    On September 29, 2015, a container ship named the El Faro was traveling from Jacksonville, Florida to San Juan, Puerto Rico. The El Faro was two football fields long and normally traveled the 1,300 mile route directly from the northeast corner of Florida to San Juan. However, because of the approach of a hurricane, the crew decided to take a more sheltered route via the Old Bahama Channel.

    The crew of the El Faro knew the ins and outs of navigating the Atlantic Ocean, and they had done so many times before. But on this voyage, the captain chose to take a more dangerous route, which would result in the ship getting caught in the eye of Hurricane Joaquin.

    The captain of the container ship El Faro decided to take the exposed route through the northern Bahamas, which would have been the Atlantic route. However, this decision was a poor one, as it caused the ship to take on water and sink.

    The captain of El Faro was operating in a permission-based environment. He was trapped into conforming to the role of an obedient bureaucrat.

    The third mate, on watch, reported the storm’s location and suggested that the ship change course, but the captain refused. He said that the winds were not strong enough to warrant a change in course, and that the third mate was just nervous.

    The crew of the 790-foot long El Faro knew they were in trouble when the winds increased to over 100 miles per hour and the ship began taking water. They discussed their options, some of which included turning south to get away from the storm, but the second mate was afraid she would be fired for changing course.
