I can't believe how many times I have seen people try this and fail to understand how to do proper RCA ("Root Cause Analysis"). This is much simpler than it seems. The best approach that I have found in my career is called the 5 Whys; this technique was invented by Sakichi Toyoda, the father of Toyota Industries: "By repeating why five times, the nature of the problem as well as its solution becomes clear." It is as simple as becoming a child again. All you have to do is ask why, and when you get an answer, just ask why on the answer. Do it five times, and you will get to the true root cause of a problem. The 5 Whys framework is so useful and simple that even the NTSB uses it when investigating accidents and other incidents.
