The Cardiovascular and Interventional Radiological Society of Europe (CIRSE) is a non-profit making, educational and scientific association aiming to improve patient care through the support of teaching, science, research and clinical practice in the field of cardiovascular and interventional radiology. CIRSE’s mission is to provide continuing education and training for its members, as well as other physicians and scientists with an active personal involvement and interest in interventional radiology and to promote the exchange of ideas and information for the further definition of the role, direction and goals of cardiovascular and interventional radiology as a subspecialty of radiology. CIRSE aims to improve ethical, technical and material conditions in cardiovascular and interventional radiology to enhance patient care.

For today’s #ClinicalIRTuesday, Marco Calandri shares some thoughts on his presentation in the upcoming focus session at #CIRSE2024, "Communication with the patient." "Interventional procedures are at the core of our work. We know how to perform them, how to manage complications, and how often they occur. Our patients, however, may not. Sometimes, they don’t even know who we are or what interventional radiology entails. For this reason, we need to dedicate more time to our patients—to establish a strong connection with them before, during, and after the procedure. In our institution, a crucial moment for the success of the treatment is the ultrasound planning visit, where we strive to build a genuine connection with our patients by explaining the pros and cons of percutaneous treatment. Even just before the procedure and anaesthesia, a calming approach is essential for ensuring a positive experience for the patient. Finally, it’s important that patients don’t feel abandoned or handed off to another physician after the procedure. Taking the time to explain the results of our treatments and making patients aware of our clear follow-up pathway dedicated to them is the best way to ensure they receive long-lasting benefits and retain a positive memory of our specialty." #CIRSEtakeover with Dr. Joo-Young Chun The last interesting case of this week! A 78-year-old diabetic man presented with worsening rest pain and a new ulcer in his left foot. Angiograms show a long and heavily calcified SFA occlusion and diffuse tibial disease. The SFA occlusion required retrograde recanalization via the dorsalis pedis and SAFARI technique. The occlusion was pre-dilated with a 3mm plain balloon followed by a 7mm Shockwave IVL and drug-eluting balloon angioplasty. Completion angiogram showed a widely patent SFA with single-vessel runoff to the foot. We’re treating more patients with diabetes, CKD on renal replacement therapy and generally more advanced PAD. As a result, calcium is becoming an increasing challenge, especially in patients with CLTI. Intravascular lithotripsy has been used successfully in cardiology for calcified coronary arteries and the technology has recently taken hold in PAD. There is still a paucity of evidence in PAD, and we need some good-quality outcome data on how effective IVL is in real-life CLTI patients. The CALCIO study is a Europe-wide, prospective, observational cohort study sponsored by CIRSE’s research arm Next Research. The study aims to recruit 400 patients over 2 years, followed by a period of 2-year follow-up. And recruitment is now open! What I like about the study design: •Real-life patients meet the inclusion criteria – All adult patients with CLTI and calcified femoropopliteal or crural lesions who are treated with IVL. This means the study findings will be relevant to the patient population we treat day-to-day. •Main study outcomes are wound healing, major amputation, and amputation-free survival - outcomes that have a clinical impact on our patients. •Patient reported quality of life assessment – this is quite unique and gives the patients a say in how much difference the treatment has made to their lives. I am very proud to be the Principal Investigator at St George's University Hospitals NHS Foundation Trust and excited to take part in this study. We had the green light to start recruiting this week!
