Fredericksburg Restaurant Inspections: Kabob Corner, Castiglia's, Great American Buffet

Several restaurants around the Fredericksburg area were cited with violations during recent visits from the local health department.

The Rappahannock Area Health District cited several restaurants around Spotsylvania, Stafford, and Fredericksburg. Some cited locations included Kabob Corner, Castiglia's, Great American Buffet, and the Gauntlet Golf Club.

FREDERICKSBURG, VA — The Rappahannock Area Health District has returned to conduct routine inspections after spending most of January offering virtual training sessions for local restaurants. During the most recent round of inspections, more than a dozen restaurants were cited with violations.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

Depending on the results of the inspection, the health department can pursue several actions. They can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or they can temporarily shut down an establishment until certain issues are solved.

  • Castiglia’s Italian Restaurant & Pizzeria, 324 William Street Fredericksburg, VA 22401
    • Date inspected: Feb. 2
    • Total violations: 6
      • This food establishment does not have an employee, with management and supervisory responsibility, that is a Certified Food Protection Manager
      • The interior surfaces of the small ice machine at the bar was observed soiled. Machine was cleaned during inspection.
      • Improper methods used to thaw ROP salmon and fish. The fish was thawed in the package. Fish was voluntarily discarded during inspection.
      • The interior of the dish machine was observed soiled. The machine was cleaned during inspection
      • The floor in front of the ice machine is damaged. Several of the ceramic tiles are missing or broken.
      • The ventilation hoods and filters above the cooks line in the kitchen were observed soiled with grime and debris.
  • Sheetz #346 Fredericksburg, 2001 Carl D. Silver Pkwy Fredericksburg, VA 22401
    • Date inspected: Feb. 2
    • Total violations: 3
      • The sides of the deep fryer cabinet was observed soiled.
      • The floors in the walk in cooler and freezer were observed soiled.
      • The filters of the deep fryer ventilation hood was observed soiled.
  • Horne's Restaurant, 25532 A P Hill Blvd Port Royal, VA 22535
    • Date inspected: Feb. 4
    • Total violations: 2
      • The ventilation heed is not sealed to adjoining ceiling and wall.
      • The ceiling above the cook's line was observed in poor repair. Light fixtures have been replaced and the location where the old fixtures have not been sealed.
  • Barnes & Noble Booksellers, Inc., 1220 Carl D. Silver Pkwy Fredericksburg, VA 22401
    • Date inspected: Feb. 4
    • Total violations: 3
      • Per discussion with the person in charge the blenders, steam pitchers and cutting board only get washed at the end of the day. EHS discussed cleaning and sanitizing procedures of TCS food contact surfaces with the person in chage.
      • Person in charge was unable to provide a thermometer for the high heat dish machine used in the facility.
      • Observed the upslash of the only coffee brewere in the facility was heavily soiled.
  • Subway, 525A Jefferson Davis Highway Fredericksburg, VA 22401
    • Date inspected: Feb. 7
    • Total violations: 2
      • Person in charge provided EHS with employee health policy but they were not signed. During the inspection the person in charge went over the policy with food employee and policy was signed was signed.
      • Observed a food employee working without a hair restraint. During the inspection the person in charge instructed employee to put on a hair restraint.
  • Kabob Corner, 1631 Carl D Silver Parkway Fredericksburg, VA 22401
    • Date inspected: Feb. 7
    • Total violations: 29
      • The person in charge has failed to ensure employees are complying with the requirements of the Regulations. Observed a lack of date marking, a lack of hand washing, improper cooling, and improper thawing.
      • A food employee was handling raw chicken then handled clean dishes without first washing their hands and donning gloves. Employee was asked to wash their hands.
      • Improper handwashing procedures observed. Employee washed hands for less than 10 seconds. Observation was discussed with the person in charge.
      • A food employee was observed cleaning their hands at the three compartment sink. Employee was instructed to wash their hands only at the handwashing sink.
      • No paper towels were provided at the handwashing sink in the kitchen. The person in charge provided paper towels during the inspection.
      • Both handwashing sinks did not have an handwashing sign. The person in charge provided hand washing signs.
      • Observed several spoiled jalapenos stored in a black container in the walk-in cooler. Spoiled Jalapenos were discarded.
      • Observed the following: 1- a metal pan of cut onions laying on top of an uncovered of cut lemons. 2- a fifty pound bag of salt that was open, ripped, and stored directly on the floor in the dry storage area. 3- two brown bags of flour stored directly on the floor that were opened. The food was either covered or discarded during the inspection.
      • Observed raw shelled eggs stored over ready to eat food and different types of raw animal foods stored in such a manner that may cause cross contamination as follows: Raw chicken and raw beef stored side by side on same pan. Items was relocated during inspection.
      • Observed several pairs of tongs, skewers, knives, and containers that were stored soiled to sight and touch. The items were removed to be washed, rinsed and sanitized.
      • A date marking system is not in proper use for the time/temperature control for safety (TCS) foods. Corrected during the inspection.
      • Observed a container of paint, a bottle of an all purpose cleaner, and a bottle of soap stored next to food and single serve articles on the shelving unit. The person in charge relocated the bottles.
      • Observed a large container of Chickpeas (65'F) stored in the walk-in cooler that measured to be improperly cooled. Employee stated the chickpeas was prepared the day prior in the evening and placed in the walk-in cooler overnight. All other TCS foods in the walk-in cooler measured appropriately.
      • Observed a large container of thawed out chicken 65'F that was left on the counter the prior night to thaw per the person in charge. The chicken was discarded.
      • Observed several flies throughout the facility.
      • Observed several boxes of spinach, lamb, and chicken stored directly on the floor in the walk-in freezer. The person in charge relocated the boxes.
      • Observed multiple employees conducted food preparation process wearing wrist jewelry. The jewelry was removed during the inspection.
      • Observed a sponge in the 3-compartment sink that is being used for in use food contact surfaces.
      • Observed single service containers were saved and re-used to store other food items. The person in charge discarded the items.
      • Observed several single service articles boxes stored directly on the floor in the dry storage area.
      • Observed two bags of raw chicken and one bag of shredded carrots stored directly in a non-food grade bags in the walk-in Freezer. The person in charge discarded the items.
      • Observed growth on the caulking of the 3-compartment sink.
      • Observed the hand washing sink in the kitchen and the 3 compartment sink area not sealed to the adjacent wall.
      • Observed tape being used to hold the gasket in place in the reach in refrigerator in the front of house.
      • Accumulations of debris were noted on the following non-food contact surfaces: stock pot burner, shelving units, gas range burners, stove, the interior of the reach in freezer and walk-in cooler.
      • Mops not hung up to air dry. The person in charge hung the mops up.
      • Observed the hood system and filters soiled with grime and debris
      • Observed 4.1 foot candles of light inside the walk-in cooler.
      • Observed several employees' personal belongings stored next to food and equipment throughout the facility. The person in charge relocated the items.
  • Great American Buffet, 1780 Carl D. Siver Parkway Fredericksburg, VA 22401
    • Date inspected: Feb. 8
    • Total violations: 12
      • Observed no handsoap available at the meat handwashing station. During the inspection the person in charge provided handsoap.
      • Observed no paper towels available at the meat station handwashing sink. During the inspection the person in charge provided paper towels.
      • The following food contact surfaces of equipment were observed soiled to sight and touch: The blade of the can opener, the meat slicer, the veggie slicer. During the inspection the can openener, meat slicer and veggie slicer were moved to the dishwashing area.
      • The following cold holding foods were observed out of temperature: One container of cottage cheese (45 degrees F), one container of macaroni salad (44-45 degrees F). Foods had been put out at the buffet line since 8:30am, person in charge could not say if foods had been refilled by 12:30 when EHS checked temperatues. Foods were voluntarily discarded for safety.
      • Observed the absence of thermometers in all of the cooling units of the facility.
      • Observed numerous plastic containers stored stacked while wet.
      • Observed the front handsink, dessert area handsink and mop sink are in need of recaulking.
      • Observed several heavily rusted shelves in the meat room, back prep area, fry line area and walk in coolers.
      • Observed the person in charge provided expired test strips for the chemical sanitizer solution.
      • The following non food contact surfaces were observed soiled: Shelves holding equipment and foods throughout the facility, upslash area of the floor mixer, inside and sides of the fryer cabinets, and the surrounding area of the dish machine.
      • Observed walls behind and around all handsinks, floors of all walk in coolers and freezer, ceiling vents throughout kitchen, and walls around frying station are in need of cleaning.
      • Observed several loose ceiling tiles in the dry storage area.
  • Gauntlet Golf Club, 18 Fairway Drive Fredericksburg, VA 22406
    • Date inspected: Feb. 8
    • Total violations: 15
      • The person in charge could not communicate how the facility will address clean-up of vomiting and diarrheal events. Discussed requirements and provided handout using bleach.
      • Observed 3 food thermometers soiled. The thermometers were removed to the warewashing area for cleaning.
      • Observed interior surfaces of the microwave oven soiled.
      • Observed the can opener attached to the prep table soiled. The can opener was removed to the 3 compartment sink to be cleaned and sanitized. The person in charge reported the can opener was not used since opening.
      • Observed tray of hot dogs (121 degrees F) hot holding at improper temperatures. The burned on the range was turned on. The person in charge reported the hot dogs were placed on the range for hot holding at 10:30 a.m. and temperature was observed at 12:30 p.m. The person in charge removed the hot dogs to take home for personal use.
      • The menu includes hamburgers, which may be cooked to order, without adequate disclosure of menu items which may be ordered raw and/or undercooked. Discussed including an asterisk next to the foods which may be cooked to order which is denoted to the reminder statement on the menu.
      • The person in charge could not provide a Nitrate/Nitrate sampling report conducted within the past 12 months. The last sampling report was dated 1/28/2021. Instructed the person in charge to provide Nitrate/Nitrite sampling results
      • Observed numerous wiping cloths held in Quaternary Ammonum sanitizer without any measurable concentration observed with testing kit. The person in charge removed the wiping cloths to be laundered.
      • Observed significant accumulation of ice on the interior surfaces of all chest freezers. Chest freezer #2 (downstairs) does not adequately close due to the accumulation of ice on the top of the unit.
      • Observed quaternary Ammonum test kits discolored. Instructed the person in charge to purchase new test kits.
      • Accumulation of debris was observed on the following nonfood contact surfaces: Interior fryer door, backsplash of the range, pan used to hold clean spatulas and base of the convection ovens. Instructed the person in charge to removed the spatulas for cleaning.
      • Observed the following in poor repair: Lose faucet at the bar handsink and the ice well drain tubing is being held with duct tape under the bar handsink.
      • Observed accumulation of debris/brush in the waste receptacle enclosure.
      • Observed ceiling tiles in the kitchen near the hood were absorbent and not smooth or easily cleanable.
      • The floors and ceiling light covers throughout the facility were noted in need of cleaning.
  • Miso Asian Grill and Sushi Bar, 1305 Emancipation Highway Fredericksburg, VA 22401
    • Date inspected: Feb.8
    • Total violations: 18
      • Observed food employee wash hands in less than 2 seconds. EHS asked food employee to rewash hands. Food employee washed hands in less than 5 seconds. EHS demonstrated handwashing procedure to food employee. Food employee rewashed hands using proper handwashing procedure.
      • Observed hand soap at the sushi bar dump sink. During inspection, PIC removed hand soap.
      • During inspection, observed a food delivery where TCS food was received at improper temperatures: Three packages of refrigerated lo mein noodles (57°F) and one packs of refrigerated wonton wrappers (52°F ). The PIC returned the TCS food to driver.
      • Observed the interior of the ice machine soiled.
      • Observed the following soiled to touch and sight: 1. Two knives on the magnetic strip next to the 3 compartment sink. 2. One dough scraper next to the dump sink in the sushi station. During the inspection, the items were relocated to the warewashing area
      • The following were observed at improper temperatures: 1. One gallon of milk (45°F) in the walk-in cooler. 2. One container of tempura sweet potato (51°F) in the 1 door prep unit that was prepared on 02/07/2022. During the inspection, the food employee discarded the food.
      • No thin probe thermometer.
      • Observed numerous wet wiping cloths throughout the kitchen.
      • The following in-use utensils were observed stored improperly during use: 1. One spatula in a container of water (70°F) behind the rice cookers. 2. One ice cream scoop in a container of water (56°F) on the server counter. 3. One small bowl in a large plastic container of General Tso chicken in the 1 door prep unit. During the inspection, the PIC and food employee removed the utensils.
      • The following were observed: 1. At the server station, a container of utensils with the food contact surfaces exposed to contamination. 2. At the take out station, a box of plastic forks stored with the food contact surface exposed to contamination.
      • The following were observed: 1. A plastic gallon container with the top portion cut being stored over the 3 compartment sink that is used as a funnel. 2. Two seaweed salad plastic containers in the walk-in cooler being reused to store cooked brown rice. During the inspection, the food employee discarded the plastic containers
      • The following are in need of repair: 1. The handle of RIC #1 is loose. 2. The microwave over the small RIC is missing the handle. 3. Ice buildup in chest freezer located in hallway. 4. In the women's restroom the faucet is loose.
      • The following are in need of repair and/or cleaning: 1. The caulking at the 3 compartment sink and dishwasher. 2. The bottom shelf of prep tables.
      • The interior of the dishwasher is heavily soiled.
      • The exterior surface of the cooking equipment on the cook line are in need of cleaning.
      • The floor under all equipment is in need of cleaning.
      • The following are in need of repair: 1. Several peeling ceiling tile throughout the kitchen. 2. The threshold into the storage room (broken walk-in cooler).
      • Observed employee personal items throughout the kitchen: 1. Cell phone on top of vegetable oil bottles and the prep table. 2. Personal water bottles on prep table, behind the microwave, on numerous shelves and in a box containing food.
  • Q-Ball Café, 960 Bragg Road Fredericksburg, VA 22407
    • Date inspected: Feb. 8
    • Total violations: 5
      • The food contact surfaces of the deep fryer baskets and the interior of the microwave oven was observed soiled. Both items were cleaned during inspection.
      • The Curing board along the prep unit and the cutting board stored under the prep table were observed stained. Both boards were taken to the 3 compartment sink to be cleaned.
      • The sides of the deep fryer cabinets and the interior and exterior of the reach in freezer were observed soiled.
      • The floors under the equipment in the kitchen were observed soiled.
      • The ventilation hood filters were observed soiled.
  • Dunkin, 150 Warrenton Road Fredericksburg, VA 22405
    • Date inspected: Feb. 8
    • Total violations: 8
      • Observed using single use disposable gloves being used for multiple tasks. Observed the following: (1) Food employee touch their face wearing single use disposable glove and then proceed to prepared beverages for a customers order without removing gloves and washing their hands and (2) Observed a food employee adjust their facemask wearing single use disposable gloves without removing gloves and washing their hands prior to engaging in food preparation tasks. Employees were instructed to remove gloves and wash their hands prior to donning new gloves.
      • Observed accumulation of debris on the upsplash of the coffee brewers and interior of the ice machine.
      • Through discussion with the person in charge the steamer wands on the espresso machines are cleaned and sanitized once daily. Informed the person in charge the steamer wands, which are in contact with TCS beverages are required to be washed and sanitized at least once every 24 hours.
      • Observed container of half and half (46 degrees F) and oat milk (45 degrees F) cold holding at improper temperatures in the 1 door reach in cooler #2. The person in charge reported the beverages were pulled from the walk in cooler at 7:00 a.m. and placed in the unit. The TCS beverages were removed to the walk in cooler and keg cooler to cool to 41 degrees F within 4 hours (11:00 a.m.). The temperature of the unit was lowered and observed at 40 degrees at the completion of the inspection.
      • Observed the following toxic materials inadequately stored to prevent contamination: (1) Wiping cloth bucket stored on top of the ice machine, (2) Two chemical spray bottles hanging on the wire shelving which also holds single use disposable articles (napkins, condiments), and (3) Espresso machine cleaner container stored on top of the espresso machine. The toxic chemicals were relocated to prevent contamination during the inspection.
      • Observed the drive thru window does not self-close when opened. Discussed installation of a self-closing door.
      • Observed the Chlorine test kit expired September 2021. Instructed the person in charge to purchase a new testing kit.
      • Accumulation of debris observed on the following nonfood contact surfaces: Base, shelving and exterior handles of the convection ovens and shelving at the TurboChef oven.
  • Subway, 8135 Kings Highway King George, VA 22485
    • Date inspected: Feb. 8
    • Total violations: 4
      • The hand washing sink in the kitchen was blocked, preventing it's use. Observed two tables in front of the sink. The person in charge said that they were only there during prep. I suggested that prep time was the time they needed to use the hand sink. The tables were removed during inspection.
      • Observed gaps along the bottom and side of the outer door in the kitchen.
      • The cutting board(s) along the make line are heavily scratched and scored. The food contact surface is no longer easily cleaned and sanitized due to condition.
      • The cooling fan in the ceiling above the walk in freezer was observed heavily soiled.
  • Hampton Inn, 4391 Hospitality Way King George, VA 22485
    • Date inspected: Feb. 8
    • Total violations: 3
      • The cavity of the microwave oven in the kitchen was observed soiled. Oven was cleaned during inspection.
      • The hand sink in the kitchen is not sealed to adjoining equipment or walls.
      • The drain plugs for the dish washing sinks are not tight fitting, allowing water to leak out of the sinks.
  • Minnieland #54, 1420 Central Park Blvd #110 Fredericksburg, VA 22401
    • Date inspected: Feb. 8
    • Total violations: 3
      • Observed several sheet trays heavily soiled with grease and grime. Person in charge stated new sheet trays have been ordered.
      • Observed several single service use containers being used as serving utensils stored inside the ceral [sic] bags.
      • Observed several ceiling tiles and air vents in the kitchen soiled.
