ARLINGTON, VA — A number of facilities in Arlington recorded violations during recent inspections from the Arlington County Public Health Department. Inspectors conducted the most recent visits between March 14 and March 15.

In Virginia, all food service locations are supposed to be inspected at least once per year. Facilities with higher risk factors may be inspected as often as four times per year, according to the Virginia Department of Health.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected. Sometimes, the follow-up inspection only considers the most important "priority" violations.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.Depending on the results of the inspection, the health department can pursue several actions.

They can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or they can temporarily shut down an establishment until certain issues are solved.

The Virginia Department of Health only recommends restaurants have their permits suspended when "the continued operation of the food service presents a substantial and imminent threat to the public health."

  • Rodent burrows observed in hill behind dumpster corral. Corrective Actions: The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by using methods such as trapping devices or other methods as required.
  • Shelf under table with fryer is rusty and deteriorating. Corrective Actions: Maintain equipment in proper repair.
  • No cold water available at bar hand sink. Trap is disassembled under kitchen hand sink. Corrective Actions: Maintain plumbing systems in proper repair.
  • There is no air gap between spray hose nozzle and 3 part sink assembly. Corrective Actions: Provide an air gap with a diameter that is at least twice the diameter of the water supply inlet and not less that 1 inch for proper backflow prevention by the air gap.
  • Dumpster is missing drain plug. Corrective Actions: Provide a drain plug on dumpster.
  • Accumulation of grease observed on grease barrel. Corrective Actions: Refuse containers are to be cleaned at a frequency that prevents the build up of soil.
  • Floor tiles chipped and damaged in kitchen. Corrective Actions: Maintain physical facility in proper repair.
  • Exhaust fan cover is missing in ladies' restroom. Corrective Actions: Maintain ventilation systems in proper repair.

  • Small container of cooked fish nuggets (87 F) observed improperly stored at room temperature along the cook-line. Corrective Actions: Product placed under refrigeration by person in charge.
  • Chemical spray bottle containers in the dish-machine area observed in need of properly labeling with the common name. Corrective Actions: Properly label chemical containers with the product common name.
  • Proper stem thermometer for measuring internal food temperatures not observed. Corrective Actions: Provide and purchase proper stem thermometer, manager will order.
  • Several boxes / containers of food product and single-service items observed improperly stored on kitchen floor and in walk-in refrigerator / freezer units. Corrective Actions: Food shall be protected from contamination by storing the food: (1) In a clean, dry location; (2) Where it is not exposed to splash, dust, or other contamination; and (3) At least 6 inches above the floor.
  • Food preparation / dumpling making observed improperly taking place in back walk-in refrigerator / freezer dry storage area. Corrective Actions: Discontinue food preparation in back storage area.
  • Handsink basin along cook-line in need of cleaning. Corrective Actions: Properly clean plumbing fixtures.

  • Chicken on shelf above wok's at 111°F; reheat at or above 165°F and maintain above 135°F. Corrective Actions: Hot hold potentially hazardous food at 135°F or above to inhibit the growth of harmful bacteria.
  • Lo mein on shelf at 65°F and pooled eggs on chef's cart at room temp; keep refrigerated. Fried chicken, General Tso's chicken and tofu in the left prep refrigerator at 48-52°Fold holding at improper temperatures; adjust the unit or repair the unit to hold foods at or below 41°F. Corrective Actions: Cold hold potentially hazardous food at 41°F or below to inhibit the growth of harmful bacteria.
  • Mouse droppings observed on ledge of wall by the back door. Corrective Actions: Eliminate harborage conditions. Insects and other pests are capable of transmitting disease to man by contaminating food and food-contact surfaces.
  • Back door found open; screen door isn't tight fitting either (air gaps) keep door closed; screen doors shall be tight fitting. Corrective Actions: Protect the food establishment against the entry of insects and rodents by: 1. Filling or closing holes and other gaps along floors, walls, and ceilings; 2. Closed, tight fitting windows; and 3. Solid, self-closing, tight-fitting doors. Insects and rodents are vectors of disease-causing microorganisms which may be transmitted to humans by contamination of food and food-contact surfaces.
  • Some boxes of food stored on the floor. Corrective Actions: Elevate food storage onto approved shelving with minimum 6" legs or casters.
  • Knife stored in-between counter and prep refrigerator. Corrective Actions: Store in-use utensils or dispensing utensils in one of the following manners: 1) In the food with their handles above the top of the food and the container. 2) In food that is not potentially hazardous with their handles above the top of the food within containers or equipment that can be closed. 3) On a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food-contact surface of the food preparation table or cooking equipment are cleaned and sanitized every 4 hours. 4) In running water of sufficient velocity to flush particulates to the drain. 5) In a clean, protected location if the utensils are used only with a food that is not potentially hazardous. 6) In a container of water if the water is maintained at a temperature of at least 135°F and the container.
  • Walk-in refrigerator shelves, prep refrigerator shelves/door gaskets/door panels/interior lids, walk-in refrigerator condenser fan cover, chef's cart and chef's spice containers aren't kept clean. Corrective Actions: Maintain non-food-contact surfaces of equipment clean.
  • Mop sink hose found in sink basin; faucet has no back flow prevention. Men's rest-room urinal bagged; repair or remove. Corrective Actions: Install an approved backflow prevention device or alter the length of the hose to provide the necessary separation between the water supply and the flood rim level of the sink basin. The minimum allowable separation distance must be at least 2x the diameter of the water supply inlet and at least 1 inch. Plumbing shall be in good repair.
  • Grease & cardboard dumpsters outside aren't kept covered; keep lids closed. Corrective Actions: Cover all waste containers when not in continuous use.
  • There are some water damaged ceiling tiles in the storage area; repair/fix leak and replace the damaged ceiling tiles. There is missing grout in-between some floor tiles inside the walk-in refrigerator and just outside the walk-in refrigerator with standing water water. Back of building downspouts are broken. S are broken. Some peeling paint and damaged wood on exterior of building. Corrective Actions: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.
  • Rest-room doors, light switches and some walls aren't kept clean. Corrective Actions: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food. A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner.

Corrective Actions: Obtain a quat-sanitizer test kit so that employees can accurately monitor the concentration of the sanitizing solutions and maintain them at a safe and effective level.

Corrective Actions: Cool time/temperature control for safety (TCS) food(s) from 135°Fto 41°F within a total of 6 hours and from 135°F to 70°F within 2 hours. A longer time for cooling allows an ideal situation for bacterial growth. This has been shown to be the major contributing factor in many foodborne illnesses. The person in charge discarded the chicken.
  • Observed the following ready-to-eat, potentially hazardous food being held for more than 7 days at 41°F: Pasta dated 2/25, diced tomato dated 3/6. Corrective Actions: Discard the food at this time and ensure all prepared RTE (TCS) food is served, sold or discarded within 7 days. PIC discarded items.
  • Observed that the establishment is vacuum sealing several items, including deli meats and cheeses, and steaks. The establishment does not have an approved HACCP plan for the food that is packaged using the reduced oxygen packaging (ROP) process. Corrective Actions: Cease the service or sale of ROP food. An approved HACCP plan must be in place prior to packaging food using the ROP processing method. Instructed PIC to discard deli meats sealed under ROP, and immediately open frozen ROP items.
  • Wiping cloths improperly stored on shelf by griddle between use. Corrective Actions: Ensure wet wiping cloths are stored in a chemical sanitizer at the proper concentration between use.
  • Observed that walls behind cookline were in need of cleaning. Corrective Actions: Clean walls frequently to prevent buildup of debris.
