Finally, full-service healthcare comes home. At Home Harmony is built to enable seniors and individuals who are disabled to thrive at home. Our integrated model is made up of a full-service pharmacy, in-home medical practice, remote patient monitoring technology, and personal care. We are the perfect local pharmacy partner and personal care agency for Central VA's Assisted Living and Independent Living communities! Our services are covered by Medicare. We can serve as a client's primary care provider, or we can coordinate with existing doctors and can keep family members updated as often as they'd like notifications. Have you met Scott, one of our stellar home-based care providers? With a master’s degree in nursing, N. Scott Owens got his start in healthcare as a medical social worker before earning his master’s degree in nursing from the University of Alabama at Birmingham. He has served patients in a number of settings, including primary care, free clinics, palliative care, and geriatrics. Scott was born in Richmond and earned his undergraduate degree in social work from Virginia Commonwealth University. We are so grateful for the care you provide our Harmony patients, Scott!

Medicare's new GUIDE program is LIVE today! We are incredibly proud to have been approved to participate in this transformative program in which Medicare is covering personal care via voucher for the first time. Now, through the GUIDE program, in addition to our other home-based care services, we can offer the following special services for patients with dementia: 🧡 Added services for the cognitively impaired and their unpaid caregiver 🧡 Personal care - for the first time, up to 80 hours of personal care can be covered by Medicare! 🧡 Family and caregiver training and support Call 804-210-3103 to learn more about how we can GUIDE an improved dementia experience for you and your loved one with cognitive impairment. This program is available to those with Original Medicare.
