California's nonprofit health data network
About us
You have a job: Making healthcare better We have a job: Getting you the information you need We know the problems you’re facing. Not being able to control costs for your patients. Crowded emergency departments. Patient leakage. Treatment adherence. Readmission penalties. Daily action lists. The success of your healthcare organization - the jobs of the people working for you and the outcomes of your patients - depends on your ability to solve these very tough issues. You cannot do any of this effectively with just the information in front of you or in your patients’ medical records. You need to know who their entire care team is. You need to know what medications they are taking. You need to know when your patients are being discharged from the hospital and need a helping hand. Manifest MedEx—a nonprofit health network serving all of California— is helping health leaders reach their goals of improving outcomes and reducing costs. We’re unlocking information from silos so you can put it to work to make healthcare better. Manifest MedEx facilitates the secure exchange clinical and claims records through a network of 125+ hospitals, 1500 ambulatory care sites, and 12 health plans. We are proving that collaboration and sharing patient data is good medicine. We are providing information that: - Notifies you immediately when one of your patients is admitted or discharged from the hospital. - Gives your care coordinators and care managers access to detailed, longitudinal health records including claims, clinical records and lab results. - Shows the entire medication list when you prescribe a new medicine. - Means you can follow-up with a discharged patient before they even arrive home. - Helps you identify and manage high need patients. - Enables you to analyze and predict trends in your patient populations. -Reduces the burden of HEDIS® reporting with our NCQA validated data

“We've had those sort of requests bouncing toward us for a long time. There are lots of great use cases for that and we are hopeful that through the QHIOs connecting with each other and everybody giving their ADT information to the QHIOs, we will both, as producers of ADT messages, be able to say, hey, we're not going to be managing this anymore. You guys can go out and contact one of your QHIOs and they'll be able to provide you with that. Also, as receivers, we will now be able to subscribe and get a fuller picture of where patients who we are primarily responsible for their care are getting care. I am excited about that, and I'd like to see how that develops in the next six months.” -- Adam Davis, MD, Physician Informaticist, Sutter Health https://lnkd.in/gt8U6ydx X:

Hospitals need a thorough picture of their patients’ health information to better address their needs, but patients often visit facilities and sites that use different EHRs than the hospital’s, many of which are not connected to national networks. With longitudinal patient records from MX Access, an easy-to-use web-based platform, hospital care teams can streamline ED throughput and optimize population health insights with patients’ medical histories aggregated from EHRs across the state, including recent encounters, a list of your patients’ providers, and lab results from recent doctor visits.
