The Minister of State with responsibility for the National Drugs Strategy, Colm Burke TD, visited the Substance Misuse Team in Waterford recently. The team provides a range of free and confidential services in Waterford City and County for anyone affected by drug or alcohol misuse. Their mission is to promote wellness and recovery while improving the overall quality of life for individuals and their families struggling with substance misuse. The team oversee a Drugs Outreach and Community-Based Initiative where workers provide harm reduction education, relapse-prevention and family support. This includes a Needle and Syringe programme, individual and group counselling, and support to individuals to engage in community-based detoxification programmes. The Patient Flow Academy Excellence Awards are now open for applications. Together with the Spark Innovation Programme, we are excited to celebrate projects across #OurHealthService that have improved patient flow. This initiative seeks to recognise and celebrate projects completed over the last five years which have improved patient flow. This initiative offers awards of up to €5,000. We are particularly interested in projects related to the following priority areas: - Integrated Operations; - Demand and Capacity Management/Analysis; - Ward Processes; - Integrated Discharge Processes; - Older Persons and Frailty; and - Length of Stay Reduction. We also welcome applications for other projects related to patient flow. The application deadline is 30th September COB. Projects awarded funding will be invited to the HSE Patient Flow Academy conference on 13th November. Frank O'Connell, who was diagnosed with COPD nine years ago, shared his experience of virtual care in recent weeks. Frank is one of the patients in the West of Ireland who are now able to access medical care from home as part of a virtual healthcare initiative rolled out by Galway University Hospitals and Community Healthcare West. Since the introduction of the service in April, 30 episodes of care have been delivered via the pathway, saving 205.4 bed days with the average length of stay in virtual care now at 7.6 days, a 35.5% improvement on the length of stay compared to the inpatient national average. Opportunistic Case Finding Programme Since its launch in January 2022, the Opportunistic Case Finding Programme has seen GPs assess nearly 200,000 patients at risk of cardiovascular disease or diabetes by May 2024. This report findings show that: 105,569 patients were confirmed at high risk and enrolled in the HSE’s chronic disease prevention programme. 6,331 patients were newly diagnosed with chronic diseases and enrolled in the HSE’s chronic disease treatment programme. Common risk factors prompting assessments were hypertension, dyslipidaemia (abnormal levels of fats in the blood), obesity, smoking, and chronic kidney disease. While cholesterol levels were generally normal, high triglycerides were noted. A substantial number of patients were newly diagnosed with prediabetes or diabetes and placed in appropriate programs. A significant finding is the undiagnosed chronic kidney disease: 26,344 patients had moderate chronic kidney disease (stage 3). 1,917 patients had serious kidney disease (stages 4 or 5). These results highlight the programme's effectiveness in identifying at-risk patients and managing their conditions and the need to expand the Chronic Disease Management (CDM) Programme to include kidney disease. Recent, positive research into Alzheimer's disease has been carried out in Tallaght University Hospital. The Institute of Memory and Cognition looked at the effectiveness of a new blood test in identifying the presence of Alzheimer’s disease. The use of blood tests has clear advantages over the current lumbar puncture. They are less invasive and more straightforward for patients, easier to scale up and are less costly.
