A family has revealed their 'lives have been ripped apart' after a rising footy star was identified as the 18-year-old who lost his life in a car crash in Perth on Saturday night.

Nick Campo, 18, a player for South Fremantle Football Club, was sat in the rear of a Toyota Hilux ute when it crashed and rolled at about 11.40pm on Farrington Road in North Lake.

The ute then smashed into a black Jeep patriot SUV that was travelling in the opposite direction.

Two of Campo's teammates were in the ute and taken to hospital, but Campo has since died.

The driver, a 17-year-old, was also taken to hospital, while the 26-year-old male driver and 27-year-old female passenger of the Jeep suffered minor injuries.

Cameron Britt, CEO of South Fremantle, said the club was in 'deep shock' following the tragedy.

'South Fremantle Football Club is deeply saddened by news of a car accident overnight involving several of our Colts players, resulting in the tragic death of South Fremantle footballer Nick Campo,' he said.

'We are a club in deep shock, focused upon supporting our players, staff and their families in these tragic circumstances.

'We ask that all involved are given the appropriate space and respect to grieve.'

Campo's parents have also shared their heartbreak following the accident, saying their lives had been 'ripped apart'. .

'In the early hours of this morning our beautiful son Nick passed away after he was involved in a tragic car accident,' they said on Facebook.

'Our lives have been ripped apart, 18 years old, a future with so much hope taken away, we are praying others in the car involved will be ok.'

Campo had played four matches for the Colts this year and was involved in last weekend's loss to Claremont.

During round 13's win over East Perth, Campo was one of the best on ground with 26 disposals.
