Dentro del plan anual de capacitaciones es necesario que el empleador garantice a los trabajadores una jornada mediante la cual se de a conocer la responsabilidad de identificar los peligros en su lugar de trabajo, incluyendo además metodologías que les permita el reporte oportuno de dichos factores de riesgo y el manejo adecuado de situaciones de emergencia. El sistema de gestión de seguridad y salud en el trabajo aplica a todos los niveles de la empresa, pero es responsabilidad del empleador generar la cultura del autocuidado y propiciar espacios de entrenamiento adecuado para para la prevención de riesgos laborales.

Para estar seguros que se reportan comportamientos riesgosos o evitar que se materialicen eventos debemos tener a nuestros trabajadores informados de las políticas respecto a este tema y sus actualizaciones, sin embargo, esto no es suficiente, las demostraciones juegan un papel importante a la hora de crear conciencia sobre los riesgos y los peligros a los cuales nos vemos expuestos en el sitio de trabajo, entender cuales pueden ser sus consecuencias y como seguir adecuadamente los procedimientos de seguridad que hemos adoptado para prevenir los accidentes o incidentes y promover el comportamiento seguro. La seguridad no solo es una actitud, también es un comportamiento que se refleja en la cultura de la organización.

In my opinion Using a digital platform is the best way to ensure proper registering and reporting of all unsafe observations, and effective follow up for closure, and analysis afterwards to get insights about areas of improvement, to have a history that can be recalled and reviewed for auditing evidence

I agree with Ahmed. Creating an online reporting platform is a great way to report safety hazards. A tool that also would be able to alert the Administrator of urgent matters immediately after a report.

To ensure all team members know how to report safety concerns, I think that joint inspection and coaching them how to identify the hazards, near miss/ safety concerns will be helpful as a base. Beside the following: 1- We should provide clear, instructions on the reporting process. 2- Use straightforward language and practical examples. 3-Conduct regular training sessions and create easy-to-access written guidelines. 4-Encourage open communication and make sure there are multiple reporting channels available. 5- Regularly review and reinforce the procedures to keep them top of mind.

Actions taken after an hazard has been reported is very important in building a good reporting system which in turn helps in developing a strong safety culture. When an hazard has been reported, there should be transparent and all-inclusive steps taken to analyze and control such hazard with the entire team of that section involved to emotional satisfaction is gained by the team who reported such hazard in the first place; this will help in ensuring continuity in reporting of hazards and concerns. Follow-ups should be made to ensure any control measure put in place is highly effective as analyzed.

I would like to adopt the concept of "Deming Cycle" into the establishment of feedback loop in my company. Personally not into public recognition. Again juniors, new employees or apprentices might not report things if they are to be mentioned publicly

Behavioural based safety is a layer that often gets missed, coupled with systems thinking. A great example of this is you will often see KPI’s on visual management boards asking for number of hazards identified. This is a must to be actively hazard hunting but the missed focus or double click into this is, “what was the behaviour or system deficiency that caused the hazard to be present” this is the mindset shift and bias to action to attempt to fix the root cause whether behaviour based or systems based.

Give people different ways of reporting them... verbal may be good for one but poor way to communicate for another. App based may be good for one but poor for an older or tech "unsavvy" employee... You need to allow different ways of communicating these issues up to the right people. Much like consultation... different strokes for different folks.
