This is a very good topic. I believe that the best way to prepare for post acute needs starts with assessing the potential pathways when the patient enters the hospital system on admission. Having a person in place to serve as a liaison would also be helpful to ensure that the gaps are closed along the way.

The term post acute care is not to yet a household name in my country although the concept is practised. Currently, patients are discharged upon reaching a stable condition as assessed by their attending physicians. Follow-up clinics are scheduled based on factors such as the severity of the patient's condition, their progress during admission, and patient preferences. For instance, if a patient receives comprehensive care from both doctors and physiotherapists during their hospital stay, they are discharged by the doctors, and a follow-up clinic appointment is provided both by physiotherapist for continued rehabilitation from home.

Letting patients know that their input into their care is important and their decision. “ You are still responsible and in control of your care.”

While post acute care is incredibly important access to it is absent or lacking in many rural areas. It is vital that health systems evaluate regional resources and access as well as any potential challenges to educate both staff and patients and improve outcomes.

My mother often went to Riverwoods in Lewisburg, PA to visit other relatives and friends who were living there. She often said that if time came when she needed senior care, she wanted to live at Riverwoods. Of course, my family, researches for symptoms, possibilities and possible remedies beforehand. I was surprised to learn there are different brands of artificial joints and some of them have had multiple recalls. As a patient, I pointed out to my surgeon which brand I'd prefer. My Mom has recently moved from her home to Grand Oaks Assisted Living. She has a cozy beautiful apartment. Most important to Mom and me: It is her home and the team of staff there show so much respect as they provide care in her home. It is important every day that the nursing staff, care staff, chef, servers, administrator, activity director, front desk team are absolutely exceptional. It is her home first, and second they help set up and take her to appointments; and then immediately order and implement changes in meds and home health orders. The Activity Director matched her with other residents who immediately became friends. They meet daily for meals and watch out for each other. Around every corner, I see smiles.

Don't wait until a crisis; by visiting when you're well, you can make informed decisions with a clear mind about what you want and where you'd like to go when the need arises. Discharge planners are often forbidden from recommending specific providers, which does a disservice to the patient. Simply providing a list gives no direction. Patients need someone to help them choose quality providers instead of whichever is closest to home, or the first one that says they will admit. The fact is that some providers should be avoided and patients will not get good care from them. But patients do not know if no one tells them. They find out the hard way. Medicare ratings are often outdated and inaccurate. Asking the provider won't get you an honest answer; no business is going to admit their service is poor. There is rarely time for touring potential placements before discharge and transfer.

Yes, some folks are fully capable of making informed choices based on their ability to research concerns. That said, I implore providers to truly get to know those they are caring for! In our rapidly aging world it is the responsible thing to do. A great number of aging patients do not have access or the ability to do self research nor coordinate their own services.
