In patients with STEMI transferred for primary PCI, pretreatment with P2Y12 inhibitors significantly reduced the risk of 30-day major adverse cardiac events (MACE) without increasing bleeding risk, particularly when administered more than 80 minutes before PCI. This suggests that early administration of P2Y12 inhibitors can provide a superior net clinical benefit compared to in-cath lab administration.
