WOODBRIDGE, VA —Several facilities in the Woodbridge area recorded violations during recent health department inspections between March 14 and 26.

Health inspectors conduct most restaurant, school, child care center and other health inspections in person. However, some visits, such as those for training purposes, can be conducted virtually.

Violations typically focus on factors that can contribute to food contamination. The local health department can also perform a re-inspection to ensure that possible violations have been corrected.

For every observed violation, the inspector provides a specific corrective action that can be completed to rectify the violation. Sometimes these are simple, and violations can be corrected during the course of the inspection.   • Employees and conditional employees are not aware of the reporting requirements concerning information about their health and activities as they relate to diseases that are transmissible through food.
  • The food establishment does not have written procedures for cleaning up vomiting or diarrheal events that happen in the facility.
  • TCS FOOD cold holding at improper temperatures. *Tomato, mushroom, grilled chicken and tomato sauce out of temp. 45 - 52.
  • Prep/serving station is entirely out of temp.
  • Observed the following food thawing using an improper method: Grilled chicken being thawed under oven using the heat from the oven to thaw the chicken.
  • Unlabeled powders stored in unmarked bins. (Sugar and flower).
  • No test strips in order to test sanitizing solution used at 3VAT sink.
  • Mop sink has 'Y' connector/splitter is running from the faucet to the chemical connector.

Hibachi Grill & Supreme Buffet: 1965 Daniel Stuart Sq Woodbridge, VA 22191

Violations:

  • Observed employees directly contacting exposed raw beef at the deli slicer that is not in a ready-to-eat form with their bare hands or arms in a manner that could contaminate the food.
  • Observed that hand soap was not provided at each handwashing sink.
  • No disposable towels were provided at the hand washing sinks in multiple locations.
  • A sign or poster that notifies food employees to wash their hands is not provided and/or is not clearly visible at all handwashing sinks used by food employees.
  • The person in charge is not able to provide written procedures for the use of time as a public health control with the following potentially hazardous food item(s): Sushi.
  • Unlabeled food containers with the following food items that are not easily identified by appearance: bulk flour, sugar, salt, and corn starch.
  • Wet wiping cloth buckets improperly stored.
  • The shelving inside the walk-in cooler was observed in a state of dis-repair and condition that prevents necessary maintenance and easy cleaning.
  • There is no chemical sanitizing test kit present to ensure the chemical sanitizing solution used is at the proper concentration.
  • Men's urinal is not maintained in good repair.
  • There are no refuse container at the hand sinks.
  • Observed that the walls in the walk-in cooler is in need of cleaning.

Woodbridge Inn: 1302 F St Woodbridge, VA 22191

Violations:

  • Provide a Certified Food Protection Manager.
  • Review the Food Employee Reporting Agreement with all employees and maintain copies with signatures for all employees.
  • Review with all employees and post the VDH Guidance for Clean-up of Vomiting and Diarrheal Events near the mop sink in the kitchen. Recommend obtaining a Bodily Fluid clean up kit.
  • Remove the hand soap dispenser, the paper towel dispenser and the handwashing sign from the 3 compartment sink area and relocate them to the handwashing sink area.
  • Relocate the hanging clean equipment so that it is not near the handwashing sink.
  • Set the sink up as follows: Sanitize/Rinse/Wash. OR Provide a splash guard in between the handwashing sink and the closest basin (if it will contain clean equipment).
  • Provide a 1" minimum air gap between the wasteline of the 3 compartment sink and the floor drain as discussed.
  • Provide plugs/stoppers for use in the 3 compartment. Plastic basins inserted into the sink basins being used currently.
  • Provide a test kit for measuring the concentration of sanitizer in the 3 compartment sink.
  • Provide thermometers in the three (3) refrigeration units in the bar area.
  • Provide 50 foot candles of light at all food preparation areas to allow employees to see for proper cleaning and handling (cutting) of food.
  • Provide light bulb covers (sleeves) over the 4' light bulbs directly over the prep reach-in refrigerator to protect foods from contamination of glass should the light bulbs break.
  • Provide a mop hanging rack at the mop sink.
  • Provide a covered waste receptacle in the women's restroom for the disposal of feminine articles.
  • Provide a self closing device on the door to the men's restroom.
  • Replace missing front panels on the bottom front area of both the True 2 door upright refrigerator and True 2 door upright freezer.

D'rocco's Virginia Grill: 301 Mill Street Occoquan, VA 22125

Violations:

  • There is no person in charge with a valid Food Protection Manager Certification from an accredited agency recognized by the Conference for Food Protection.
  • Management, food employees or conditional employees were not aware or informed of their responsibilities in reporting symptoms or diagnosis of diseases transmissable through food.
  • This food establishment has not established procedures for cleaning up vomiting or diarrheal events that happen in the facility.
  • Hand sink in kitchen observed with food debris (indicating sink being used to dump food in). Hand sink in basement next to 3VAT sink is clogged with food.
  • Salmon can be cooked to order - no parasite destruction confirmation from supplier.
  • Disclaimer concerning the consumption of raw/undercooked meat and seafood is on the menu - however - there is no ' * ' indicating which foods can be cooked to order.
  • Cutting boards throughout kitchen observed scored and carved up. Discoloration and cuts indicate the boards require soaking in chlorine or sanding.
  • Non-food contact surfaces observed coated in food debris, dirt, grease and other contaminates. Opening the drawers in order to get food can begin cross contamination.

  • Mop sink faucet has a 'Y' connection running to soap/sanitizer with water pressure controlled via hose and not faucet - bypassing the backflow prevention.

  • The outer opening of the food establishment is not protected against entry of insects and rodents (vermin) as the solid outer opening back door does not seal properly at the bottom.
  • Food (granola) was observed to be stored on the floor in the back area of the kitchen.
  • An employee handling food was observed to not be wearing a proper hair restraint.
  • Single service napkins were stored on the floor in the back area of the kitchen.
  • The True 2 door upright refrigerator in the back was not holding a temperature of 41 degrees F or below and the seals around the doors are in visible disrepair.
  • The interior of the chest freezer was visibly soiled with large accumulation of food debris as well as ice build up.
  • The hose attached to the mop sink faucet was observed to be fully extended into the basin thus creating a potential backflow issue.
  • The sprayer arm at the prep sink out front extends down into the basin; the end of the fresh water inlet at the dipper well is completely submerged into the water.
  • The floor in the back kitchen area is in disrepair with several tiles broken and/or missing completely.
  • Wet mop observed stored on floor near mop sink with accumulation of water on floor.
