When Dr Alex Harborne joined Tower Hamlets Community Health Services as Clinical Director in 2022, she heard the same story from every team she met: a tangible disconnect between primary care and CHS and a high proportion of rejected referrals. She therefore decided to start a QI project to address this complex challenge. In this video members of the project team reflect on the impact of the resulting work: https://lnkd.in/g5QFcQg3 East London NHS Foundation Trust Dr Alex Harborne

The project involved the co-design and testing of over 25 change ideas across the Trust. The result: tangible changes to the environment including reductions in several areas of violence (physical, verbal, sexual, racial).

Staff on Sally Sherman Ward, which provides continuing care to those with dementia and complex needs, were determined improve safety – they recognise the devastating impact that falls can have for patients and their loved ones.
