How can you revise your performance management policies to be more effective?
Good performance management processes enable the business to clearly identify levels of performance (business and personal). KPIs or <insert other corporate framework> are great for setting the high level business goals and objectives, but when it comes to individual performance there is no substitute for continuous and meaningful performance conversations. These conversations should focus on the performance of an individual in their specific role and take place between managers and HR. They allow for a more proactive approach to performance management, where issues can be dealt with at source rather than waiting to see if the KPI dials are moving, then retrospectively asking why they are or are not.

How can Employee Relations professionals help employees set goals that are both challenging and achievable?
Continuous assessment of performance is the way forward. A regular, scheduled performance 'pulse' check between HR and managers can provide very useful data which can help the business make informed decisions. It doesn't need to be complicated, simply a regular assessment of performance using whatever metrics you choose (high-medium-low is about as much as you need to know). The reason(s) for any performance anomalies can be discussed in the pulse check, and actions agreed if necessary. As always, clearly communicating this information to staff is essential as it helps align expectations.
