Calling all researchers and clinicians with an interest in maternal health. Postpartum Haemorrhage (PPH) affects millions of women every year and accounts for over 20% of all maternal deaths reported globally. The Second Global Call from the World Health Organisation is out to develop a robust evidence base for the definition of postpartum Haemorrhage (PPH). How much blood loss is too much and when do we define PPH to promote early intervention. There is substantial disagreement on several domains, including the amount of blood loss, time of measurement, inclusion of clinical signs and symptoms of haemodynamic instability, and mode of delivery. As the requirement for surgical intervention and blood transfusions increases exponentially when blood loss hits critical levels this work will be invaluable to help define new thresholds for conservative and refractory PPH management. What is PPH (Post Partum Haemorrhage) and why should we care about it? PPH represents a critical condition characterised by severe bleeding that typically arises within a timeframe spanning from 24 hours to 12 weeks after childbirth and is a main cause of maternal mortality. • The global prevalence of PPH ranges from 7.2% in Oceania to as high as 25.7% in Africa. Each year, approximately 14 million women experience PPH, leading to an estimated 80,000 maternal deaths across the globe. • PPH is a serious condition that, when left untreated, can result in severe consequences, including complications related to blood transfusions, hysterectomy, disseminated intravascular coagulation (DIC), consumptive coagulopathy, multiorgan failure, organ failure, hypovolemic shock and death. • PPH is defined as an estimated blood loss exceeding 500 ml. Severe or secondary PPH is defined by a loss of 1000 ml, while massive, life-threatening PPH is attributed to a blood loss exceeding 2500 ml or the presence of hypovolemic shock. • There are a wide variety of risk factors that contribute to the occurrence of PPH, including a previous history of PPH, uterine inversion, delivery-related trauma of any cause, placenta praevia, placental abruption, uterine rupture, and multiple gestation. • A primary cause of PPH is uterine atony (insufficient contraction of the uterus following childbirth).
