CMS is pleased to announce that the States Advancing All-Payer Health Equity Approaches and Development (AHEAD) Model has announced the first round of state participants: Connecticut, Maryland, and Vermont. Hawaii will also participate, upon satisfaction of certain requirements. CMS and participating states will operate the AHEAD Model in partnership to curb health care cost growth; improve population health; and advance health equity by reducing disparities in health outcomes. CMS will issue Cooperative Agreements to up to eight states, each of which will have the opportunity to receive up to $12 million from CMS during the first five and a half to six years of the model to support implementation. CMS has released the evaluation findings for the first five performance years of the Vermont All-Payer ACO Model: https://lnkd.in/eq83qWGD. This model aims to provide Vermonters with better medical care by tying payment to the quality of care given, rather than the number of tests, procedures, or office visits. This is also known as #ValueBasedCare: go.cms.gov/VBC. Providers emphasized the model’s value in providing a focal point for collaborating on care delivery transformation and population health initiatives. Overall, the model improved understanding and acceptance of value-based care among providers and inspired collaborative population health initiatives.
