    doing the same things repeatedly will lead to different results.

    possibility to consider a different…

    The COVID-19 pandemic in its second year has shown that current systems are vastly
    challenged by the complexity and intersectoral affects around the globe. Quick fixes and problemsolving in a fractioned, comparted way do no longer serve. And still, there seem to be the notion that
    doing the same things repeatedly will lead to different results.
    This is to review and appreciate the efforts so far in Pandemic Preparedness and to highlight the
    possibility to consider a different, system theory approach – the concept of social change 2.0 of David
    Gershon. As the pandemic progresses into various phases, the complexity for sectors, systems and
    countries worldwide is unprecedented. Various documents and even more assessments were put together to identify best practices and gaps were scheduled to support the governments. The assessment reports revealed that all were using differnt measurements. While the focus was on identifying gaps, little is available on the progress made and where potential overlaps might occur. Nethertheless, there is a lot of good intention and a huge amount of efforts and resources being put onto these assessments. Interoperability, connecting the dots seemed to be superseded by an intersectional activism where different entities need to leave a footprint with single-sided solutions instead of a collaborative (behavior driven), systematic, holistic and integrated approach in form of integrated programs.
    In conclusion, the huge efforts of everyone involved in pandemic preparedness and management,
    particularly of the frontline workers, are acknowledged. Drawing from various assessments’ best
    practices and lessons learnt, fractioned approaches continue to occur and do suggest a mindset
    change to drop the assumption that insufficient structures and systems can serve as solid baseline.
    Identifying gaps is no longer enough. Pandemic preparedness is suggested to be an integrated part of
    systems theory, in particular the social change 2.0 concept.

    Background. Nonalcoholic fatty liver disease is now acknowledged as a complex public health issue linked to sedentary lifestyle, obesity, and related disorders like type 2 diabetes and metabolic syndrome. Aims. We aimed to retrieve its trends out of the huge amount of published data. Therefore, we conducted an extensive literature search to identify possible biomarker and/or biomarker combinations by retrospectively assessing and evaluating common and novel biomarkers to predict progression and prognosis of obesity related liver diseases. Methodology. We analyzed finally 62 articles accounting for 157 cohorts and 45,288 subjects. Results. Despite the various approaches, most cohorts were considerably small and rarely comparable. Also, we found that the same standard parameters were measured rather than novel biomarkers. Diagnostics approaches appeared incomparable. Conclusions. Further collaborative investigations on harmonizing ways of data acquisition and identifying such biomarkers for clinical use are necessary to yield sufficient significant results of potential biomarkers.

    We analyzed retrospectively 21 immune-competent travelers with chronic traveler’s diarrhea (3–6 weeks) after returning from recreational travel to the tropics with stool samples positive for microsporidia. Nine patients had been treated with albendazole and 12 patients had been treated symptomatically. Diarrhea resolved in 8 of 9 and 12 of 12 patients, respectively. In the albendazole group, Encephalitozoon intestinalis was cleared in 4 of 4 patients and Enterocytozoon bieneusi persisted in 7of…

In the symptomatic treated group microsporidia persisted in stool samples of all patients. We conclude that there is only a transient correlation between detection of microsporidia in stool and gastrointestinal symptoms, and suggest that microsporidia infection may cause clinical symptoms during the early stages of infection that resolve even though the microsporidia may persist.
