The following incident exemplifies the importance of detecting gas release and preventing potential escalation. On a gas compressor on an offshore installation, the pressure safety valve bolts had been in place since installation - approximately 10 years ago. The loading and unloading of propane requires specific design and operational procedures, as illustrated by the following incident. A leakage occurred during the unloading of a truck containing liquid petroleum gas (LPG) in a chemical plant. The LPG caught fire and heated the truck, resulting in a boiling liquid expanding vapour cloud explosion (BLEVE). The following incident illustrates the importance of ensuring that hazard identification and risk management are included in safety training. A warehouse and yard worker was rolling a flex stabiliser on a sawhorse rack. One end of the tool dropped from the rack, causing the opposite end to sadly fatally strike him. It's crucial to make sure that risk assessments are conducted with the right expertise to address potential risks at your facility, as demonstrated by the following incident. During a major shutdown at a facility, a venturi effect drew hydrocarbon from the flare warm wet gas riser/pilots into the air annulus towards the spare flare. A flammable atmosphere was then created and ignited, leading to a deflagration followed by a detonation, which caused the flare warm wet bellows to rupture. The following incident demonstrates how important it is to ensure that alarm systems prioritise critical alerts. A cargo vessel collided with a wind turbine generator (WTG) foundation and caused significant damage. No technicians were present at the WTG since the collision occurred late at night. The vessel did not raise the alarm or report the incident, and it was only observed by maritime authorities when the vessel arrived in port the next day. It's important to make sure that Step 7 conversations (the Assist and Assure programme) sufficiently cover all jobs, even those considered routine. In this incident, while shifting stock fence panels, the middle finger of a worker was pinched between two panels, and sadly the finger was amputated. This tragic incident demonstrates the importance of implementing comprehensive combustible dust control procedures at facilities. A series of explosions and fires occurred at a sugar dust refinery. Explosions occurred throughout the building as a result of sugar dust accumulating across the facility and meeting an ignition source, which caused a chain reaction of secondary explosions. Electrical grounding and lightning protection systems should be installed in all facilities, and the following incident serves as an example of that. During an electrical storm, lightning hit a mobile crude oil storage tank and lit up the gases coming out of a vent, starting a fire. The fire was put out without any serious damage to the facility, however it could have resulted in harm to workers as well as the facility.
