DementiaReady by Montefiore is here to help by providing personalised post-diagnostic support for all types and stages of dementia.

 

Our aim is to support you or your loved one to feel empowered to live at home, engage with the community, and maintain independence after diagnosis.

 

The DementiaReady service is available to anyone who: 

  • Has been diagnosed with a form of dementia by a qualified health professional
  • Is responsible for caring for someone living with dementia
  • Lives at home in Sydney or surrounding areas
  • Is willing and able to engage with the service

Our Services

DementiaReady by Montefiore

The DementiaReady service is tailored to your individual needs living with or supporting a person with dementia. Specifically, we offer: 

  • In-home assessment with our experienced Dementia Consultant
  • Personalised strategies to support you or your loved one to stay at home longer and maintain independence
  • Connection to other community resources and support services
  • What to ask your GP about your or your loved one’s diagnosis and treatment
  • Support with future planning
  • Guidance throughout your loved one’s unique journey of living with dementia 

DementiaReady offers these services via three different options: coaching, consulting, and COPE. 

Coaching

This is an entry-level service if you are seeking general advice and guidance. At DementiaReady, we understand that people may be at different levels of readiness when it comes to receiving information and accessing support. Perhaps you or a loved one have received a diagnosis, but not everyone is ready to talk about it yet. Or you have preliminary concerns about yourself, a family member, relative, neighbour or friend, and you are unsure what to do next.

This service can be offered in-person or via telehealth.

Consulting

This is a consulting-based option where our Dementia Consultants will meet with the person living with dementia, and a support person of choice, in your home. Our Dementia Consultants will complete holistic assessments to better understand the person, and their abilities, day-to-day routine, home environment, and support network. Our Dementia Consultants will also guide a discussion around specific areas you are experiencing difficulty with and explore strength-focused strategies to address these.

After the visit, you will receive a report with recommendations and strategies tailored specifically to you. We’ll go through these with you and coach, model and/or demonstrate the strategies so that you feel confident in actioning the recommendations and strategies.

Reaching out for support, unpacking your journey and experiences, and then taking on information and learning new approaches can feel daunting, and it takes time. For this reason, DementiaReady is not a one-off service. Our DementiaReady consultants will ensure that you feel you have the support and tools you need.

COPE

COPE stands for Care of People with Dementia in their Environments. It is an Occupational Therapy and Nursing intervention designed to enhance the wellbeing and quality of life of both the person living with dementia and their care-partner over five structured sessions (including the Initial Consultation). During these five sessions, you can expect: 

  • A comprehensive assessment of the person living with dementia, the care-partner, and the home environment.
  • A structured, tailored intervention period addressing key concerns raised.
  • A generalisation phase, which helps to adapt the strategies learned for the future. 

Please note that an initial assessment is required before commencing COPE Program.

Francesca Glamorgan is an experienced Registered Nurse with a passion for Dementia Care. Francesca has worked in aged care for over 15 years in multiple roles, including as a Director of Care & Services.

Amy Sender

Amy Sender is a dedicated and passionate Occupational Therapist. In 2020, Amy completed her Masters of Dementia. Amy has been working for Montefiore and with people living with dementia since 2017.

Frequently Asked Questions

You do not need a referral for the DementiaReady service. You do need to be diagnosed with a form of dementia by a qualified health professional or be providing care at home to someone diagnosed with a form of dementia by a qualified health professional.

There is a fee for service. If you are receiving a Home Care Package from the government, you may be able to use this package to fund the service.  

Depending on your eligibility, you may be able to access other funding options through private health and/or a GP Chronic Disease Management Plan. Please discuss this with one of our friendly Dementia Consultants.

The DementiaReady service is only available to people who are living at home with dementia.
