Accident and incident investigations should consider why human failures occurred. Finding the underlying causes is key to reducing the risk of similar adverse events, whilst lessons learned provide insights to effective improvements. Join us in September for our ‘Human Factors in Accident and Incident Investigation’ training course which gives delegates knowledge and practical support for investigating human factors issues, helping them to find ways of achieving effective change within their own organisational context. HSE as the Building Safety Regulator (BSR) is the building control authority for all higher-risk building work in England. Developers, clients and others who need building control approval for projects in scope must apply to BSR rather than local authorities or private building control. Ensuring that your application clearly demonstrates compliance with building regulations is crucial for a smooth approval process. “Our hearts are broken" The heartfelt words from Thomas Brooke's grieving wife. On 29 April 2019, Tom was walking across the yard when he was struck by a JCB wheel loader. He tragically died from his injuries. At the time of the incident, Tom’s son Rupert was aged four. Describing the relationship Tom had with Rupert, Jessica said: “They were as thick as thieves, both being at their happiest when in the other’s company." Our investigation found that John Brooke (Timber Treatments) Limited did not properly organise its workplace to keep pedestrians safe. “I have suffered a loss of independence.” Caroline Leafe was walking her dog with husband Kenneth along a deer path in Ashdown Forest when she was struck by a tree. She suffered a traumatic brain injury, a fractured collar bone and fractured ribs as a result. The tree fell onto Caroline and Kenneth after it was cut into by a forest ranger. A HSE investigation into the incident found The Conservators of Ashdown Forest, the body that manages Ashdown Forest, failed to identify the risks to members of the public from tree felling. This meant precautions, such as posting warning signs and using barriers and banksmen, were not implemented. Read more here: https://lnkd.in/eRkeHRDC

Falls from height remain the leading cause of death within the construction industry. A sole-trading builder from London was given a suspended prison sentence last week after he put workers at risk due to unsafe scaffold. The scaffold at the site was found to be unsafe and at risk of collapse by HSE inspectors. David Beadle also failed to comply with the terms of a prohibition notice and as a result was ordered to complete 150 hours on unpaid work by magistrates in Bexley.
