Developing Individualized and Comprehensive Strategies Empowering Sites to Achieve Cardiometabolic Excellence
About us
Founded by Saint Luke's Mid America Heart Institute. Instead of addressing cardiovascular, diabetes, and kidney conditions or risk factors separately, the Cardiometabolic Center Alliance has taken a novel approach to treating these comorbidities together—providing team-based, comprehensive care specifically tailored for each patient. The Cardiometabolic Center Alliance provides its members with the proper training, protocols, and resources needed to establish their own Centers of Excellence. As the Alliance enhances members' ability to measure outcomes, improve processes, and expand research opportunities on a national scale, the ultimate benefit is improved patient outcomes. By expanding this clinical model of care, our vision to help patients better manage their conditions for a better quality of life and to ease the burden on healthcare organizations will become a reality. To improve the quality of care and outcomes for patients—no matter where they live—we seek to partner with health care organizations, academic and professional societies, and other key partners nationally.

The Cardiometabolic Center Alliance was founded in 2020 with the overarching mission to improve the quality of care and outcomes for patients living with CKM (cardiovascular-kidney-metabolic) disease. We are excited to be working alongside the AHA on the development of a CKM certification program for healthcare practitioners and health systems. This initiative will greatly support ongoing advancements and improvements in CKM care delivery.
A third of U.S. adults are at high risk for CKM syndrome, but many patients receive fragmented care. Today we are proud to launch an initiative to change that with American College of Physicians, American Diabetes Association, American Society of Nephrology, National Kidney Foundation, STOP Obesity Alliance at The George Washington University- Milken Institute School of Public Health and Cardiometabolic Center Alliance. Over the next four years, the Cardiovascular-Kidney-Metabolic Health Initiative will work to implement a person-centered approach to awareness, professional and patient education, and treatment of #CKMSyndrome. #cardiology #nephrology
