The Boys fans were left ‘sick to their stomaches’ after the Amazon Prime series delivered the most ‘unhinged episode in television history.’

The superhero show came back with a bang and the first three episodes of season four shocked as viewers were presented with Tilda Swinton playing an Octopus that has sex with a human, a graphic sex scene that referenced The Human Centipede and a close-up bum shot

But despite the stiff competition, there was one scene in particular that disgusted fans and had them reaching for their remotes – a gruesome and graphic lobotomy.

Sister Sage (Susan Heyward) performs the extreme operation on herself using an icepick to dumb down her super smart brain, explaining why she fell for the misogynistic Deep (Chace Crawford), who would not have been her former self’s type.

Disturbed fans flocked to X to share their utter shock over Sister Sage’s decision, with @Ben74450057 declaring: ‘That lobotomy scene is one of the grossest [things] I have ever seen on TV like why tf did they film it up close like that.’

Echoing their sentiment, @negressvelaryon said: ‘NAH WATCHING DEEP GIVE SAGE A LOBOTOMY WAS SOOOOOOO….. I couldn’t even watch i had to cover my eyes but the sounds????? #theboys.’

@eridoof added: ‘Just watched that #theboys lobotomy scene im sick to my stomach.’

Susan recently spoke about her character Sister Sage’s decision to lobotomise herself, telling Variety: ‘[Showrunner Eric] Kripke and I talked a lot about Sage’s sense of isolation. How her brain is a blessing and a curse because she’s trapped among all this information all the time and she can never really escape and just be.

‘And so I think [the risk of losing her eye with a lobotomy] is worth it, for her sanity, because if she doesn’t get a break from her brain, she’s gonna go even crazier than she already is.’

In the episode, fans were also captivated by the storyline focusing on Homelander going back to the lab where he was raised with a Fudgie the Whale ice cream cake for the lab techs – sounds cute right? Wrong.

Over the course of the episode, the superhero terrorises the lab technicians who experimented on him as a child with psychological torture.

He forces one worker to incinerate himself in an oven and later lasers a hole into another – after forcing him to pleasure himself in front of his colleagues. 

He then eviscerates the rest of the lab techs and locks the lone survivor in a chamber with their bloodied remains – seemingly to die in there.

Homelander then steps into a lift, covered in blood and guts, and smiles to himself for seeking revenge on the people who raised him.

Fans loved this gory episode and insist that this sinister performance needs to earn Antony all the awards as Homelander is now definitely one of the worst TV villains of all time.

‘This might be one of the most unhinged episodes in television history. Antony Starr is the real star of this episode. What a performance.#TheBoys,’ said Scenesoftheboys.

‘Antony Starr as Homelander is the best villain in television just give him his awards already,’ said Azirascrowley.

‘Homelander has joined the best villains of all time now #TheBoys, said Bluerayangel.

‘After this episode, I can say that…Homelander is The best villain on the planet and The best villain in the history of television…#TheBoys,’  said AbOoD0GOT.

The episode also showed a duet between two of the main superheroes, and a scathing commentary about abortion confidentiality.

The episode was shocking and gory but earned sincere praise from fans who were amazed that the creators were once again able to blend such serious issues with comic genius and depravity.

Fans had best enjoy the show while they can as it was announced that The Boys would be coming to a close after season five.

This means there are just around 12 new episodes left to wrap up this incredible show.

The Boys focuses on superheroes in the real world and a main hero called Homelander (played by Antony Starr) who is completely evil and devoid of love because he was a lab experiment that was tested on for his entire life.
