This single, statewide network of coordinated care delivers home and community based services to over 66,000 older adults and individuals with disabilities per month, providing over $600 million per year in services .The mission of MHC and our member agencies is to help individuals live at their highest level of functioning possible, in the least restrictive setting possible, for as long as possible.

In 2019, Massachusetts unveiled a plan to amplify, align, and coordinate local, regional, and statewide efforts to create a welcoming and more livable Commonwealth for residents of all ages. This year, Massachusetts is working to refresh the plan to guide the state’s age- and dementia-friendly actions for the next 5-10 years. The Executive Office of Elder Affairs will co-host listening sessions around the Commonwealth to hear more from local communities on how to strengthen Massachusetts’ age- and dementia-friendly work. There is a Zoom session on July 30th, in addition to in-person meetings in Salem, Barnstable, Boston, Lawrence, and Springfield. Congratulations to the recent grant awardees for expansion of the Hospital to Home Partnership Program: * Elder Services of Cape Cod and the Islands in partnership with Cape Cod Healthcare * Mystic Valley Elder Services in partnership with Winchester Hospital * Elder Services of Berkshire County, Inc. in partnership with Berkshire Health Systems * Highland Valley Elder Services in partnership with Cooley Dickinson Hospital * AgeSpan and Greater Lynn Senior Services, Inc. in partnership with Salem Hospital * Minuteman Senior Services in partnership with Emerson Hospital These ASAPs join our existing Hospital to Home grantees including WestMass ElderCare, Inc., Elder Services of Worcester Area, Inc., Tri-Valley, Inc., LifePath, Somerville-Cambridge Elder Services, Greater Springfield Senior Services, Inc. , Old Colony Elder Services (OCES), and Springwell, Inc. The Hospital to Home Partnership Program has been a tremendous success so far: ASAP liaisons received more than 1,800 referrals from their hospital partners in just the first eight months of the program, helping to divert nursing home placements and facilitating safe and supported discharges to patients' homes!
