If you are seeking account information as a NYSLRS member, retiree or beneficiary, you will be required to provide personal information, such as your date of birth, NYSLRS ID, and the last four digits of your Social Security Number (SSN), to ensure we can locate your account information, and that we're sharing it with the right person. If you have a Power of Attorney (POA) on file with NYSLRS and you are completing the form on behalf of a member or retiree, fill out the fields under "Your Information" with the member's or retiree's information and include your name in the "Question" field with your question or request.
