    Abstract: Blastocystis hominis is a common protozoan in the human intestinal tract that can cause anorexia, diarrhoea and abdominal pain. Ulcerative colitis is a chronic inflammatory bowel disease that, when active, may present with similar signs and symptoms to a Blastocystis infection.     Abstract: Blastocystis hominis is a common protozoan in the human intestinal tract that can cause anorexia, diarrhoea and abdominal pain. Ulcerative colitis is a chronic inflammatory bowel disease that, when active, may present with similar signs and symptoms to a Blastocystis infection. A patient with ulcerative colitis presented with anorexia, increased urgency and frequency of defecation. He had watery, bloody stools with mucus and left-sided lower abdominal pain. Further testing was required to identify the cause of symptoms. A stool test detected Blastocystis hominis and blood tests indicated inflammatory markers were within range, ruling out an ulcerative colitis flare. Treatment included the use of metronidazole, Saccharomyces boulardii, Lactobacillus rhamnosus (HN001), Bifidobacterium animalis spp. lactis BB-12, Lactobacillus acidophilus LA-5 and anti-parasitic herbs and was successful in resolving all digestive symptoms. Taking an integrated approach with antibiotic therapy, probiotics and anti-parasitic herbs may be an effective treatment for eradication of Blastocystis hominis in the symptomatic patient.
