Le #CHC ou carcinome hépatocellulaire est la tumeur primaire la plus fréquente du foie. Chaque année, plus de 10 000 nouveaux cas sont diagnostiqués en France, et près d’un million dans le monde. Les causes fréquentes incluent les hépatites B et C, l’intoxication alcoolique et la #MASH. Le diabète et l’obésité sont également des facteurs de risque. 🔎 Mais comment pouvons-nous mieux identifier les patients à haut risque ? Un algorithme d’apprentissage automatique ou machine learning, baptisé SMART-CHC, a été développé pour répondre à cette question. Basé sur des éléments clinico-biologiques et la mesure de la rigidité hépatique avec #FibroScan®, ce score permet de prédire et stratifier avec précision le risque de CHC chez les patients atteints de diverses maladies hépatiques chroniques, notamment les hépatites B/C évolutives et la stéatohépatite non alcoolique (MASH). Il aide ainsi les cliniciens à sélectionner les patients nécessitant un dépistage échographique du CHC et rassure ceux à faible risque. Chaque année en France, environ 8200 cas de cancer du foie sont diagnostiqués, avec une prédominance chez les hommes. Selon les chercheurs du Centre international de recherche sur le cancer (CIRC), cette maladie est en constante progression et pourrait augmenter de plus de 55 % d’ici 2040. Les facteurs responsables incluent la hausse de l’obésité, du diabète de type 2 et la persistance de causes classiques telles que l’alcoolisme et les hépatites B ou C. 🔍 Un diagnostic souvent tardif : Le cancer du foie reste longtemps silencieux, ce qui entraîne souvent un diagnostic à un stade avancé. Il est donc crucial de sensibiliser et de mettre en œuvre des stratégies de dépistage précoce pour améliorer les chances de traitement et de survie. Pour diagnostiquer une maladie du foie et en évaluer sa gravité, l’élastographie avec l’appareil #FibroScan® est utilisée afin de mesurer l’élasticité du foie. Le Pr Yves Menu explique que cette technique permet de renseigner rapidement sur la présence éventuelle d’une fibrose hépatique et d’une surcharge en graisse du foie. Or une fibrose pouvant évoluer en cirrhose puis en cancer avec le temps, les patients chez qui on la détecte feront l’objet d’un suivi régulier.
