Fredericksburg Restaurant Inspections: Happy Endings, Sam's Pizza, Red Crab, Tapa Rio

More than two dozen locations around Fredericksburg were cited with violations during recent health department inspections.

FREDERICKSBURG, VA — Recent restaurant inspections by the Rappahannock Area Health District uncovered violations at more than two dozen locations around Fredericksburg and Stafford. The inspections were conducted between Dec. 15 and Dec. 21.

As coronavirus restrictions continue to fluctuate around the state, health inspectors are returning to conduct some restaurant inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

Depending on the results of the inspection, the health department can pursue several actions. They can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or they can temporarily shut down an establishment until certain issues are solved.

Here are the most recent violations as listed by the Rappahannock Area Health Department:

  • BJ’s Restaurants and Brewhouse, 1861 Carl D. Silver Parkway Fredericksburg, VA 22401
    • Date inspected: Dec. 15
    • Total violations: 10
      • Observed the kitchen chemical sanitizing dish washing machine was not dispensing sanitizer. Person in charge instructed staff to use the 3 compartment sink and a call was put in to a technician. Problem was resolved before leaving the facility, EHS varified 50ppm level of sanitizing chemical.
      • Observed numerous sheet trays and pizza pans (large and small) with heavy build up of encrusted grease.
      • Observed the interior surfaces of the microwaves in the facility were soiled.
      • Observed 3 containers of batter cold holding at improper temperatures (47, 47 and 50 degrees F). Per discussion with the person in charge, batters had been put in the unit since 10am. Person in charge discarded the batters for safety.
      • Observed two bottles of degreaser stored on a shelf above cleaned and sanitized equipment and dishes. During the inspection the person in charge re-located the chemicals.
      • Observed accumulation of ice on the air curtains of the walk in freezer.
      • The following non-food contact surfaces were observed soiled: The warming hood area above the pasta prep station is heavily encrusted with grime and grease, the inside surfaces and gaskets of the appetizer freezer.
      • Observed an accumulation of water on the floors of the bar area. In discussion with the bar staff the drain overflows at times because of the position of the condenstation draining lines.
      • Observed the following are in poor disrepair: Chipping flooring at the cooks line, damaged/loose coving tiles inside the walk in cooler near the entrance of the walk in freezer, threshold is missing at the entrance of the walk in cooler.
      • Observed the following: Floor drains and floors underneath equipment are in need of cleaning, AC vents are dusty and have mold like staining, several ceiling tiles throughout kitchen are stained.
  • Dickey’s Barbeque Pit #783, 10008 Southpoint Pkwy Ste 100 Fredericksburg, VA 22407
    • Date inspected: Dec. 15
    • Total violations: 4
      • The following physical facility items were noted in need of cleaning: 1-the walls in the kitchen, 2-the grout along the 3 compartment sink, 3-the floors in the kitchen.
      • The following non-food contact surfaces were observed soiled: 1-interior surfaces of the rear food warmer, 2-the gaskets on the rear food warmer, 3-the wire shelving in the kitchen, 4-inside if the deep fryer cabinets
      • Observed food employees without wearing beard guards.
      • The small tabletop grill on the line was observed heavily soiled. The grill was cleaned during insepction.
  • Best Western Aquia / Quantico, 2868 Jefferson Davis Highway Stafford, VA 22554
    • Date inspected: Dec. 15
    • Total violations: 9
      • The person in charge could not provide documentation of the employee health policy for two new food employees. The employees were knowledgeable of health reporting requirements. Ensure food employees are trained on the employee health policy prior to engaging in food preparation activities. Provided sample employee health policy in Spanish and English.
      • Through discussion with the person in charge, the food establishment is removing the dispensing nozzles for juices to be cleaning and sanitized once of twice a week. Discussed removing the nozzles once daily for cleaning and sanitizing. The person in charge agreed to comply.
      • Observed commercially prepared hard boiled eggs (44, 44 degrees F) cold holding at improper temperatures in the door of the Frigidare 1 door upright cooler. The person in charge voluntarily discarded the eggs for safety. The ambient air temperature was observed at 50 degrees F and TCS foods observed 41 degrees F or less were relocated to another refrigeration unit out of an abundance of caution. The temperature in the unit was lowered during the inspection. Discontinue using the unit to store TCS foods. Discussed contacting a technician to evaluate the cold holding capacity if the unti temperature does not lower to 41 degrees F or less. Recommended ambient air temperature of 38 degrees F or less.
      • Observed cabinet which holds pastries and bread products, which are available for customer self-service, are not labeled or otherwise indicated to identify the ingredients or potential allergens.
      • Observed the door leading into the breakfast area is not self-closing and tight sealing. The door needs to be pushed to completely close.
      • Observed food employees without proper hair restraints. Food employees prepare food and clean and sanitize food contact equipment.
      • The person in charge provided a Chlorine test kit which expired February 2021. Instructed the person in charge to purchase a new test kit to monitor the concentration of Chlorine sanitizer.
      • Observed accumulation of debris and equipment around the dumpster area.
      • Observed equipment not necessary for food service operations being stored in the storage room where refrigeration and dry storage of food is located.
  • Residence Inn, 60 Towne Centre Blvd. Fredericksburg, VA 22407
    • Date inspected: Dec. 15
    • Total violations: 2
      • There is no test kit for monitoring the concentration of the chemical sanitizing solution in the 3 compartment sink.
      • The mop sink, the floor under all equipment, and the wall around the gas range were observed soiled.
  • Sam's Pizza and Subs, 27 South Gateway Drive Suite #125 Fredericksburg, VA 22406
    • Date inspected: Dec. 15
    • Total violations: 6
      • Observed the following food contact surfaces soiled: 1- Observed debris on the bottom of the mixer bowl, used to prepare pizza dough prepared from Non-TCS ingredients. 2- three plastic food storage containers in side the top of the large prep cooler, and 3- one pitcher inside prep cooler #2. The food contact equipment with the exception of the mixer bowl was removed from cleaning. Instructed the person in charge to clean and sanitize the mixer bowl prior to use.
      • Observed some of the shelving inside the walk in cooler have become rusted an no longer have a smooth and easily cleanable surface. Discussed painting, sealing or replacing the shelving.
      • Accumulation of debris was observed on the following nonfood contact surfaces: Bottom of the legs of the dishwasher, inside the left side door of the large prep cooler, shelf under the flattop and upsplash of the pre-wash spray nozzle.
      • Observed the back left side of the oil storage container does not completely seal when closed. Observed the corner is bent which may attract pests. Discussed contacting their contractor for repair or replacement.
      • Observed the following in poor repair: Blown out ceiling lights, kitchen floor tile is in need of grouting, seal the men's toilet to the floor and ceiling tile above the soda syrup station is not sealed to the ceiling tile frames.
      • Observed 0.6 foot candles of light inside the walk in cooler.
  • Wendy's #876, 23890 Rogers Clark Blvd Ruther Glen, VA 22546
    • Date inspected: Dec. 16
    • Total violations: 19
      • Absence of active managerial control observed during the inspection. The person in charge is not monitoring temperatures of TCS foods during cold holding, monitoring temperatures of Quaternary Ammonium sanitizing solutions, and improper handwashing observed.
      • Observed a food employee handle soiled equipment in the wash basin of the 3 compartment sink wearing single use disposable gloves, remove gloves, and then proceed to don new disposable gloves to engage in food preparation tasks without washing their hands. Instructed the food employee to remove gloves and wash their hands.
      • Observed a food employee lather their hands under running water at the kitchen handsink. Hands were not effectively cleaned due to hand soap being flushed down the drain. Discussed correct handwashing procedures and instructed the food employee to wash their hands.
      • Observed six stainless steel containers of cooked bacon in hot holding steam wells uncovered or other wise protected from contamination. The person in charge placed a lid over the containers during the inspection.
      • Observed the interior surfaces of the microwave oven soiled.
      • Observed in use Quaternary Ammonium sanitizing solution in the 3 compartment sink at 64 degrees F and 200 ppm. Instructed the person in charge to drain the sanitizing solution. Worked with the person in charge to set up the sanitizer basin with Quaternary Ammoniunm sanitizing solution at 103 degrees F (tested at 75 degrees F) and 200 ppm.
      • Observed numerous and stacked individually prepared salads improperly cooled in the 1 door upright cooler. The person in charge reported the salads were prepared the day prior and held in the unit overnight. Packaged salads were observed at 45-48 degrees F. The person in charge voluntarily discarded the salads for safety. Discussed cooling of salads in the walk in cooler.
      • Observed the following TCS food cold holding at improper temperatures: Prep cooler (food held overnight in the unit): Sliced cheese (46 degrees F), cut lettuce (50 degrees F), spring mix (49 degrees F), cut lettuce in salad (49 degrees F), commercially prepared jalapeno cream cheese (46 degrees F) - the person in charge voluntarily discarded the foods for safety; Hamburger well cooler - Shell eggs (55 degrees F), raw hamburger patty (45 degrees F) - the person in charge reported the eggs and hamburger patties were pulled from the walk in cooler and placed in the unit at 10:00 a.m. Food items were relocated to the walk in cooler to cool to 41 degrees F. Observed shell eggs cooled to 44 degrees F and raw hamburger patties to 40 degrees by 12:00 p.m. ; Single use container of sour ream (48 degrees F) held in a unit in the front of the house which the person in charge was not sure if it was refrigerated. The person in charge voluntarily discarded the sour cream for safety, he could not communicate how long they were held in the unit.
      • Improper cooling methods observed for prepared salads. The facility is preparing numerous packaged salads and stacking them in the 1 door upright cooler to cool. Observed salads prepared the day prior at 45-48 degrees F. Discussed cooling of salads in the walk in cooler to facilitate rapid cooling.
      • Observed boxes of food stored on the floor of the walk in freezers and in the dry storage area.
      • Observed employees in the back of the house without proper hair and beard restraints.
      • Observed wiping cloths held in a bucket with no measurable concentration of Quaternary Ammonium sanitizing solution. The person in charge instructed a food employee to discard the solution and set up a new bucket.
      • Observed the male and female toilet room countertops, which the handwashing sinks are located, are not sealed to the adjacent walls.
      • Accumulation of grime and debris was noted on nonfood contact surfaces of all equipment including inside refrigeration equipment, inside fryers, cabinet under the dining room soda machine, upsplash of the dining room soda machine and shelving units. Discussed thorough cleaning of nonfood contact surfaces.
      • Observed significant accumulation of debris around the dumpster area. Observed what appears to be spilled and dried shortening on the ground of the waste enclosure. Discussed cleaning the area to prevent attraction of pests.
      • Observed the oil storage container and the recyclables dumpster doors open which may attract pests.
      • The floors, walls, ceiing tiles (in both the back of the house and dining room), air vents in dry storage area and plumbing in the warewashing area are in need of cleaning. Recommended a deep cleaning of the food establishment.
      • Observed the following in poor repair: Burned out ceiling lights, ceiling light does not work in the walk in freezer #2 (outside), floor tiles are in need of grouting, caulk toilet in the men's toilet room, and rusted air vent in the female toilet room.
      • Observed vent filters above the fryers soiled.
  • Country Inn & Suites, 5327 Jefferson Davis Highway Fredericksburg, VA 22408
    • Date inspected: Dec. 16
    • Total violations: 3
      • The milk (53'F) and the yogurt (54'F) in the display coolers on the serving line were observed cold holding at improper temperatures. One of the display units was a proper temperature (36'F), but the other was not (54'F). In talks with the person in charge it was discovered that the milk and the yogurt were stored overnight in reach homestyle reach in coolers in the basement. Coolers were inspected, and on on the coolers (which the milk and the yogurt was stored in) measured 57'F. Because it was not known how long the cooler was out of temperature, the milk abd yogurt in this unit and the display coolers on the serving line were voluntarily discarded.
      • There was no temperature measuring device located in the following cold holding equipment: one of the display reach in coolers on the serving line and the 3 homestyle reach in coolers used for storeage in the basement.
      • Observed a case of bananas and coffee stored on the floor in the kitchen. Food was placed on a cart during inspection.
  • Happy Endings Bar and Grill, 1315 Princess Anne St Fredericksburg, VA 22401
    • Date inspected: Dec. 16
    • Total violations: 14
      • The facility does not have a person that is a Certified Food Protection Manager,
      • Observed the same food employee do improper handwashing twice, they scrubbed their soapy hands under running water for less than 5 seconds. EHS discussed proper handwashing with the food employee and person in charge.
      • Observed a food employee handling prepared foods (a chicken wrap) without gloves. EHS explained bare hand contact and glove usage with the food employee.
      • Observed no soap was available at the hand washing sink of the bar area.
      • Observed no paper towels were available at the bar hand wash sink.
      • Observed the kitchen dish washing machine was not dispensing chemical sanitizer due to an empty container of sanitizer. During the inspection the person in charge provided a new sanitizer container and the dish washing machine was sanitizing at the 50ppm level.
      • Observed the inside surfaces of the bar ice machine soiled heavily with mold-like growth staining.
      • Observed a food employee prepare foods without a beard restraint.
      • The kitchen hand sink, 3 compartment sink and mop sink are no longer sealed to the adjoining wall.
      • Observed a loose gasket of the walk in cooler door and the door of the walk in cooler no longer seals on its own.
      • The person in charge was unable to provide test kit for the chemical sanitizing dish machine and test kit for the steramine tablets used for wiping cloth buckets and bar 3 compartment sink.
      • Observed the following: food debris and grime on the shelf holding plates and equipment, inside surfaces and gaskets of the cooling and freezer units are in need of cleaning.
      • Observed the walls and floors of the kitchen are in need of a deep and thorough cleaning.
      • Observed the floors of the walk in cooler are severely damaged and wall paint behind the prep unit and deep freezer is chipping away.
  • Funland of Fredericksburg, 1351 Central Park Blvd. Fredericksburg, VA 22401
    • Date inspected: Dec. 16
    • Total violations: 7
      • Observed the oil in the 2 fryers uncovered.
      • The food contact surfaces of the conveyor oven and the hot holding unit (next to the nacho cheese dispenser) were observed soiled.
      • The quaternary ammonium sanitizing solution at the 3 compartment sink in the party kitchen is reading 0 ppm. EHS recommended setting up sanitizer manually or using a chlorine sanitizing solution.
      • Observed ss articles (Pepsi and Icee paper cups, styrofoam plates, pre-cut foil and tissue) covered in construction debris on top of the hot holding unit and on a shelf above the fryer prep counter. During the inspection, the PIC discarded ss articles.
      • All non food contact surfaces in the cafe were observed covered in construction debris.
      • The following are in need of repair: 1. The walls throughout the cafe. 2. One ceiling tile missing between the hood system and beverage room. 3. The floors in the beverage room and the storage room next to the fryers.
      • The following are in need of cleaning: 1. The walls and floors throughout the cafe. 2. The wall behind the ice machine. 3. The ceiling vent between the reach in freezer and handwashing sink in the cafe. 4. The party kitchen mop sink
  • Renee’s Crepes and Cakes, 2020 Augustine Avenue Fredericksburg, VA 22401
    • Date inspected: Dec. 16
    • Total violations: 3
      • No certified food protection manager (CFPM) certificate available.\
      • Observed two 5 lb bags of shredded cheese (47 F and 48 F) that were at improper temperatures in the prep unit. During the inspection, the PIC removed the cheese from the establishment.
      • The ceiling over the ss article (take out containers) storage is not smooth and easy to clean.
  • Red Crab, 1801 Carl D. Silver Parkway Fredericksburg, VA 22401
    • Date inspected: Dec. 17
    • Total violations: 10
      • Observed a container of raw shrimp and two used food containers at several handwashing sinks of the kitchen. EHS explained to the staff and person in charge proper hand sink usage.
      • The following were observed: the surfaces of the ice machine are soiled with mold-like staining, the blade of the can opener was heavily soiled.
      • Facility is using time as a public health control for corn and two kinds of chicken wings without keeping track of time by labeling.
      • Observed an employees bottle of medication stored above a food prep area. During the inspection the person in charge relocated the medication.
      • Observed cardboard lining a kitchen shelf holding equipment.
      • Observed two kitchen handsinks and the 3 compartment sink that need resealing to the adjointing wall.
      • The person in charge was unable to provide a test kit for the chemical sanitizer of the bar 3 compartment sink.
      • Observed a leaking faucet at the 3 compartment sink.
      • Observed two loose/missing ceiling tiles at the dry storage area.
      • Observed the following are in need of cleaning:floors underneath equipment, AC vents and floor drains throughout the kitchen.
  • Nayeli Salvadorian & Mex Food, 20 Plantation Drive, #149 Fredericksburg, VA 22406
    • Date inspected: Dec. 17
    • Total violations: 7
      • The food establishment does not have a certified food protection manager, the owner's current certification expired November 2021.
      • Observed container of shell eggs store over box of green peppers in the 2 door upright cooler #1. The eggs were relocated to the bottom shelf to prevent cross contamination.
      • Observed improper cooling of in house prepared cole slaw (43, 43 degrees F). Observed the cole slaw stored in two deep and covered plastic containers in the 2 door upright cooler #2. Also observed condensation on the interior lid of one of the plastic containers. The person in charge reported the cole slaw was prepared on 12/14/21 and voluntarily discarded the cole slaw for safety. Discussed time and temperature ambient cooling requirements and cooling methods to achieve compliance.
      • Observed in house prepared cooked yucca, (prepared 12/14/21), pitcher of horchatas (prepared 12/14/21) and opened gallon of milk (opened 12/14/21) in refrigeration units which were not labeled to include the date of preparation/opening or expiration date. The person in charge placed date labels on the containers during the inspection. Discussed date marking requirements for in house prepared foods and commercially processed TCS foods.
      • Observed improper cooling methods to cool in house prepared cole slaw via ambient cooling. Observed the cole slaw in deep and covered plastic containers in the 2 door upright cooler #1. Discussed cooling cole slaw in smaller portions to facilitate ambient cooling. Also discussed pre-chilling pans in freezer units and replacing plastic containers with stainless steel to facilitate heat transfer.
      • Observed raw pork stored in direct contact with plastic shopping bag in the 1 door upright freezer. The person in charge voluntarily discarded the pork due to presence of significant freezer burn. Discontinue storing food in direct contact with plastic shopping bags which are not food grade or safe.
      • Observed the following in poor repair: Ceiling lights out in the toilet rooms, ceiling tiles in the female toilet room are not sealed to the frame, and scratches in ceiling tiles in the kitchen.
  • Holiday Inn Express, 15 Salisbury Drive Stafford, VA 22554
    • Date inspected: Dec. 17
    • Total violations: 15
      • Facilty does not have a certified food protection manager.
      • The facility does not have an employee health policy. EHS provided a copy for their use.
      • Per conversation with the kitchen employees, they are not sanitizing the equipment.
      • Observed that the interior of the ice machine requires cleaning.
      • Observed pork sausage hot holding at 117'F.
      • Observed a container of cream cheese that did not have the time indicated on the container.
      • Observed a chemical spray bottle that had 2 differnent labels on it. The kitchen attendants were unsure about the contents of the bottle. EHS requested that the spray bottle be discarded.
      • Observed boxes of oranges and waffle batter stored on the floor. Items were received yesterday.
      • Observed that the kitchen attendants are not wearing hair retraints. Both employees donned a proper restraint.
      • Observed boxes of single service items stored on the floor.
      • Observed that the sanitizer dispenser and the paper towel dispenser over the hand sink are in disrepair. Observed that the reach in freezer is in disrepair. All of the items inside of it has thawed.
      • Observed that there are no sanitizer test strips available.
      • Observed that the inside of the oven and the counters behind the equipment on the service line require cleaning.
      • Observed that the grout in the kitchen and floor under the equipment require cleaning.
      • Observed that the employee break area is in the kitchen where the foods are stored.
  • Dominos Pizza (Thornburg), 6374 Jefferson Davis Hwy 100 Spotsylvania Courthouse, VA 22551
    • Date inspected: Dec. 17
    • Total violations: 7
      • Observed that the employees are not aware of the illnesses concerning employee health.
      • Observed an employee chewing gum while preparing foods. The person in charge addressed this during the inspection.
      • Observed that the hand sink next to the 3-compartment sink is blocked with containers and cleaning equipment.
      • Observed plastic containers in the front prep area that were soiled to sight and touch. The person in charge placed the containers in the ware washing area.
      • Observed wet pizza pans and plastic containers in the front prep area.
      • Observed that the hood system, air vent covers, and underneath the floor-mounted equipment are in need of detailed cleaning.
  • Fraternal Order of Eagles #4488, 2055 Jefferson Davis Highway Stafford, VA 22554
    • Date inspected: Dec. 17
    • Total violations: 5
      • Observed package of raw bacon stored in a container with pre-cooked hot dogs. The bacon was removed to the lowest shelf during the inspection.
      • Observed dispensing utensils stored in a container with the food contact surfaces facing upward without adequate overhead protection. Discussed inverting the utensils to prevent contamination.
      • The person in charge could not provide a Quaternary Ammonium test kit (QT-10) to monitor Steramine sanitizer concentrations. The person in charge reported they are on order.
      • Observed water flows out of the plumbing vent when the kitchen handsink water is turned on. Contact a plumber for repair.
      • Absence of an air gap under any of the basins of the three compartment sink and observed the 3 compartment sinks are plumbed into a grease trap which is directly plumbed. During routine inspection conducted on 12/18/21 observed an air gap under the sanitizer basin of the three compartment sink. Instructed the person in charge to contact a plumber and the building department to install an air gap to prevent backflow.
  • El Jimador Azteca, 560 Celebrate Virginia Parkway Suite 109 Fredericksburg, VA 22406
    • Date inspected: Dec. 20
    • Total violations: 18
      • Observed absence of active ,managerial control in the food establishment to control risks to prevent transmission of foodborne illnesses. Observed the following: 1- improper glove use, 2- improper hot holding temperatures of TCS foods and 3- improper cooling and cooling methods to cool in house prepared TCS foods. Discussed requirements with the person in charge during the inspection.
      • The food establishment does not have an employee who is a certified food protection manager. The facility has a certificate posted indicating an employee has completed a certified food manager course but could not provide successful completion of the exam.
      • Observed the hot water at the handwashing sink by the beverage storage area at 66 degrees F. Instructed the person in charge to contact a plumber to restore hot water.
      • Observed a food employee place raw animal foods on the flattop grill using single use disposable gloves and then proceed to use the same gloves to handle cleaned dispensing utensils at the grill. Discussed the issue with the person in charge who instructed the food employee to remove gloves and wash their hands.
      • Observed the can opener soiled in the back of the house and through discussion with the person in charge, the facility is removing the iced tea and soda nozzles for cleaning twice a week. Instructed the person in charge to remove beverage nozzles daily and the can opener was removed to the dishwasher for cleaning.
      • Observed improper cooling of in house prepared cheese sauce in the walk in cooler. Observed the cheese sauce in a deep and covered plastic container holding at 45 degrees F. The person in charge reported the cheese sauce was prepared the evening prior and voluntarily discarded the cheese sauce for safety. Discussed methods of cooling to achieve compliance and provided cooling handout in Spanish.
      • Observed cooked ground beef (127 degrees F) hot holding at improper temperatures in an oven which was not on. The person in charge reported a food employee cooked the ground beef and placed it in the oven at 10:30 a.m. The temperature was observed at 11:47 a.m. Instructed the person in charge to reheat to 165 degrees F or higher by 12:30 p.m. Observed the ground beef reheated to 180 degrees F by 12:02 p.m.
      • Absence of consumer advisory for eggs, which may be cooked to order, on the menu.
      • Observed working chemical spray bottle on shelf near the dishwasher which was not labeled to identify the contents. The person in charge reported it was grill cleaner and instructed a food employee to label the bottle.
      • Observed improper cooling methods to cool in house prepared cheese sauce. Observed cheese sauce (45 degrees F) in a deep and covered plastic container in the walk in cooler. Discussed methods to rapidly cool TCS cooked foods and provided handouts in Spanish.
      • Observed bottled beverages stored on a dunnage rack under the handsink by the beverage dry storage. During the inspection the person in charge instructed an employee to relocate the beverages.
      • Observed scoop used to dispense ice at the bar stored with the handle in contact with ice. The person in charge removed the ice scoop to the back of the house for cleaning.
      • Observed multiple stacks of plates stored with the food contact surface facing upward without any overhead protection. Discussed covering the top plate with plastic wrap and instruct employees to pull plates from below.
      • Observed four manufacturer containers of granulated garlic being used to store other spices on the shelving unit by the convection oven. Instructed the person in charge to discontinue reuse of manufacturer containers for food storage.
      • Upon arrival the person in charge could not provide any food grade sanitizer. The person in charge sent an employee to the store to purchase plain, unscented Clorox bleach.
      • The person in charge could not provide a test kit to monitor sanitization using Chlorine both for wiping cloth buckets and the dishwasher. The person in charge provided an Ecolab Sink and Surface Sanitizer test kit, which is dispensed from the dispenser above the mop sink, and the person in charge reported is being used to clean floors. Instructed the person in charge to purchase a Chlorine test kit.
      • Observed the following in need of cleaning: Stainless steel wall behind the cooking equipment, floor behind cook line, ceiling tiles and interior of ceiling light covers.
      • Observed the following in poor repair: Burned out ceiling lights in the kitchen, cracked floor tiles in the female toilet room and under the convection oven and small gap between vent hood filters.
  • Subway (Inside Walmart), 10001 Southpoint Parkway Fredericksburg, VA 22407

    • Total violations: 4
      • Observed accumulation of debris on the following nonfood contact surfaces: Base of the bread rack in the front of the house, rack and interior door of the Turbochef oven, shelf under the sandwich prep which holds utensils, Keurig coffee machine and iced tea brewer, container used to hold various utensils on the clean equipment shelving, and cabinet under the soda fountain in the lobby, appears they may have been a drip. The container used to hold utensils were removed to the 3 vat sink to clean and sanitize the contents.
      • Observed the following in need of cleaning: Food in the floor drain under the 3 compartment sink, interior of ceiling light covers, and the wall behind the 3 compartment sink.
      • Observed the following in poor repair: Burned out ceiling lights, Ceiling tile not secured to the frame above the microwave oven.
      • Observed 1.5 foot candles of light inside the walk in freezer and 2.6 foot candles of light inside the walk in cooler.
  • Little Caesars 3207-004, 5713 Plank Road Fredericksburg, VA 22407

      • Observed that a splash guard is needed near the front hand sink next to the pizza prep unit.
      • Observed that the gaskets on the pizza prep unit and the 2-door walk in cooler door are torn.
      • Observed that the floor fans and exterior of the dough machine require cleaning.
      • Observed that the restroom is missing a covered trash can.
      • Observed that the floors, walls, and ceiling throughout the kitchen is in need of detailed cleaning.
  • Merit School of Stafford Lakes #25, 251 University Blvd Fredericksburg, VA 22406

      • Observed a food employee remove their mask to drink from a plastic water bottle with their bare hands and proceed to handle cleaned equipment without washing their hands. Instructed the person in charge to wash their hands and discussed handwashing when there is a change in tasks.
      • Observed container of salsa, which was opened and date marked 12/2/2021 and a covered plate of cream cheese with a date mark of 12/7/2021. The commercially processed TCS foods should have been discarded on 12/8/2021 (salsa) and 12/13/2021 (cream cheese). The person in charge voluntarily discarded the TCS foods for safety. Discussed holding commercially prepared TCS foods up to a period of 7 days from opening.
      • The battery powered thin probed food thermometer the person in charge provided during the inspection does not work. Instructed the person in charge replace the batteries. The facility also has a di-metallic stem thermometer with a sensor halfway up the shaft of the thermometer.
      • Observed the handsink and drainboard of the three compartment sink are not sealed to the adjacent walls or equipment.
      • The person in charge provided two Quaternary Ammonium testing kits which expired in July 2020. Instructed the person in charge to purchase new test kits.
      • Accumulation of debris on the following nonfood contact surfaces: Base of the convection oven, exterior surfaces of the cabinets under the 3 compartment sink, countertop around the prep sink and shelving above the 3 compartment sink.
      • The water source and system are not of sufficient capacity to meet the peak water demands of the establishment. Observed the hot water at both the left and right faucets of the three compartment sink at 96-97 degrees F and the handwashing sink in the employee toilet room at 70 degrees F. Instructed the person in charge to contact a plumber to evaluate the hot water capacity of the water supply system. Instructed the person in charge to fill the wash and rinse basins of the three compartment sink halfway with hot water from the faucets and heat water on the table top convection burner to achieve a wash and rinse water temperature of at least 110 degrees F. Worked with the person in charge to set up the wash and rinse basins with adequate hot water to meet requirements.
      • Observed the following in need of cleaning: Rubber mats in the kitchen, ceiling tiles in the kitchen and interior of the mop sink.

      • Observed an open mouse trap under the front hand sink (corrected) and an open glue trap over the prep table.
      • Observed that the reach in cooler on the service line is leaking and the oven mitts are in disrepair.
      • Observed that the air vent covers in the walk in cooler and in the restrooms require cleaning.

      • Observed 1 pot and 2 bowls drying on the 3 compartment sink draining boards on wet towels. During inspection, the PIC removed the towels and relocated the equipment for washing and sanitizing
      • Observed 1 box containing 2 bags of breaded mozzarella sticks in the cold holding drawer of the pizza prep unit at 43°F - 44°F. During the inspection, the PIC discarded the 2 bags of breaded mozzarella sticks.
      • The 3 compartment sink faucet and the plumbing is leaking.
      • The following are in need of cleaning: 1. The ceiling throughout the kitchen, warewashing room, and patient service area. 2.The floor drains throughout the kitchen.
      • The floor at the 3 compartment sink is not smooth and easy to clean.
      • The following were observed: 1. A jacket hanging from the drying rack next to the 3 compartment sink. 2. A personal mug on the top shelf of a rack in the warewashing room. 3. A jacket and backpack on the tray cart in the warewashing room. During the inspection, the PIC relocated the items.
  • Vita Felice Italian Restaurant, 914 Bragg Road Fredericksburg, VA 22407

    • Total violations: 12
      • PIC does not demonstrate active managerial control based on the violations and the following observation. While testing the low temp dishwasher, the EHS observed the sanitizing solution at the dishwasher was 0 ppm. EHS informed the PIC of the observation. The PIC then took the bottle of chlorine (used at the 3 compartment sink for sanitizing) and poured it into the dishwasher sanitizing solution container. EHS requested the PIC discontinue use of the dishwasher until the proper sanitizing solution for the dishwasher could be purchased.
      • No paper towels at handwashing sink located inside main kitchen. During inspection the PIC provided paper towels.
      • Observed 3 carbon dioxide tanks, 1 box of pinot grigio, and 1 box of sparkling water blocking the handwashing sink next to the ice machine. During the inspection, the PIC relocated the tanks and boxes.
      • Observed cubed raw chicken in 2 containers over an uncovered container of chicken broth and beef broth in the prep unit. During the inspection, the PIC relocated the raw chicken to the walk in cooler.
      • The sanitizing solution at the low temp dishwasher exceeds limit. The chlorine testing strip turns dark purple and then white. The PIC stated he would discontinue use of the dishwasher and use the 3 compartment sink until he could purchase the correct sanitizer for the dishwasher.
      • Observed 1 quart carton of heavy cream being stored in the ice inside the ice machine. During the inspection, the PIC removed the heavy cream and discarded the ice that came in contact with the carton.
      • The handwashing sink faucet in the main kitchen is leaking when in use.
      • Observed two T-shirt bags in the walk in freezer containing bread knots. During the inspection, the bread knots were discarded.
      • The following were observed soiled: 1. The exterior surface of all equipment in the kitchen. 2. The top of the dishwasher. 3. The hood filters along the cook line.
      • The floor under the 3 compartment sink, the floor drains, and the floor in the mop closet are in need of cleaning.
      • The following are in need of repair: 1. Light out over the 3 compartment sink, in front of the walk in cooler, and above the handwashing sink at the server station. 2. The ceiling tile over the storage rack in the main kitchen.
      • The following were observed: 1. One deck of cards and a container of cheese balls among the spices and single use/service arcticles. 2. An open bag of chili flavored Cheetos, and a personal bottle of chili powder among the seltzer water and cans of pineapple juice used to make beverages in the bar area. During the inspection, the PIC relocated the items.

      • Observed two empty alcohol bottles stored in the hand sink at the bar. Bottles were removed during inspection.
      • Vent filters in the hood system above the pizza oven are not being maintained in a clean condition.

      • The following were observed: 1. A purse and a coat on a shelf over the prep counter and next to a container of coffee filters. 2. The employee's open coffee cup on the prep counter. During the inspection, the PIC discarded the coffee and relocated the personal items.
  • Camellia Colony of Chancellor's Village, 12100 Chancellors Village Ln Fredericksburg, VA 22407

      • The ceiling vents and the tiles around them are in need of cleaning.
      • The hand sink in the kitchen in is not sealed to the wall.
      • The equipment being cleaned in the high temperature sanitizing dish machine were not observed sanitized. The final rinse temperature of the machine was only 135'F on the surface of the equipmwent being cleaned. A repair call was made during the inspection and the 3 compartmentr sink was set up to sanitize equipment.
      • Observed an employee wearing food grade gloved crack open raw eggs into a pan the place cooked bacon on a plate to be served. Employee was instructed to wash hands and change gloves after cracking open raw eggs then touching clean food contact surfaces or ready-to-eat foods.
      • This food establishment does not have an employee, with management and supervisory responsibility, that is a Certified Food Protection Manager.

However, not all the restaurants that the health department visited had violations. Here are a list of locations that were visited, but had no violations. Some were visited as part of pre-opening inspections, while others received routine or follow-up visits.
