  • Accentuate the positives, but don’t necessarily eliminate the negatives: A cross-sectional survey of junior doctor psychiatry terms

    mededpublish.org

    Abstract
Objective: To determine the experience of prevocational junior doctors during psychiatry terms in New South Wales, Australia, and the impact on career intention and perception of psychiatry.

Method: A cross-sectional mixed-methods survey was sent to 197 prevocational junior doctors (PGY1-PGY2+) undertaking a psychiatry or drug and alcohol term across New South Wales in 2015 – 2016, and referred here as psychiatry terms.

Results: Seventy-seven junior doctors (39% response rate) participated. The psychiatry term was rated as a positive experience by 82% (n=63), with several key influences identified. The proportion of respondents interested in a psychiatry career increased by 19%, pre-term to post-term (p=0.004). Qualitative data point to improved perceptions of psychiatry post-term due to improved mental health literacy, applicability of skills across vocations, and psychiatrist role models. Challenging clinical situations were considered positive influences if managed effectively by senior staff. Poorer perceptions of psychiatry post-term were rare.

Conclusions: Our findings suggest that both career intention and overall perceptions of psychiatry are influenced by factors specific to the junior doctor role (as opposed to medical student). In addition, exposure to effective management and good supervision of challenging situations has a positive influence on some junior doctors.

Objective: In Australia and internationally, psychiatry has struggled to fill training places to keep up with demand for service. The objective of this study was to review the components of psychiatry terms and placements that determine a positive experience and potentially influence interest in vocational training in psychiatry.
Method: A literature review and narrative synthesis was undertaken on 20 papers identified as meeting inclusion criteria.
Results: The top themes contributing to positive experiences during the psychiatry term were: receiving high quality supervision; supported autonomy; and witnessing patient recovery. There was a paucity of Australian literature preventing investigation of the Australian context alone.
Conclusions: There is a need to better understand how the junior doctor and medical student psychiatry experience influences perceptions of psychiatry and intention to specialise, especially in the Australian context.

By applying careful attention to the clarification of scope with key stakeholders and addressing key pain-points in the system we were able to develop a course with credible content and experts and targeted learning methodology that was able to market itself. The course pilot and first course run have been delivered on time and budget and have already demonstrated significant improvements in participants confidence in relation to key managerial roles, in particular people management, financial management and difficult conversations.

This conference addressed 3 recommendations in relation to collaboration from the Medical Portfolio Programs Review Report.

By sharing training skills and knowledge and exploring innovation and new technologies, the conference aimed to build on improvements to healthcare across NSW and ACT.

NSWMET engage over 350 stakeholders from across the continuum of medical education, those involved in undergraduate training, prevocational training and vocational training, with an aim of improved coordination and integration of curriculum to ensure a medical workforce that provides person centred evidence based care.

A pleasing outcome of the Conference was the decision by a number of the organizations that attended to partner on future medical education related projects.
