Fredericksburg Restaurant Inspections: Violations At 29 Locations

The Rappahannock Area Health District cited more than two dozen restaurants with violations during their most recent round of inspections.

FREDERICKSBURG, VA — Officials from the Rappahannock Area Health District inspected multiple restaurants across the region from July 28 to Aug. 3; they found 29 restaurants with violations.

As coronavirus restrictions continue to fluctuate around the state, health inspectors are returning to conduct restaurant inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.

Violations at restaurants typically focus on factors that can contribute to food contamination. The local health department also performs a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.

Depending on the results of the inspection, the health department can pursue several actions. They can issue a permit for the restaurant to continue serving food, they can schedule a follow-up inspection to determine if violations have been rectified, or they can temporarily shut down an establishment until certain issues are solved.

  • Four Seasons Family Restaurant, 5201 Jefferson Davis Highway Fredericksburg, VA 22408
    • Date inspected: July 28
    • Total violations and corrective actions: 7
      • Observed the following foods cold holding at improper temperatures in the cook's prep unit: 1-Diced Ham (60'F), Cooked Pork (61'F), Hard Boiled Eggs (60'F). Foods were voluntarily discarded during inspection and a repair company was called during inspection. Other items in the unit were either placed in the walk in cooler or place on ice inside of the unit.
      • The prepared ready-to-eat (RTE) meatloaf, tuna salad, and sliced fruit salad in the refrigeration unit is not labeled with a ""consume by"" date. Items were dated during inspection.
      • The ready-to-eat (RTE) commercially processed foods in the walk in cooler (deli sliced ham and deli sliced turkey) were not properly dated for disposition after opening. Items were dated during inspection.
      • Employees observed working in food preparation areas with proper beard guards.
      • There are no covered refuse containers in the women's restroom stalls.
      • The floor in the walk in freezer was observed heavily soiled.
      • Observed that inadequate lighting was provided in the food preparation areas. Measured 20 foot candles in this area.
  • Basilico NY Deli & Italian Bakery, 7013 Harrison Rd Fredericksburg, VA 22407
    • Date inspected: July 28
    • Total violations and corrective actions: 6
      • Observed a food employee handle ready-to-eat cannoli's with their bare hands while dipping them in chocolate chips. EHS instructed the employee that gloves or utensils must be used.
      • The handwashing sink in the kitchen that serves the cook line and prep area was not functional during the inspection. PIC stated the water has been turned off to the sink due to a leak. PIC stated a repair company will be contacted to repair the sink as soon as possible. EHS informed the PIC that staff should wash their hands in the nearest handwashing sink.
      • Observed raw shell eggs stored over vegetables in the walk-in cooler. PIC relocated the raw shell eggs to the bottom shelf.
      • Observed several employees in the front prep area wearing gloves. One employee handled a soiled towel on the countertop and a soiled towel hanging from her apron, then proceeded to prepare a ready-to-eat sandwich. Another employee was observed handling soiled towels and then proceeded to prepare ready-to-eat vegetables. Discussed proper glove use and when a glove change is required with the person in charge and encouraged the PIC review this with staff members.
      • Observed an employee washing dishes at the 3-compartment sink, failing to properly sanitize the equipment. Observed employee wash equipment, rinse, and place in the sanitizer basin of the 3-compartment sink in a quaternary ammonium sanitizer solution that measured 0ppm. PIC stated he was aware the pre-mix dispensing system was not functioning correctly and has called for repair. Staff were informed the sanitizer at the 3-compartment sink is not functional and all equipment will be put through the dishmachine to sanitize. If the equipment is too large for the dishmachine, a chlorine solution will be prepared in the 3-compartment sink.
      • Observed fresh garlic/oil mixture (75'F) stored next the pizza prep table at room temperature. Garlic/oil mixture was discarded. PIC stated the mixture will be stored in the pizza prep unit.
  • Golden Corral, 10320 Spotsylvania Avenue Fredericksburg, VA 22408
    • Date inspected: July 28
    • Total violations and corrective actions: 1
      • The food-contact surfaces of the equipment being cleaned in the high temperature sanitizing dish machine were not observed sanitized. The water temperature at on the surface of the equipment only measured 155'F. It was discovered that the booster pump had not been turned on. After 20 minutes, the final rinse temperature measured 163'F on a maximum temperature reading thermometer.
  • George’s Pizza, 5306 Plank Road Fredericksburg, VA 22407
    • Date inspected: July 28
    • Total violations and corrective actions: 7
      • No certified food protection manager available. PIC stated a manager recently took the course, but did not have a copy of the certificate.
      • Person in charge was unable to provide documentation that staff are trained on the facility's employee health policy.
      • Observed person in charge handle sliced deli meat for a cold RTE sandwich with her bare hands. EHS informed PIC that gloves are needed.
      • Sliced tomatoes (45'F) cut from an ambient temperature last night and stored in a closed plastic container in the prep unit measured to be cooled improperly. Tomatoes discarded. All other TCS foods in the unit measured 41'F or less.
      • Pizza sauce (71'F) was measured cold holding at improper temperature. Pizza sauce is normally stored in prep unit, but has been at room temperature since 11am. Time as a public health control will be used today and sauce will be discarded at 3pm. Several prepared pizza doughs topped with pizza sauce and cheese (67'F, 68'F) stored at room temperature since 11am. PIC stated pizzas are normally stored in the reach-in cooler. EHS discussed using time as a public health control for pizza sauce and prepared pizzas.
      • The following foods were not dated for disposition: turkey, ham, roast beef, pastrami, and meatballs.
      • Back door to facility is not tight-fitting.
  • My Wife's Pizza & More, 2010 Princess Anne St Fredericksburg, VA 22401
    • Date inspected: July 28
    • Total violations and corrective actions: 3
      • No Certified Food Protection Manager certificate.
      • No handwashing sign in restroom. During inspection, PIC created a sign and posted in restroom.
      • The half wall between the pizza prep unit and the register is not smooth and easy to clean
  • Starbucks Coffee #9677, 2001 Plank Road Fredericksburg, VA 22401
    • Date inspected: July 28
    • Total violations and corrective actions: 1
      • No irreverisble registering temperature indicator was not provided for the hot water mechanical warewashing operation. The establishment will discontinue use of the high temperature dishwasher and use the 3 compartment sink for cleaning and sanitiizing of equipment.

    • Total violations and corrective actions: 13
      • This food establishment does not have an employee, with management and supervisory responsibility, that is a Certified Food Protection Manager.
      • No hand drying provisions were provided at the handwashing sink in the kitchen.
      • Raw animal food stored over ready-to-eat food in the refrigeration unit. Observed raw shell eggs stored above unwrapped cut watermelon and pineapple in the reach in cooler. Eggs were relocated during inspection.
      • Observed unwrapped watermelon and pineapple stored in the reach in cooler. Food was wrapped during inspection.
      • The interior of both microwave ovens in the kitchen were observed soiled.
      • The Person in Charge could not provide a food temperature measuring device.
      • Observed house salsa being stored in a used plastic milk carton.
      • There is a heavy build-up of ice inside of the chest freezer which is preventing the lid from closing.
      • The following nonfood contact surfaces have accumulations of grime and debris: 1-the water heater, 2-the fan and cover in the reach in cooler, 3-the outer surfaces of the microwave ovens, 4-the outer surfaces of the equipment on the cook's line, 5-the shelf above the expo station. IN GENERAL ALL EQUIPMENT IN THE KITCHEN IS IN NEED OF A THOROUGH CLEANING.
      • The men's restroom door is not provided with a self-closing door.
      • The walls, floor and ceiling in the kitchen is in need of a deep and thorough cleaning.
      • The ventilation head and filters over the cook's line are covered with grease and debris.
      • Light bulb in the kitchen above the dish washer not shielded, coated, or otherwise shatter-resistant.

      • The walls throughout the kitchen were observed heavily soiled.
      • The front hand sink was not sealed to the wall.
      • Observed employee cooking food with wearing a hair restraint. A hair restraint was donned during inspection.

      • Observed beans, cheese, and roasted peppers in the walk in cooler that were uncovered. Observed salsa that was uncovered. The person in charge covered all of the items.
      • Observed that several lids that were broken.
      • Observed that the floor in the walk in freezer is not smooth and easily cleanable.
      • The air vent covers in the walk in refrigeration units and the floors under the floor-mounted equipment require cleaning.

      • The hand sink is being used as a dump sink. EHS discussed this with the PIC.
      • Observed that the can opener requires cleaning. EHS placed the can opener in the dish washing area.
      • Observed cheddar cheese at 49'F and 43'F and hot dogs at 43'F all cold holding at improper temperatures. EHS discussed this with the person in charge.
      • Observed sour cream that expired on 3/30/21 and yogurt expired on 7/28/21 and 6 packs of turkey that expired on 7/22/21. All items were discarded.
      • The facility could not provide a small probe thermometer.
      • Observed that the cook is not wearing a hair restraint.
      • Observed serving bowls stacked wet in storage.
      • Observed equipment stored on the floor in the storage room.
      • Observed plastic serving bowls that were stored with the food-contact surface facing upwards.
      • Observed that the upright reach in refrigerator is leaking. The facility manager placed a call to the maintenance office to have it repaired.
      • Observed plastic on the reach in refrigeration units.
      • Observed that the inside of the reach in refrigerators require cleaning.

      • Observed accumulation of debris inside and exterior doors of reach in refrigeration equipment and wheels of equipment in the cooking area.

      • The warewashing machine is not equipped with a device to warn of low sanitizer level.
      • Observed employee working in the food service area without proper hair restraints
      • Consumer self-dispensing bulk food containing cereals is not properly labeled.
      • Observed a bottle of hand sanitizer next to ss articles on cart, spray bottle of sanitizer between bagels and steam oven and packets of all purpose floor cleaner over the 3 compartment sink.
      • The dishwasher is not sanitizing (0 ppm) equipment and utensils. The PIC will discontinue using dishwasher and use the 3 compartment sink for cleaning and sanitizing.

      • The prepared ready-to-eat (RTE) cooked Battered chicken and cooked pork egg rolls in the walk in cooler are not labeled with a "consume by" date. Items were labeled during inspection.

    • Date inspected: Aug. 2
    • Total violations and corrective actions: 22
      • Observed food employee enter the kitchen while drinking from an open Pepsi cup and placing it on a shelf over the prep unit next to the flat top grill. EHS informed PIC of observation. PIC had food employee discard beverage.
      • Observed 1 food employee scrub his hands under running water and another employee wash their hands in less than 5 seconds. EHS informed the employees of the proper handwashing procedure. Employees rewashed their hands.
      • Observed food employee use his bare hand to add scallions to the ready-to-eat ramen. EHS informed the food employee of observation. Food employee discarded food.
      • No paper towels in the employee restroom. During inspection, paper towels were provided.
      • Observed 1 dented can of tapioca pudding and 1 punctured soy sauce can in the dry storage room. During the inspection, the PIC discarded the cans.
      • No written agreement/statement from the supplier/aquaculturist stipulating that the fish were raised (open water in net pens; or land-based operations such as ponds or tanks) and pellet fed.
      • Observed raw salmon over a container of teriyaki sauce and cherries in the prep unit and raw chicken over cooked chicken in the walk-in cooler. During the inspection, the food was relocated and covered.
      • Observed a heavily soiled cutting board leaning against the prep sink faucet, one knife being stored between the prep sink and prep table, and one knife on the magnetic strip next to the rice cookers. During inspection, the cutting board was discarded and the knives were relocated to the dishwashing area.
      • The interior of ice machine was observed soiled.
      • The sushi rice for which time rather than temperature is being used as a control was not labeled or otherwise marked with a 4 hour cook, serve or discard by time. During the inspection, the PIC labeled the sushi rice with the 4 hour serve by time.
      • Observed a can of Raid next to the SS articles and a bug spray bottle next to bottled beverages in the dry storage area next to the employee restroom. During the inspection, the PIC had an employee relocate the items.
      • Observed 3 ROP tuna in the walk-in cooler that were completely thawed. The package indicated the tuna needed to be removed from package prior to thawing. During the inspection, the PIC had the food employee discard the tuna.
      • The following were observed: 1. One tray of bagged ground beef and packages of beef ribs on the floor of the walk-in freezer. 2. A bottle of Sunny-D inside a stainless steel food container of cut carrots, mushrooms, and broccoli in the prep unit. During the inspection, the food in the walk-in freezer was relocated. Also, the Sunny-D bottle was relocated and the vegetables discarded.
      • Observed a towel over the sushi rice. Food employee stated it was being used to keep the rice from drying out. EHS asked the food employee to remove the towel and discard the contaminated rice.
      • Wiping cloths improperly stored between use, not in chemical sanitizing solution, in the following locations: In the warewashing area, on the cookline, on the prep tables, and hanging behind SS articles in the sushi bar area. During inspection, the wiping cloths were placed in the sanitizing buckets.
      • Observed the soup spoons, knives, and forks are displayed so that the food contact surfaces are being exposed to contamination. During the inspection, the PIC relocated the utensils to the warewashing area for cleaning and sanitizing.
      • Observed several stainless steel and plastic food containers over the 3 compartment sink that were stacked while wet after cleaning and sanitizing.
      • Observed a red chili paste container being reused to store seaweed on the shelf above the prep unit (CORRECTED during inspection) and several seaweed containers in the sushi cooler with imitation crab and cut cucumber.
      • The following were observed: 1. A gap between the hood filters near the fryers and rice cookers. 2. Frost built-up in the curved top storage freezer. 3. The cinder block and wood shim under the ice machine. 4. The wood shim under the rice cooker table.
      • The following were observed: 1. Three non-food grad take out bags of ground chicken and 12 non-food grade take out bags of ground beef found in the walk-in freezer. 2. One non-food grade take out bag of calamari in the curved top storage freezer. 3. Observed food employee piercing raw shell eggs with a thumb tack. During the inspection, the PIC had the food employee discard the food.
      • The following are in need of cleaning: 1. The floor and walls in all the dry storage areas. 2. The floor drain along the cook line.
      • The following were observed: 1. A bottle of Sunny-D, hat, and earbuds in the warewashing area. 2. Personal cell phone on a cart next to the spices. 3. Personal items in a cabinet over the utensils in the food server station. 4. Employee food stored among the restaurant food in the prep units, reach in coolers, and dry storage areas.
  • McDonald’s, 1 Village Center Drive Fredericksburg, VA 22406
    • Date inspected: Aug. 2
    • Total violations and corrective actions: 1
      • Observed stack of plastic food containers and pitchers stacked while wet after chemical sanitizer on open shelving above the three compartment sink. Worked with the person in charge to separate the containers on the shelving unit to facilitate air drying.

    • Total violations and corrective actions: 5
      • Observed the floors and ceiling vents throughout the facility in need of cleaning. Observed significant accumulation of food debris under the sanitizer basin of the three compartment sink.
      • Observed accumulation of debris on the following nonfood contact surface: Gaskets and exterior doors of reach in refrigeration units, wheels of mobile carts, dry storage shelving, base of 2 door upright freezer and inside fryers.
      • Observed multiple stacks of cleaned and sanitized stainless steel food storage containers staked while wet on open shelving in the warewashing area.
      • Observed 3 sauces used to brush on top of the biscuits which were not labeled to identify the product. The person in charge reported the sauces included a margarine blend, cinnamon topping and oil.
      • Observed absence of a working thermometer in the 2 door upright freezer and drawer cooler.
  • Roma Pizza, 5293 Palivoda Drive King George, VA 22485
    • Date inspected: Aug. 2
    • Total violations and corrective actions: 8
      • The filters in the ventilation hood over the cook's line was observed soiled.
      • The outer surfaces of the grease refuse containers located outside the establishment were observed heavily soiled.
      • The following nonfood contact surfaces have accumulations of grime and debris: 1-the wire shelving in the prep room, 2-the outer surfaces of all reach in coolers and freezers, 3-the outer and inner surfaces of the deep fryer cabinets, 4-the outer surfaces of the plastic insert pans on the clean pot rack (has reminants of stickers on them.
      • Observed the following ready-to-eat, potentially hazardous food being held for more than the specified time/temperature allowed: whole liquid eggs. Eggs were date marked to be discarded on 7-24-21 (9 days ago). Eggs were voluntarily discarded during inspection.
      • Observed the following ready-to-eat, potentially hazardous food being held for more than the specified time/temperature allowed: 1-house marinara sauce (expired 5-3-21 (92 days ago)), 2-house hot sauce (expired 2-19-21 (63 days ago)). Both items were voluntarily discarded during inspection
      • The food contact surfaces of the interior of the ice machine were observed soiled.
      • The interior of the toaster oven was observed heavily soiled.
      • This food establishment does not have an employee, with management and supervisory responsibility, that is a Certified Food Protection Manager.
  • Paradise Diner, 260 Warrenton Road Fredericksburg, VA 22405
    • Date inspected: Aug. 3
    • Total violations and corrective actions: 2
      • Observed food contact surfaces of the meat slicer soiled. The person in charge reported the meat slicer was not used since opening. Instructed the person in charge to clean and sanitize the slicer prior to next use.
      • The person in charge could not provide a Chlorine test kit. The person in charge provided a Quaternary Ammonium test kit, however, the facility uses Chlorine sanitizer both in their wiping cloth buckets and dishwasher.
  • Fredericksburg Country Club, 11031 Tidewater Trail Fredericksburg, VA 22408
    • Date inspected: Aug. 3
    • Total violations and corrective actions: 3
      • Observed the meat slicer soiled. Person in charge said it had not been used on this day.
      • Observed a small cockroach and flies in the kitchen.
      • Observed several lights over three compartment sink and dish machine not covered.

    • Total violations and corrective actions: 1
      • Observed dead pests on the floor in several places and inside of the light shields.
  • Pizza Hut, 35 Walpole Street #101 Stafford, VA 22554
    • Date inspected: Aug. 3
    • Total violations and corrective actions: 11
      • Observed less than 10 foot candles of light at the front of the walk in cooler near the door and less than one foot candle of light at the back of the walk in cooler.
      • The following were observed in need of cleaning: Ceiling tiles in the kitchen, floors and walls throughout especially under equipment and the warewashing area, walls in the kitchen and ceiling vents. Observed presence of dead crawling insects on the floor near the soda syrup rack.
      • Observed the following in poor repair: 1- leak in the ceiling in near the hallway between the kitchen and the warewashing area, 2- one blown out shielded lightbulb above the pizza oven and 3- cracked floor tiles near the pizza prep cooler. The leak in the ceiling does not present a contamination hazard to food, food contact equipment or single use articles. Instructed the person in charge to contact a technician to address the leak and a bucket was placed under the leak to catch the water.
      • Accumulations of debris were noted on the following nonfood contact surfaces: walls of the hood system above the fryers, shelf next to the pizza oven, grate of the top pizza oven, gaskets of the pizza prep cooler, wheels of equipment and floor of proofer.
      • The person in charge could not provide a testing kit to monitor the Quaternary Ammonium and Chlorine sanitizers. The food establishment has a Chlorine testing kit which expired 4/1/2019.
      • Observed spray bottles of degreaser and Peroxide disinfectant stored on top of prep table used to to place seasonings on pizza and stored over single use disposable articles and spray bottle of degreaser stored over the microwave oven on a shelving unit. Chemical spray bottles were relocated to prevent contamination.
      • Observed interior of the microwave oven soiled.
      • Observed the low temperature dish washing not adequately sanitizing food contact equipment after cleaning. Observed the dishwasher sanitizing food contact equipment and utensils at 10 ppm after five cycles. The final rinse temperature was observed at 120 degrees F. Instructed the person in charge to discontinue use of the dishwasher and set up the three compartment sink to wash, rinse and sanitizer food contact equipment until repairs are completed.
      • Observed stack of stainless steel bowls and one rectangular baking pan soiled on open shelving in the kitchen. Soiled equipment was relocated to the three compartment sink to be cleaned and sanitized.
      • Observed the automatic hand soap dispenser at the warewashing area was not dispensing hand soap. The person in charge replaced the batteries during the inspection, however, the dispenser continued to be malfunctioning. The person in charge reported no other hand soap was available in the food establishment and the delivery driver would pick a pump dispenser this afternoon. Monitor hand washing at the kitchen handsink to ensure food employees are adequately washing their hands.
      • Observed two disposable beverage containers without a lid stored on shelf next to and over packaged single use articles near the beverage reach in cooler. The person in charge voluntarily discarded the beverages during the inspection.
  • Popeyes, 1903 Plank Road Fredericksburg, VA 22401
    • Date inspected: Aug. 3
    • Total violations and corrective actions: 8
      • Observed a handwashing sign and paper towels at the sink in the front area. PIC stated the sink is used as a dump sink. During inspection, PIC removed paper towels and sign.
      • Observed 3 soiled stainless steel food containers on a prep table next to dry storage area. During inspection, PIC relocated soiled containers to dishwashing area.
      • Observed 22 small and 7 large containers of coleslaw in the small prep unit that were at improper temperatures (49F - 50F). PIC discarded food.
      • Observed flies in the prep area and mop sink.
      • Observed rusted shelves over the 3 compartment sink next to the dry storage area and homemade shelves (wood) under the front registers.
      • Observed gaps between hood filters and the microwave touchpad is being held together by tape.
      • The following are in need of cleaning: 1. The walls above the 3 compartment sink. 2. Walls, ceilings, and floors throughout the facility.
      • The following are in need of repair: 1. Floor tile of mop sink. 2. The floor tile throughout the facility needs re-grouting. 3. A hole in the wall between the dry storage shelves.

    • Total violations and corrective actions: 7
      • The interior surfaces of the ice machine were observed soiled.
      • The following physical facilities were observed soiled with grime and debris: 1- the floors underneath the shelving in the dry storage, 2-the screen on the dry storage door 3-the floor in the walk in freezer 4-the floors underneath the equipment on the cooks line.
      • The following equipment was noted in need of cleaning: 1-the filter and filter cover on the ice machine 2-the floor fan in the kitchen 3-the top of the coffee makers.
      • PIC was unable to provide a proper test kit for the sanitizer for the three compartment sink.
      • The shelves above the three compartment sink were covered in rust preventing easy cleaning.
      • Observed employees working in the food preparation area without proper hair restraints. Observed 3 employees on the cooks line without beard guards.
      • Observed what appeared to be fruit flies and moths flying throughout facility.
      • The interior surfaces of the ice machine were observed soiled.
