Employers increasingly are turning to medical stop-loss captives to manage healthcare risk and control costs, with total premium in stop-loss captives more than tripling since 2020. But not all captives are created equal, with wide variances in administrative costs, cost-containment opportunities, and the percentage of unspent claims dollars returned to the employer. The differences can mean as much as 35 percent or more front load (administrative) cost, and, thus, that much less underwriting profit. Furthermore, the tax treatment of underwriting profits is drastically different depending on the captive model. In this instructive and eye-opening discussion, Nelson and veteran captive manager Phil Holowka compare the differences among the various captive designs and give pointers on how to determine the right captive for an employer.

When 20% of medical diagnoses are completely wrong, this poses a severe threat to the life and health of patients. Most misdiagnosis can be caught and corrected simply by requiring an independent third-party medical second opinion. While second opinions are an obvious solution to the risks of misdiagnosis, NOT ONE of the BUCAH insurance companies require a second opinion for serious diagnoses. This policy puts 20% of their members at serious risk. NextGen Benefits advisers require second opinions as part of the plan design, to protect employees and avoid the cost of wrong or unnecessary care. Imagine a provider network of physicians and hospitals with an industry redefining level of transparency for all stakeholders and financial control for employers. Imagine controlling costs on high-dollar in-patient hospital procedures…without the need for RBP. A transparent network landscape where the healthcare providers clearly can see the employers and advisers they’re serving. And, likewise, employers and advisers can see the providers that their health plans love to use and easily can determine which providers should be available to plan members for zero-dollar out-of-pocket. Join host Nelson Griswold and guest Morgan Smith, Coral Platform COO, as they discuss the new Unity “un-network” network that will redefine your concept of “network.”
