    Did you know that 60% of documents in the electronic medical records come with improper identification, such as the wrong title? Or that one in four documents have the wrong date—or no date at all? Those are just two of the many significant gaps in basic document information that we’ve found in our research. In the best case, these gaps make it difficult for clinicians to find the data they’re looking for. But they can also lead to confusion when, for example, a colonoscopy record uploaded in 2023 describes a procedure done in 2012. Luckily, AI can help turn this chaos into clarity.     In response to a recent poll conducted by our VP of Value-Based Care, Dana McCalley, MBA, two-thirds of respondents said they planned on doing RxHCC (prescription drug hierarchical condition categories) risk adjustment coding in 2025. But why is RxHCC important and how can organizations ensure they’re coding accurately? As part of the Inflation Reduction Act (IRA) prescription drug benefit redesign, health plans will be encumbered with significantly higher liabilities than before. Plans will seek to offset some of these headwinds by better representing patient health status and risk profile through RxHCC coding, which has been recalibrated to better account to the changes in the IRA. With accurate risk coding more important than ever to both payers and providers, Navina has introduced robust support for RxHCC risk adjustment coding.
