Last week, AHI's Director of Planning & Development, Jennifer Muthig, joined a ride-along with Kelly, a paramedic from the Cambridge Valley Rescue Squad as part of their Community Check Program. Jennifer had the opportunity to visit with consumers and gain firsthand insight into the paramedicine services that help individuals stay in their homes. These visits are crucial for monitoring health metrics like blood sugar levels, lung capacity, heart rate, and managing medication changes post-hospitalization, all while ensuring the consumer's primary care doctor is kept in the loop. The program also addresses the mental health needs of consumers, discussing the importance of recognizing and treating depression, especially for those with limited mobility, and even provides guidance on using technology to order groceries. By tracking consumers in their homes, the Community Check Program offers a proactive approach to reducing emergency department visits. This experience will enhance our understanding of the needs of our aging population and strengthen our efforts to provide the essential social supports that allow them to live and age in the place they call home. This past Friday, the AHI and Adirondacks ACO teams gathered in beautiful Bolton Landing to celebrate summer, our achievements, and the incredible contributions of Mary McLaughlin, who is heading into a well-deserved retirement. 🎉 The day was filled with fun for the whole family! Yesterday, our Telehealth Project Manager, Nancy DelMastro, attended the NYS Digital Equity Network Convening in Syracuse with about 50 stakeholders invested in the future of broadband for all New Yorkers!
