    Considerable attention has been paid to the role that healthcare systems have in combating climate change.1 Recent analysis has calculated the global carbon footprint of healthcare as equivalent to 2-2.4 Gt of CO2, about 4-5% of total global emissions.

    The focus is often on reducing emissions in high-income countries (HICs) and adaptation in low and middle-income countries (LMICs). Few LMICs are included in studies on carbon emissions from healthcare.

    This imbalance is understandable, given the greater relative contribution to greenhouse gas emissions from health systems in HICs and the disproportionate burden of climate change impacts on LMICs. Nevertheless, healthcare in LMICs is a carbon-intensive activity and is likely to grow with the delivery of commitments to universal health coverage (UN Sustainable Development Goal 3.8).

    The objectives of Climate-ADAPT are (1) to facilitate the collection, sharing and use of information on climate change impacts, vulnerability and adaptation, and build a consistent and updated knowledge base; (2) to assist the effective uptake of the relevant knowledge by decision-makers; and (3) to contribute to a greater level of coordination among sectors and institutional levels.

In order to optimise this, this study evaluated and reconceptualised a sustainable development assessment tool for health and care settings in England.

    User/expert involvement has enabled an informed approach to a reconceptualisation of a sustainable assessment tool for health and care settings. The tool will support organisations to build their mandatory Sustainable Development Management Plans, as part of the National Public Health Outcomes Framework. Alignment of the tool to the UN’s SDGs provides an opportunity for health and care organisations to demonstrate accountability and progress against the UN’s set of transformational goals. Furthermore, the tool could be adapted to other public service providers.

This chapter offers a replicable model that other business schools and universities could integrate into their core curriculum while also enriching their educational methods (PRME Principle 3). The focus of the described Greenhouse Gas Management project is on the UN Sustainable Development Goal (SDG) #13 (Climate Action), but the partnership model could also be used to address other SDGs. The model is based on the assumption that students can contribute to solving sustainable challenges. Instead of learning frameworks for later use in life, students directly apply the concepts they encounter in a real-life business. In this chapter, the deployment of such work-based learning in the core curriculum at Nottingham Business School is described, along with details of how this aids the development of employability skills of the participating students

    This report is the first in a series of annual publications from the SDU looking at progress of sustainable development across the health and care system.

    Summary of findings
    Overall organisations are cutting their carbon footprint and saving money through reducing energy use, but having less success in addressing water use and the increased costs from waste disposal. Progress in sustainable approaches to commissioning, procurement and across the social care sector is more difficult to measure and more needs to be done in this area. Sustainable development is increasingly becoming a core part of work. More organisations have board approved Sustainable Development Management Plans (SDMPs) and are reporting annually on their work.

    National Organisation Commitments
    The report also includes - for the first time - commitments from nine major health organisations to deliver an environmentally and socially care system. The national organisations making their pledges are the Department of Health, NHS England, Public Health England (PHE), NHS Improvement, the Care Quality Commission, Health Education England, the National Institute for Health and Care Excellence (NICE), NHS Digital and NHS Property Services – in partnership with the Department of Health.

These include providing mental health services to people being treated for physical health needs in hospitals, encouraging staff to travel to work by bike or foot and increasing use of technology for communication – for instance between staff and for patients with long term conditions.

    The first carbon footprint for any NHS region in the UK
    This report has identified a baseline regional carbon footprint for the East Midlands using bottom-up Trust level data to complement the national footprint produced by the NHS Sustainable Development Unit.
    The footprint for 07/08 has been calculated as 967,900 tonnes CO2 equivalent (tCO2e) and for 08/09 as 1,070,000 tCO2e. The 07/08 footprint represented around 5% of the national NHS carbon footprint. By comparison, around 8% of the national NHS budget (including salaries) was spent in the East Midlands in 07/08.
    While the national footprint was based largely on 07/08 financial data, it has been found however that to produce a bottom-up footprint from NHS organizations data, the 07/08 data was not of sufficiently good quality to produce a reliable footprint, so the 08/09 footprint is recommended as a better representation of the regional baseline.
    In general, the spread of the region's CO2e emissions across emissions areas is broadly in line with the spread shown at the national level. Buildings emissions in the region however show a higher prominence than the national picture reflecting a higher than typical coal consumption in NHS organizations in the region. Coal accounts for 15% of the region's waste and utility-related emissions. These emissions are focussed on a small number of large coal-fired heat plants. Working to reduce the NHS use of coal in the region is thus covered in one of the recommended regional Phase II pilot projects.
    While the regional picture produces some useful results in itself, this work has drilled down further to an organization by organization and site by site analysis, to identify hotspots and areas for future work within NHS organizations. The level of data within the report should be of use to individual NHS organizations in this regard.

    This Strategy and the associated Action Plan identifies the key technologies and programmes required to enable Nottingham to play its part in meeting the national and local targets on carbon reduction and low or zero-carbon energy generation. The Strategy sets the headline targets and key strategic priorities; the Action Plan details how the City can meet these targets. Implementation of the Strategy will accelerate Nottingham’s development as the UK’s leading City in low carbon energy security.

    Leading on the health and care sector wide report on adaptation to climate change. The report is part of the rolling 5 year national legal obligations under the Climate Change Act - Adaptation Reporting Power. The work requires national stakeholder engagement and assessments of direct and systemic risks from extreme weather, accross a sector that includes in the region of 45,000 organisations employing around 2.5million staff.

  • Chair of National Working Group on Low Carbon Inhalers

    Asthma inhalers in England are responsible for about 850,000 tonnes of CO2e per year, more than the entire emissions of all NHS business travel, and greater than the carbon footprint of many small countries. This is due to the fluorinated gases used as propellants. I chair the national cross healthcare system working group to reduce this impact.

    I commisioned NHS Employers to establish a successful national engagement programme and awards (linked to the 10 categories of the Health and Care SDA Tool) to raise the profile of sustainable healthcare. The programme has been highlighted by the UK UNSDG advocate and Govt. Ministers.

  • Core Carbon footprint of NHS Trusts and Foundation Trusts – emissions per full time equivalent

    Development of an approved carbon indicator that provides a national quantification of the carbon impact of core emissions by NHS providers per FTE. This allows measurement of progress by NHS providers emissions linked to the potential fluctuation in staff which provides both a proxy for changes in activity e.g. patient activity but also allows an organisation to normalise its emissions based on its own growth or reduction

    I commissioned and set scope for the Natural Resources Footprint for the health and care system in England. I extended the scope from carbon only to include the first ever health system wide water footprint worldwide, as well as including air pollution and waste to allow a more balanced approach for targetting priority work at a national and local level.

    I gave evidence before parliamentary committee session focused on UK progress to date in reducing F-gas emissions and issues associated with enforcing relevant regulations. It examined the options facing the UK after it leaves the EU, in terms of maintaining or going further than existing targets and what it will mean in terms of devolution.

    The second panel concentrated on the issues surrounding a move from Metered Dose Inhalers (MDIs), which contain F-gases with a high global warming potential (GWP), to lower GWP alternatives.

    Millions of MDIs are prescribed each year for a range of respiratory conditions and have been identified by the NHS as representing a carbon ‘hot spot’. The Committee is interested in this as a good example of an area where progress on reducing F-gas emissions can be made.

  • Heath Outcomes of Travel Tool

    I commissioned the creation of a detailed interactive air pollution footprint and resource for the NHS in England. The resource provides editable baseline impact data to a trust and health geography level as well as national summary.   • DG Cars Sustainability Options Assessment

    Outline assessment of the environmental impact of DG cars, Nottingham's largest taxi firm. Shows that the company's vehicles contribute around 3% of the city's transport carbon emissions. This summary report outlines how DG might position start to itself in relation to increasing demands for lower carbon, lower air pollution services from large clients.

  • Phase 2 East Midlands Regional NHS Carbon Reduction Programme

    In 2011 I and my team were successful in bidding to deliver 3 significant carbon reduction projects for the NHS in the regions.
    1. Sustainable care homes- A carbon footprint for residential care in the UK
    2. Sustainable procurement workshops
    3. Energy in Estates, AMR, Voltage Optimisation and renewable energy

    The projects combined to achive annual savings for the NHS of around 2,400tCO2e and about £1.4m.

    In Mid 2008 I was commissioned by NHS Nottingham and NHS Nottinghamshire to develop and support delivery of full organisational carbon management plans. The project took the form of a 3 year programme for which I recruited a small team. We initially developed a quick wins estates investment and carbon reduction plan from site surveys, this was approved and delivered. We then went on to develop the first full scope 1,2 and 3 carbon footprints for any NHS Trusts in the UK. We continued to work with the 2 trusts throughout programme delivery until April 2012. To date both Trusts have achieved 25% cuts in CO2 emissions, saved up to £800k per year in natural resource costs, achieved recycling rates of over 85% and installed in excess of 20 solar panel systems. In 2011 The Trusts and NEP were awarded the Global British Medical Journal award for 'Sustainable Healthcare' for this project and our long term public health partnership. I and my team continued to provide transitional support to the 2 Trusts as they handed over their commissioning activities, estate and staff to the new NHS commissioning and primary care structures.
