for foodborne illnesses on-site can be transmitted by food. EHS provided an employee health policy sample. The only employee who works in the kitchen read and signed the report while EHS is typing the report.
      • The food establishment does not have written procedures for cleaning up vomiting or diarrheal events that happen in the facility. EHS provided a bodily fluid clean up procedures sample to be posted in the kitchen.
      • Observed the following time/temperature control for safety items that were opened for over 24 hours without being labeled with an open or discard date: 1) jar of whole milk, 2) mild and medium salsa, 3) carton of liquid eggs. Milk was open five days prior to inspection, eggs and salsa were discarded as the opened over 7 days prior to the inspection.
      • Observed the following time/temperature control for safety items that were opened for over 24 hours without being labeled with an open or discard date: 1) jar of whole milk, 2) mild and medium salsa, 3) carton of liquid eggs. Milk was open five days prior to inspection, eggs and salsa were discarded as the opened over 7 days prior to the inspection.
      • Observed the only food employee in the kitchen without proper hair restraint.
      • Observed the reach in cooler and the microwave in the school aged facility in a near by building need to be cleaned.
  • Encompass Rehabilitation Hospital of Fredericksburg, 300 Park Hill Dr Fredericksburg, VA 22401
    • Date inspected: Jan. 10
    • Total violations: 15
      • Absences of active managerial control observed during the inspection. The person in charge is not monitoring cold holding and hot hold temperatures. Though discussion with a food employee nursing staff is entering the kitchen to wash dishes.
      • Observed asparagus hot holding at 125 degrees. Asparagus was placed in the hot holding unit at 10:00am. Asparagus was removed for reheating.
      • Observed containers of unsalted clarified ghee and minced garlic in oil at 72 degrees on the prep table. Through discussion with an employee food had been out on the prep table since 9:30am. At 10:15 am foods were relocated to the walk-in freezer to be properly cooled to 41 or below within 3 hours and 15mins.
      • Observed a spray bottle of smart clean on the prep table next to foods being prepared. Spray bottle was removed and relocated during the inspection.
      • Observed food employees wearing watches and a bracelet.
      • Observed a single use black souffle cup being used as a scoop inside a container of thick and easy powder. EHS asked an employee how long the cup had been in the container. Employee didn't know. Cup was removed from the container.
      • Observed two cutting broads heavily scored.
      • Observed in poor repair a gap in the hood vent and frost buildup in the ice cream reach-in freezer.
      • Observed numerous pot and pans heavily encrusted with residue on the nonfood contact surface.
      • Observed the following nonfood contact surfaces in need of cleaning: 1. Interior of fryer cabinets 2. Up splash of ice machine 3. Interior of cabinets under the soda machine 4. Interior of ovens 5. Exterior of cooking equipment 6. Shelving throughout the kitchen.
      • Observed a dual check backflow prevention device connected to a Y Splitter connected to the soda machine and the ice and water dispenser.
      • Observed walls, floors (including floor drains) and ceilings throughout the facility are in need of cleaning.
      • Observed ceiling tiles above the soda machines in the self-service area are not smooth and easily cleanable.
      • Through discussion with an employee numerus piece of catering equipment and white plastic puree molds are not being used.
      • Observed employees personal belonging not stored in a designed area. Jackets on shelving used to store clean equipment, protein powder next to customers foods on shelves, small containers of employee's salad dressing on prep table.
  • Pimenta, 1108 Caroline St Fredericksburg, VA 22401
    • Date inspected: Jan. 10
    • Total violations: 1
      • The person in charge is not a certified food protection manager (CFPM) (Servsafe# 16717558 expired on 07/18/2023). A certificate is needed for the type of food that is offered for sale in the facility as per the VA Food Regulations. Person in charge will sign up to take the exam and obtain a valid CFPM.
  • Best Western Aquia / Quantico, 2868 Richmond Hwy Stafford, VA 22554
    • Date inspected: Jan. 10
    • Total violations: 6
      • Facility does not have a valid certification of the training on food protection management.
      • Through discussion with the person in charge, hard boiled eggs stored in the homestyle refrigerator in the kitchen were prepared at employee's home. PIC will not serve the eggs. In addition, PIC will discontinue preparing eggs at home.
      • Observed the following held at improper temperature in the kitchen homestyle refrigerator: 1) cream cheese 43 degrees F. 2) srambeled egg 44 degrees F. Items were out in unit 3 hours prior. Temperature of unit was turned down. Food items were check 1 hour later and temperatures were in range.
      • Observed the no date marking system of TCS foods held for over 24 hours throughout cooling units.
      • Person in charge could not provide valid test strips for chemical sanitizer.
      • Observed the following in need of cleaning: 1) observed debri and black growth on dish rack used to dry dishes. 2) cabinets in self service area. 3) shelving rack over 3-comp sink.
  • The Melting Pot, 1618 Carl D. Silver Parkway Fredericksburg, VA 22401
    • Date inspected: Jan. 10
    • Total violations: 11
      • Person in charge was unable to provide an employee health policy for the facility. EHS provided a copy during the inspection.
      • Person in charge was unable to provide written procedures for how the facility tends to bodily fluid clean ups. EHS provided a copy during the inspection.
      • Observed a food employee use their personal cell phone fail to wash their hands afterwards before putting on a new pair of gloves.
      • Observed the following improper uses for a hand sink: 1. in the ware washing area hand sink a strainer was sitting inside the basin 2. observed an employee fill a plastic food container with water from one of the kitchen hand sinks 3. in the bar area a bar glass washer (scrub pad) was inside the handwashing basin 4. in the kitchen a container of soda nozzles were sitting by the hand sink faucet During the inspection the strainer, glass scrub pad, and container of nozzles were relocated.
      • Observed the following violations: Per discussion with a food employee, employees use the 2 compartment sink to wash dishes and the bar dish machine was in poor working order. After several attempts and switching out the chemicals the machine was at 0ppm. EHS discussed required ware washing procedures of dishes that must happen at the 3 compartment sink or ware washing machine and to call a technician to repair the ware washing machine.
      • Observed the facility does not have a date marking system in place.
      • Observed a steam table used to melt/warm chocolate setting uncovered on the drain board of a 2 compartment sink.
      • Observed a food employee place an unwrapped straw into a customers beverage with their bare hand. EHS advised person in charge to handle to straws with tongs or purchase wrapped straws.
      • Observed rusted shelving throughout the facility. One table in particular in the ware washing area is heavily rusted.
      • Observed cutting boards and a prep station cutting board in need of replacement, they are heavily stained and scratched.
      • The test strips for the quaternary ammonium expired in October 2021 and the chlorine test strips did not have an expiration date on them. EHS advised person in charge to purchase new test strips for both chemicals.
  • Paramount Senior Living at Fredericksburg, 3500 Meekins Drive Fredericksburg, VA 22407
    • Date inspected: Jan. 11
    • Total violations: 6
      • Person in charge was unable to provide written procedures for the clean-up of vomit and diarrhea. A sample copy was provided to the person in charge.
      • Observed two cans of light kidney beans and one can of diced tomatoes dented on the sealed edge. Also one can of light kidney beans with a large dent on the side on the can. Cans were relocated so they will not be used.
      • Observed the plastic plate inside of the ice machine soiled.
      • Observed several ceiling tiles near the walk-in freezer and fire suppression system out of place and gaps around the conduits for the fire suppression system. There is work order in already for the tiles.
      • Observed several stainless steel containers and sheet trays stacked while still wet. Items relocated to the dish machine area.
      • Observed countertops, shelving, and wire shelving units throughout the facility soiled. Also the non-food contact surface of the meat slicer soiled.
  • Merit School of Park Ridge #14, 1081 Parkway Blvd, Stafford, VA, USA Stafford, VA 22554
    • Date inspected: Jan. 11
    • Total violations: 1
      • Observed the following items in the kitchen without a label to identify the contents: 1) a pitcher on the drainboard of the three compartment sink used to store a blue substance, person in charge (PIC) reported was dish detergent, 2) two spray bottles stored under the three compartment sink that contained chemicals. PIC placed a label on the container during the inspection.
  • Riverside Center for the Performing Arts, 95 Riverside Parkway Fredericksburg, VA 22406
    • Date inspected: Jan. 11
    • Total violations: 12
      • Absence of handwashing sink in the bar area without another easily accessible handwashing sink. Bar 3 compartment sink will be used as a handsink and barware will be taken to the kitchen for washing and sanitizing. Ensure handsoap and paper towels are placed at the handsink. Handwashing signage was emailed to the operator.
      • Observed gaps between plumbing and conduits through the walls and ceilings in the food establishment. Discussed sealing the gaps during the inspection.
      • Observed plumbing lines running along the bottom wall in the bar area which do not permit enough space for cleaning. Discussed sealing the plumbing lines.
      • Observed two gaps in hood vents above the fryers and one shield light is out in the hood above the 2 tiered oven.
      • Observed scratched and scored red and green cutting boards.
      • Observed accumulation of debris on the following nonfood contact surfaces: Shelving in walk in cooler by the main kitchen, dishwasher curtains, base of the top 2-tiered convection oven, backsplash of the soda machines, inside fryers, range including stainless steel backsplash of the range.
      • Observed hot water not working on the right side of the 3 vat sink. Instructed the operator to contact a plumber for repairs. Currently the facility does not have the means to fill the sanitizer basin of the 3 vat sink.
      • Observed plumbing connections under the 3 compartment sink in the kitchen are directly plumbed through the grease trap. Absence of adequate backflow prevention. Instructed the operator an air gap or air brake is required before the grease trap and handouts were emailed to the person in charge. Contact Stafford County Building Officials to ascertain if a plumbing permit is required.
      • Absence of covered receptacle in the employee toilet room.
      • Observed the following in need of cleaning: ceiling tiles, floors behind cook lines and drains.
      • Observed unsealed concrete flooring at the bar and gaps in floor tiles in the bar area with unsealed concrete. Discussed sealing concrete to create a smooth and easily cleanable floor surface.
      • Observed the the wall under the dishwasher drainboard damaged and blown out ceiling lights.
  • Merit School of Stafford #75, 1001 Parkway Blvd. Stafford, VA 22554
    • Date inspected: Jan. 11
    • Total violations: 1
      • Observed a hose attached to a Y connector at the mop sink without an air break or a back flow prevention device.
  • Coffee Bar 1010, 1010 Corporate Dr Suite# 101 Stafford, VA 22554
    • Date inspected: Jan. 11
    • Total violations: 5
      • No paper towels at the front hand sink. Paper towels were placed at the sink during inspection.
      • Observed the interior microwave in the prep area in need of cleaning.
      • Per discussion with the person in charge, the wand on the espresso machine, which comes in contact with dairy products, is only being wiped by a sanitizer cloth. The wand needs to be washed, rinsed and sanitized.
      • Observed interior of ice machine in need of cleaning.
      • Observed food employee not wearing hair restraint.
  • El Molcajete Moya 2, 125 Corporate Drive Suite #105 Stafford, VA 22554
    • Date inspected: Jan. 11
    • Total violations: 12
      • Person in charge could not provide documentation on the training of employee health policy.
      • No handwash signage at the hand sink in the ware wash area.
      • Observed the interior of all microwaves in the kitchen in need of cleaning.
      • Observed the can opener soiled. Item was removed to 3-comp sink for cleaning.
      • Observed no date marking system of RTE and TCS foods held for over 24 hours in the walk-in cooler.
      • Observed a package of raw chicken (26 degrees F) and beef (31 degrees F) thawing in a container on the prep table. Items were put out 30 minutes prior. Person in charge relocated items to the walk-in cooler.
      • Observed a fly insect trap device hanging on a shelf in the kitchen. Device was removed.
      • Observed TCS foods on the floor in the walk-in freezer. Items were relocated off the floor.
      • Observed ice build-up on the ceiling and packaged frozen foods in the walk-in freezer. Person in charge removed ice and relocated items.
      • Observed the following in need of caulking: 1) both hand sinks in the kitchen. 2) mop sink.
      • Person in charge could not provide valid test strips for chemical sanitizer and dishmachine.
      • No back flow prevention on hose connected to a Y connector at the mop sink.
  • Hampton Inn, 4391 Hospitality Way King George, VA 22485
    • Date inspected: Jan. 11
    • Total violations: 3
      • The equipment being cleaned in the 3 compartment sink was not observed sanitized. There was no measurable sanitizer in the sink due to the sanitizer supply bottle being empty. Sanitizer was re-stocked during inspection.
      • Observed hard boiled eggs (55'F0 on the serving line and scrambled eggs (49'F), milk (50'F), yogurt (50'F), hard boiled eggs (50'F) in the kitchen reach in cooler cold holding at improper temperatures. Items were voluntarily discarded during inspection.
      • Observed kitchen utensils stored in a container on the microwave oven with the food contact surfaces facing up.
  • Los Dos Amigos, 117 N Main St Bowling Green, VA 22427
    • Date inspected: Jan. 16
    • Total violations: 3
      • There are no hand washing signs in either restroom.
      • The interior surfaces of the ice machine were observed soiled. Machine was cleaned during inspection.
      • Observed employees preparing food without hair restraints.
