So grateful that MACPAC shared this Access brief, focusing on NCI-AD data analysis! NCI-AD is rich person-reported data showing outcomes of older adults and people with physical disabilities who receive publicly funded LTSS. States use the data in a myriad of ways to understand and improve quality of their service delivery. Thankful to participating states and our project partners Stephanie Giordano, Eric Lam, and Nilufer Isvan at Human Services Research Institute.

The CDC estimates there were 139.8 million emergency room visits in 2021. The costs of a visit to the ER can vary widely, but these unexpected expenses can be devastating, especially for people with disabilities and older adults who have lower incomes and often have other disability- or age-related medical expenses. According to NCI-AD ACS data from 2022-23: • 2 out of every 5 respondents had gone to the emergency room for any reason in the past 12 months. • Almost one-third of respondents had an overnight stay in a hospital in the past 12 months. At the same time, we see high rates of people reporting they are able to access primary care services, such as annual check-ups and mental health services. These high rates of health care utilization among NCI-AD ACS respondents emphasize the importance of having high quality data on disability, so our systems can appropriately monitor the use and quality of health care for people with disabilities and non-disabled peers. Each year, HSRI and our partners at ADvancing States work closely with states to administer the National Core Indicators Aging and Disability Adult Consumer Survey (NCI-AD ACS) to a diverse sample of older adults and people with physical disability who receive publicly funded long-term services and supports. As a result of these efforts, NCI-AD ACS is the most comprehensive source of information on the demographics and outcomes of this diverse population. Among NCI-AD ACS respondents in 2022-23: • Over one-quarter identify as a racial or ethnic minority • One-third are 65 years old or younger • Almost two-thirds have a physical disability • One-third have a co-occurring mental health diagnosis There is growing recognition that people with multiple intersecting identities, such as having a disability and representing a racial minority group, can have greater inequities in health than peers who do not have intersecting identities, such as non-disabled BIPOC individuals. It is crucial that we do a better job of examining the differences in outcomes for these groups; one example of this is from our colleagues at the Community Living Policy Center. They used data from NCI-AD in 2017-2018 to demonstrate that unmet need for HCBS was significantly higher among people from minoritized racial/ethnic groups. As we improve our understanding of inequities for people with disabilities, we can improve our approaches to making services work better for everyone.
