Injuries & Moves: Hentges to have surgery; Gose DFA'd; Cantillo up from Triple-A

Sept. 10: LHP to have left shoulder surgery
Hentges will have left shoulder surgery on Sept. 18 after an assessment by Dr. Neal ElAttrache in Los Angeles confirmed an injury involving Hentges' capsule and labrum. The recovery and rehab is expected to take 12 to 14 months, meaning Hentges will likely miss the entire 2025 season.

Hentges, who began the season on the IL and didn't debut until May because of left middle finger inflammation, recorded a 3.04 ERA over 23 2/3 innings (25 appearances) this year. He landed on the IL for the second time on July 12 after experiencing left shoulder fatigue.

Sept. 9: LHP designated for assignment; LHP recalled from Triple-A Columbus
With Alex Cobb still recovering from a blister, the Guardians needed an extra starter before Tuesday. The team decided to shift Ben Lively's start from Monday to Tuesday and bring up Cantillo to make his fifth start for the Guardians this season. He owned a 7.71 ERA in his first 18 2/3 big league innings. To make room on the roster, the Guardians designated Gose for assignment.

Sept. 8: RHP designated for assignment; LHP selected from Triple-A Columbus
The Guardians designated Barlow for assignment prior to their series finale against the Dodgers on Sunday. Acquired in a trade with the Padres before the season, Barlow went 3-3 with a 4.25 ERA and 68 strikeouts against 31 walks in 55 innings for the Guardians this year.

He had struggled as of late, allowing 12 hits and 11 earned runs in his last 11 2/3 innings.

“We're devastated,” Guardians manager Stephen Vogt said. “It's never fun to deliver that news to anyone, let alone somebody like Scott Barlow. Just after where we were coming out of last night's game and looking forward to this next week, we needed to make a move. And unfortunately, sometimes the business in this game is cruel.”

Gose went 5-4 with a 3.27 ERA and 10 saves for Columbus. He made two appearances for the Guardians this season and allowed six hits and three runs in 3 1/3 innings.

Sept. 6: RHP (blister) won’t start Saturday
The temperatures in Los Angeles aren’t the only thing blistering this weekend, with the heat convincing the Guardians to skip Cobb for one full turn in the rotation. Cobb had a small bister surface on the inside of the middle finger on his throwing hand when he faced the Pirates on Sunday. Gavin Williams will now start against the Dodgers on Saturday, while Tanner Bibee moves into Williams’ spot on Sunday.

RHP (hip strain)
Expected return: Mid-September
Carrasco threw a simulated game on Aug. 28 and continues to be built back up from his hip strain. He sustained the injury in his start on Aug. 7 against the D-backs. In 21 starts this season, Carrasco is 3-10 with a 5.64 ERA.

RHP (right shoulder fatigue)

Karinchak was sent on a rehab assignment to Triple-A Columbus after touching as high as 93 mph while pitching at the club’s facility in Goodyear, Ariz. Manager Steven Vogt said there remains a possibility that Karinchak could pitch for the Guardians before the regular season ends. Karinchak reported to Spring Training with right shoulder fatigue and has been working his way back ever since. (Last updated: Sept. 6)

Espino underwent surgery to repair his right shoulder capsule and rotator cuff injuries on March 13. He's expected to stay in Goodyear, Ariz., to work through his rehab. Although the Guardians don't have a firm timetable on when he may return to action, the team does not expect him to pitch during the 2024 season. (Last updated: May 15)

Bieber was transferred to the 60-day IL on April 10, and there's no return in sight for Cleveland's ace. He wasn't bouncing back as well as he is used to after his Opening Day start and elbow pain persisted throughout his outing in Seattle.

Bieber underwent imaging that revealed the same problems with his UCL that he had last year. He underwent UCL surgery on April 12. (Last updated: April 13)

Expected return: 2025
When Stephan's elbow was examined for discomfort on Feb. 28, a deep bone bruise was discovered. But after it continued to nag him, other opinions were needed. It was determined that his UCL is not providing adequate stability, which caused even more pain in the elbow.

Stephan underwent surgery on March 27 and was given a 12 to 16 month timetable for recovery. (Last updated: March 28)
