Fredericksburg Restaurant Inspections: Violations At 26 Places

FREDERICKSBURG, VA — Officials from the Rappahannock Area Health District inspected several restaurants across the region from Sept. 8 to Sept. 14; they cited more than two dozen restaurants with violations.

As coronavirus restrictions continue to fluctuate around the state, health inspectors are returning to conduct restaurant inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

Depending on the results of the inspection, the health department can pursue several actions. They can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or they can temporarily shut down an establishment until certain issues are solved.

  • Outback Steakhouse, 308 Worth Avenue #101 Stafford, VA 22556
    • Date inspected: Sept. 8
    • Total violations: 9
      • Observed a food employee handle raw battered shrimp with a gloved hand, switch tasks before washing hands first.
      • Observed the following stored food contact surfaces soiled to the sight and touch: 1. a knife 2. several food containers on storage racks. During the inpsection the person in charge removed the items to the 3 compartment sink for rewashing.
      • Observed the following in the walk in cooler: 2 containers of heavy cream with dispose by date of 8/28/21 and 9/4, and a milk container with a dispose by date of 9/2. During the inspection the person in charge discarded the expired food items.
      • Observed the following toxic chemicals not stored in the appropriate space: 1.1 bottle of degreaser by the hot line 2. 1 bottle of sanitizing solution hanging from a handsink 3. 1 squirt bottle of unidentified solution stored by a handsink The person in charge removed the items during the inspection.
      • Observed wet stacking of stored stainless steel food containers.
      • Observed an accumulation of ice on the floor and condenser of the walk in freezer.
      • Observed debris around the dumpster area.
      • Observed the following in need of repair: the toilet lid of the womens employees bathroom and blown out lightbulbs of the hood vent.
      • The following areas were observed soil: 1. floors behind equipment 2. floors of walk in cooler, freezer and dry storage area
  • Taco Bell #27587, 9625 Jefferson Davis Highway Fredericksburg, VA 22407
    • Date inspected: Sept. 8
    • Total violations: 1
      • Observed that the ceiling HVAC intake vent is heavily soiled.
  • Chili's Grill & Bar #1830, 10010 Southpoint Parkway Fredericksburg, VA 22407

    • Total violations: 8
      • The interior of several microwaves were observed soiled.
      • The following were at improper temperatures in the salad prep unit: one container of shredded cheese (46F - 51F), one container of pico (47F), and one container of corn salsa (52F - 59F). During the inspection, the food was discarded.
      • Observed the handles of several in-use scoops in the shredded cheese and shredded lettuce. During the inspection, the PIC removed the in-use scoops.
      • Observed several metal food containers throughout the facility that were stacked while wet after cleaning and sanitizing.
      • Observed gaps between hood filters and the salad prep unit not holding temperature.
      • The following were observed soiled: 1. Several fryer baskets at the fryers, 2. The interior of the fryer cabinets, 3. The exterior of the conveyor oven, 4. The door gasket on the under the counter RIC at the To Go area.
      • The back exterior door is in need of repair.
      • The following are in need of cleaning: 1. The floors under all equipment, 2. The floor and wall along the cook line, 3. The mop sink basin.
  • Danny’s Pizza #3, 9837 Courthouse Road Spotsylvania, VA 22553
    • Date inspected: Sept. 8
    • Total violations: 1
      • The following foods were observed cold holding at improper temperatures in the sandwich prep unit: deli sliced turkey (48'F), deli sliced ham (47'F), sliced tomatoes (47'F) and raw chicken (47'F). The items had been stored in the walk in cooler and were put in the prep unit ay 10:45 am. Items were placed back in the walk in cooler and the unit temperature was turned down,
  • Taste of Jamaica, 263 Garrisonville Road Stafford, VA 22554
    • Date inspected: Sept. 8
    • Total violations: 2
      • Observed the cook handle the sanitizer bucket then continue to reheat foods without washing his hands. EHS discussed this with the cook.
      • Repeat Observed red beans in the walk in cooler that was not dated for disposition.
  • Mattaponi Springs Golf Club, 22490 Penola Rd Ruther Glen, VA 22546
    • Date inspected: Sept. 8
    • Total violations: 8
      • Absence of hand drying provision noted at the bar hand sink. The person in charge placed paper towels at the hand sink.
      • Observed package of raw bratwurst sausage stored over ready to eat containers of salad dressing and wrapped sliced turkey in the 1 door upright cooler. The bratwurst was relocated to prevent contamination.
      • The following food contact equipment was observed soiled: Food contact surfaces of the meat slicer, cone colander stored above the 2 vat sink, knife in the knife rack, stack of stainless steel containers above the prep cooler and tongs at the bar. The person in charge removed all items for cleaning with the exception of the meat slicer. Instructed the person in charge to clean and sanitizer the meat slicer prior to next use.
      • The person in charge could not provide a copy of the water sampling report, the report is held off site and will be emailed.
      • Observed rust-like buildup inside the basins of the kitchen and bar handwashing sinks and handle of a stainless steel colander is broken. Discussed replacing the colander during the inspection.
      • Observed accumulation of debris on the following nonfood contact surfaces: Inside 2 vat sink, meat slicer, base and gasket of 1 door reach in freezer, interior of 2 door reach in cooler and prep cooler, wells used to store bags of lemons and limes, shelf under griddle/char broiler, and interior of fryers.
      • Observed the following areas in need of cleaning: Interior of ceiling light covers in the dry storage room and female toilet room, floors under and behind the fryers and bar 3 vat sink and ice well, and soiled ceiling tiles in the female toilet room.
      • Observed 8.7 foot candles of light in the dry storage room, observed blown out ceiling lightbulbs .
  • Chili's Grill & Bar #1828, 3104 Plank Road Fredericksburg, VA 22407
    • Date inspected: Sept. 9
    • Total violations: 6
      • The following are in need of cleaning: 1. The sanitizer dispenser to the right of the dishwasher, 2. The light shields throughout the kitchen.
      • The non food contact surfaces of the following equipment were observed soiled: 1. The conveyor oven, 2. The filter of 2 door freezer across from the fryers, 3. The lip of the ice machine door.
      • The following were in need of maintenance/repair: 1. Ice build-up on the WIF condenser. 2. Gap between hood filters over the fryers and convotherm oven.
      • Observed several plastic food containers under the prep table that were stacked while wet after cleaning and sanitizing.
      • Observed salsa at improper temperatures in the bar WIC: Two containers (54F - 61F) and eleven To Go containers (56F - 58F) that were prepared on 09/07/2021. During the inspection, the PIC discarded the salsa.
      • Observed food employee apply soap and scrub hands under running water under 10 seconds. EHS informed food employee of the proper handwashing procedure
  • Papa John’s Pizza, 2769 Jefferson Davis Hwy 119 Stafford, VA 22554
    • Date inspected: Sept. 9
    • Total violations: 9
      • Observed a food employee lather their hands under running water at the front handsink. Hands were not effectively cleaned because the handsoap was flushed down the drain.
      • Observed a food employee set up sanitizer at the 3 vat sink with their bare hands and then proceed to place boxes of food in the walk in cooler. Also observed a food employee touch their facemask and hat with their bare hands and then proceed to handle a pizza box to place a pizza order in without washing their hands. Instructed the food employees to wash their hands.
      • Observed the only can opener in the food establishment soiled. The can opener was removed to the 3 compartment sink for cleaning.
      • The person in charge could not provided a food thermometer during the inspection.
      • Observed a food employee preparing pizza with their bare hands with long painted fingernails. At the beginning of the inspection this was the only employee engaged in food preparation tasks.
      • Observed the front and warewashing handsink and 3 compartment sinks are not sealed to the adjacent walls.
      • Observed accumulation of debris on the following nonfood contact surfaces: interior gears of the pizza oven, dust in hood vents and track of the beverage cooler.
      • Observed the lids of the recycling and waste dumpsters opened.
      • Observed the following in poor repair: Blown out ceiling lights in the dry storage and warewashing areas, crack in floor tile near warewashing handsink, missing ceiling tile above bottle beverages storage, ceiling tiles are not flush with the ceiling frame in the dry storage and warewashing area, and damaged ceiling in the employee toilet room, ceiling appears to be peeling.

    • Date inspected: Sept. 9
    • Total violations: 8
      • Observed the following Non TCS food contact surfaces soiled, the blade of the counter can opener, and the wall mounted potato dicer. During the inspection the can opener was relocated to the 3 compartment sink for rewashing.
      • Observed the fryer baskets in the deep frying station with a heavy grease build up.
      • Observed several bottles of toxic cleaning chemicals near food prep and storage areas.
      • Observed plastic bowls stacked while wet. During the inspection the wet equipment was moved to the 3 compartment sink for rewashing.
      • The following non food contact surfaces were observed in need of cleaning: -walls and sides of equipment -ac vent and duct in the kitchen -shelving throughout kitchen -inside of the fry cabinet
      • The following were observed in need of cleaning: -walls of the kitchen, storage and dishroom -floors behind equipment throughout the kitchen, storage, dishroom -ceiling tiles of the kitchen, storage and dishroom
      • Observed the following in disrepair: -floor tiles in the kitchen -light fixture in the dishroom area -replace blown out lightbulbs in the prep area -the hole in the ceiling in the storage area
      • Observed the hood vent and filters heavily soiled.
  • Harrison Road Elementary School, 6230 Harrison Road Fredericksburg, VA 22407
    • Date inspected: Sept. 9
    • Total violations: 3
      • The faucet on the hot holding wells on the B side of the serving line is leaking.
      • The lights in the walk in freezer are not working. Measured 0 foot candles inside of unit.
      • All of the ceiling HVAC intake vents in the kitchen are dirty.
  • Faith Baptist Church, 4105 Plank Road Fredericksburg, VA 22407
    • Date inspected: Sept. 10
    • Total violations: 5
      • Observed that the interior of the ice machine require cleaning.
      • Observed single service utensils and plates that were improperly stored with the food contact surface facing upwards. The PIC provided protection for the single service items.
      • Observed that both hand sinks are coming off the wall.
      • Observed that the cutting boards are scratched and scored.
      • Observed that an open site hose is sitting inside the mop sink without a proper backflow preventer installed.
  • Children of America Lee’s Hill, LLC, 4555 Empire Court Fredericksburg, VA 22408
    • Date inspected: Sept. 10
    • Total violations: 6
      • Observed food debris in the basin and drain of the only handwashing sink in the kitchen. Instructed the person in charge handsinks are only to be used for handwashing.
      • Observed counter top mounted can opener soiled.
      • Observed wet cloths stored on the faucet of the three compartment sink. The person in charge removed the cloths to be washed in the washing machine.
      • Observed two rolls of toilet paper and one roll of paper towels stored on an open shelf in the toilet room used by staff unwrapped or otherwise protected from cross contamination. The person in charge removed the toilet paper and paper towels to prevent contamination.
      • Observed accumulation of debris inside cabinets in the kitchen, some of the cabinets had a sticky residue. Also observed the base and interior door of the oven soiled.
      • Observed cleaning is needed on the walls in the kitchen and interior of the mop sink.
  • Taj Indian Cuisine, 2032 Plank Rd Fredericksburg, VA 22401
    • Date inspected: Sept. 10
    • Total violations: 21
      • Observed a food employee scrubbing his hands under running water. During the inspection EHS informed the employee on proper hand washing techniques.
      • Observed one employee washing their hands at the 3 compartment sink and another employee washing their hands at the mop sink. EHS informed employees of where to wash their hands.
      • No hand soap available at the bar hand sink. During the inspection soap was provided.
      • Observed the following foods uncovered: 1.In the walk in freezer, one pan of samosas and one pan of red chicken 2. In the walk in cooler, one pan of samosas and 1 pan of chickpeas 3. In the upright cooler, one container of raw shrimp and 1 container of raw fish.
      • The following TCS foods were cold holding at improper temperatures: 1. In the bottom reach in cooler 1 (prepared on 09/09/21): one container of red chicken (47°F), one container of cubed chicken (50°F), one bowl of shredded lettuce (47°F), one container of veggie samosas (50°F), two bags of chicken samosas (50°F), one container veggie fritters (45-55°F). 2. The bottom reach in cooler 2: one container of green chutney (63°F), one container and six (2oz cups) of yogurt (66° F). 3. On the bottom shelf of prep table (prepared 09/09/21): one bowl of spinach fritters (78°F). During the inspection, the foods was discarded.
      • Observed the following: 1. One container of Vicks rub next to a rolling pin on a shelf. 2. Two bottles of dietary supplements on a shelf over single serve articles. During the inspection, the items were relocated.
      • Observed a chemical spray bottle containing bleach without a label. During the inspection the person in charge discarded the spray bottle.
      • The person in charge was unable to provide a food thermometer.
      • Observed several food containers of spices and cooking ingredients (salt, sugar, cornstarch) without a label.
      • Observed a food employee working without a restraint for facial hair.
      • Observed a food employee wearing a bracelet while preparing foods.
      • Observed the ice scoop handle in direct contact with the ice in the ice machine.
      • Observed an employee drying dishes with a wiping cloth.
      • Observed several plastic food containers on a rack over the pre rinse sink that were stacked while wet after cleaning and sanitizing.
      • Observed single service use containers of garlic being reused to store spices and sauces.
      • Observed the caulking of the 3 compartment sink and the utility sink soiled.
      • The bottom reach in cooler 1 is in need of repair.
      • The following are in need of repair: 1. The prep sink faucet is leaking. 2. The bar hand sink faucet is broken.
      • The following are in need of repair: 1. Soiled ceiling tiles throughout the facility, 2. The door frame into the dishwashing room, 3. The employee bathroom wall, 4. The hallway walls into the dry storage area.
      • Observed personal items (socks, shoes, shirts, jackets, cologne, shampoo, personal food) above, below, and hanging over food, single service and single use articles throughout the facility.
      • Observed the lighting intensity of 3.6 foot candles in the walk in cooler.
  • Cedar Forest Elementary School, 3412 Massaponax Church Road Fredericksburg, VA 22408
    • Date inspected: Sept. 13
    • Total violations: 3
      • The blade of the can opener and the interior of the ice machine were observed soiled. Can opener was taken to dish room to be cleaned and the ice machine was cleaned during inspection.
      • Observed broccoli (118'F) and chicken nuggets (112' F) hot holding at improper temperatures on the serving line. Food was removed from the line and placed in the oven to re-heat and replaced with product that measured proper temperature.
      • The following physical facility items were observed soiled: 1-the ceiling vents throughout the kitchen, 2-the ceiling tiles around the ceiling vents.

      • The following physical facility items were observed soiled: 1-the caulking around the hand sinks, 2-The wall above the 3 compartment sink.

      • Observed the following: 1.several uncovered containers of food in the upright freezer and at the cooks line 2. Two uncovered containers of flour and cinnamon sugar mix. 3. One peeled banana above a prep unit. During the inspection the person in charge provided a cover for the food containers, and a food container for the banana.
      • The following food contact surfaces were observed soiled: several stored utensils (spatulas, measuring cups, two knives) fryer baskets, dicer, meat slicer, toaster
      • Observed all of the microwaves throughout the facility soiled.
  • Roma Italian Restaurant, 114 W Broaddus Ave Bowling Green, VA 22427
    • Date inspected: Sept. 13
    • Total violations: 6
      • The rear door in the kitchen is not self-closing.
      • The frame around the door between the dining room and the kitchen is bare wood.
      • The 3 compartment sink and the hand sink in the kitchen need to be re-sealed to the wall.
      • The following non-food contact surfaces were obse3rved soiled: 1-the top and back of dish machine, 2-the wire and plastic shelving in the walk in cooler.
      • The following physical facility items were observed soiled with grime and debris: 1-the ventilation hood over the pizza oven, 2-the flexible ductwork in the kitchen, 3-the back door.
      • The light intensity is below 10 foot candles in the walk in cooler. Observed 2 foot candles in this area
  • Italian Station Coffee Shop, 620 Caroline Street Fredericksburg, VA 22401
    • Date inspected: Sept. 14
    • Total violations: 3
      • Observed flying insects in the dry storage room and the mop sink.
      • The wall behind the chest freezer and the wall at the mop sink are in need of repair.
      • Observed personal items (backpack, umbrella, sweater,) hanging over a blender, plastic food containers, spice containers, and jars of capers and artichoke hearts.
  • Sedona Taphouse, 591 William St. Fredericksburg, VA 22401
    • Date inspected: Sept. 14
    • Total violations: 7
      • No paper towels at the hand sink at the end of the cook line. During inspection, the PIC provided paper towels.
      • No hand soap at the hand sink at the end of the cook line. During inspection, the PIC provided hand soap.
      • Observed a plastic pitcher in the hand sink at the end of the prep line. During inspection, the PIC removed the pitcher.
      • Observed 1 container of cooked pork belly cold holding at 51°F in the reach in cooler that was prepared on 09/11/21. During the inspection, the PIC discarded the pork belly.
      • The following were observed: 1. Several Styrofoam cups in the sugar and flour bulk containers. 2. Four knives being stored between prep unit and prep table. 3. Observed spatulas and serving spoons being stored in a container of water (103°F) on the prep table next to hot holding unit. During the inspection, the PIC discarded the Styrofoam cups, relocated the knives and utensils to the warewashing area.
      • The following are in need of repair/maintenance: 1. The handle of the handwashing sink located at the end of the cook line. 2. The walk in cooler #2 condenser fan. 3. The prep unit (located in front of the grill) shut off. During the inspection, the PIC discarded all TCS food that had exceeded the 4 hours of improper cold holding temperature: 2 containers of cooked chicken 62°F, 2 containers of hummus 59°F - 66°F, 1 container of stuffed jalapenos 62°F, 1 1/2 trays of stuffed shrimp 62°F, sliced tomatoes 61°F - 63°F, 1 container of pico 69°F, shredded cheese 62°F, and 1 container of rice 67°F.
      • Observed personal items (employee food, keys, phones, purses, backpacks, and jackets) being stored over clean equipment, food, single use articles, and on the prep line. During the inspection, the PIC relocated the personal items.
  • Starbucks Coffee #7918, 1495 Stafford Market Place Stafford, VA 22556
    • Date inspected: Sept. 14
    • Total violations: 3
      • Observed the employees wash their hands then turn the water off with their clean hands. EHS discussed this with the person in charge.
      • Observed that the air vent covers and the floors under the equipment are in need of detailed cleaning.
      • Observed lotion stored on the hand sink. The PIC removed the product.
  • Patriot Subs, 1131 Jefferson Davis Hwy Fredericksburg, VA 22401
    • Date inspected: Sept. 14
    • Total violations: 3
      • Observed a plastic bottle and ladle in the handwashing sink. During inspection, PIC removed items from items from the handsink.
      • The non food contact surfaces of the following equipment are soiled: interior of air fryer and filters of conveyor oven.
      • Observed the deli slicer soiled. During the inspection, the PIC cleaned and sanitized the slicer.
  • The Sunken Well Tavern, 720 Littlepage Street Fredericksburg, VA 22401
    • Date inspected: Sept. 14
    • Total violations: 15
      • Observed a food employee place a raw hamburger on the grill with gloved hand, then continued to don new gloves without washing hands first. EHS instructed the employee to wash hands after handling raw animal product.
      • Observed no paper towels available at the hand washing sink of the bar area. During the inspection person in charge provided paper towels.
      • Observed the following food contact surfaces soiled: a knife stored on a knife block, plates under food prep table, tongs and cutting board on metal shelf. Items were removed to the dishwashing area for rewashing.
      • The following foods were observed cold holding at improper temperatures:- 1. one container of pico de gallo (47 degrees). 2. one container of chorizo (47 degrees). 3.one container of pork belly (47 degrees). 4. one container of chilli (48 degrees). 5. one container of rice (48 degrees). 6. one container of veggie broth (50 degrees). 7.one container of sausage links (49 degrees). 8. one container of par cooked french fries (50 degrees). 9. one container of par cooked brussel sprouts (50 degrees). 10.one container of queso (47 degrees). 11.cheese slices (47 degrees). 12.one container of cream cheese (48 degrees) During the inspection the person in charge discarded the foods for safety.
      • Observed two bottles of oven cleaner on a shelf at the cook line. During the inspection the person in charge relocated the items.
      • Observed a bottle of advil stored with cooking utensils. During the inspection the person in charge relocated the item.
      • Observed the following foods stored on the floor of the walk in: 1.two containers of chicken and one container of ground beef.
      • Observed food employees working without hair and beard restraints.
      • Observed a wet wiping cloth being used to hold a cutting board in place, and a wiping cloth used to hold an ice scoop in the bar area.
      • Observed stainless steel containers stored stacked while wet.
      • Observed single service use containers being reused to hold spices.
      • The caulking of the handsink, 3 compartment sink and dish machine area are missing.
      • Observed the following in poor repair: a leak in the walk in cooler and the knobs of the grill are missing.
      • The following are observed in need of cleaning: 1. ac vents of the bathrooms 2. floors underneath equipment 3. walls behind dish machine and 3 compartment sink area.
      • Observed employees drinking containers stored near food preparation and equipment storage areas. During the inspection the containers were removed.
  • Cook Out Restaurant, 10108 Jefferson Davis Hwy #300 Fredericksburg, VA 22407
    • Date inspected: Sept. 14
    • Total violations: 12
      • This food establishment does not have an employee, with management and supervisory responsibility, that is a Certified Food Protection Manager.
      • The paper towel dispensers near the manager's desk and in the employee restroom do not work. Paper towel was provided during the inspection.
      • Chlorine sanitizer in the wiping cloth bucket does not meet the requirements of 40 CFR 180.940 or 40 CFR 180.2020. Chlorine sanitizer measured well over 400 ppm. Sanitizer was diluted to 50 ppm during inspection.
      • Observed spray bottles of sanitizer, bleach and stainless steel cleaner hanging above the prep table. Chemicals were relocated during inspection.
      • Observed the ice scoop in the ice bin at the drive thru station stored with it's handle in contact with the ice. Scoop was removed from the ice during inspection.
      • Observed several plastic line insert pans stacked while wet after cleaning and chemical sanitization.
      • Observed 2 plastic spatulas that were gouged and pitted. Spatulas were discarded during inspection.
      • Observed a large gap in the filters under the ventilation hood over the cook's line.
      • The following non-food contact surfaces were observed soiled with grime and debris: 1-the interior of the deep fryer cabinets, 2-the upsplash of the soda dispenser at the drive-thru window.
      • There was no covered garbage can in the employee restroom.
      • Observed 3 burnt out light bulbs under the hood over the cook's line.
      • The following physical facilities were noted in need of cleaning: 1-the caulking along the 3 compartment sink (mold), 2-the walls near the employee restroom and the door itself, 3-the caulking along the employee restroom sink.
  • Top Chef, 43 Town and Country Drive #109 Fredericksburg, VA 22405
    • Date inspected: Sept. 14
    • Total violations: 8
      • Observed tongs hanging over the fryer and the fry baskets that require cleaning. The person in charge took the items to the 3-compartment sink for cleaning.
      • Observed WD-40 stored with the spices. The person in charge removed the WD-40.
      • Observed that the rear screen door is not self-closing.
      • Observed several flies in the rear prep area.
      • Observed that the stopper is missing for the wash sink.
      • The facility does not have bleach test strips.
      • The following non-food contact surfaces require cleaning: tops of the equipment, tops of the fryers, inside of the reach in freezer, and outside of the rice cooker.
      • The following physical facilities are in need of cleaning: the hood system, in-between the floor-mounted equipment, ceiling in the women's restroom, and the floor drains.

    • Total violations: 14
      • Observed food employee plate beef dumpling with bare hands. EHS informed PIC of observation. The food employee discarded dumplings.
      • Observed hand soap at the prep sink and observed food employee attempt to wash hands at prep sink. During inspection, the PIC removed the hand soap from the prep sink and informed the food employee on where to wash their hands.
      • No parasite destruction letter from supplier for salmon.
      • The following were observed in the walk in cooler: 1. Raw chicken over a bag of carrots. 2. Raw beef over cut vegetables. During inspection, the food employee relocated the raw food to the bottom shelf.
      • The dishwasher was not sanitizing (0 ppm) equipment and utensil due to the sanitizer bottle being empty. During the inspection, a food employee replaced the sanitizer bottle.
      • The sushi rice for which time rather than temperature is being used as a control was not labeled or otherwise marked with a 4 hour cook, serve, or discard time.
      • Observed several flying insects along the back hallway used for dry food storage.
      • Observed heavily soiled floors and a box on the floor with chopped vegetables next to the exterior back door.
      • Observed the following food being stored on the floor of the walk in cooler: 1 bag of yellow onions, 1 tub of beef dumplings, and 1 box of raw shrimp. During the inspection, the food employee removed the food off the floor.
      • Observed several wet wiping cloths on the cook line, prep line, and counters. During the inspection, the employee placed the wet wiping cloths in a sanitizing bucket.
      • The caulking at the 3 compartment sink and the dishwasher are heavily soiled.
      • The following are in need of cleaning: 1. The non food contact surfaces of all equipment along the cook line. 2. The shelves in the walk in cooler. 3. The hood filters above the flat top grill.
      • The following are in need of cleaning: 1. The storage room across from the mop sink. 2. The wall and floor behind the cook line. 3. The floors under all equipment.
      • Observed personal items (cups, mugs, bottle beverages, phones, and food) on a shelf over the soup and on the prep line. During the inspection, the food employees relocated their personal items.
