Accountable for Health (A4H) is a national advocacy organization accelerating the adoption of sustainable and effective accountable care to improve health care quality and outcomes and lower costs. We represent a broad, diverse group of accountable care stakeholders working to improve the way health care is delivered to people across the county, across various payers, programs and delivery models. A4H provides advocacy, research and communications to develop models that improve care, reduce fragmentation in care delivery and lower costs. Accountable care models make providers responsible for quality, patient experience and cost across all payers and program types.

In June, The Centers for Medicare & Medicaid Services (CMS) announced the practices in Colorado, Massachusetts, Minnesota, New Mexico, New Jersey, New York, North Carolina, and Washington that will participate in the Making Care Primary (MCP) Model. MCP aims to improve care for beneficiaries by supporting the delivery of advanced primary care services, which are foundational for a high-performing health system. The Health Care Transformation Task Force (HCTTF) and the National Association of ACOs (NAACOS) have released a new resource addressing how policies can be strengthened to enhance beneficiary engagement in accountable care models. Effective communication is crucial for patient involvement in ACO governance, care redesign, and personalized care planning. Many patients are unaware of their participation in ACOs, and current policies can be confusing. A roundtable with ACO and patient representatives developed policy recommendations to evolve CMS accountable care models, focusing on beneficiary communications and education, beneficiary engagement in care delivery redesign, and beneficiary participation and input in ACO governance.
