Great case for a GGO today using Noah Galaxy. 16mm LUL with average HF of -586. Navigated in 20 seconds and then corrected the CTBD inherent in all robotic platforms (EMN and shape sensing) using TILT (see image 1 below). This updates in real time the lesion location and gives augmented fluoroscopy. There was no airway into this lesion so we used a Medtronic needle to make a hole targeting the lesion using augmented Fluro and the Galaxy targeting system (see image 2 below). This aspirate was positive for tumor (see image 3 below). Then, because we didn't have to blind ourselves and remove the optics, we could see the hole we made that lead directly into the tumor and pass our forceps in for larger biopsies (see image 4 below). the TBBx tissue was positive for tumor on ROSE but also the VanGogh system (see image 5 below). Total radiation was 19mGy for the entire case (radiation used for 1 TILT, 2 aspirates, 4 biopsies, and a fiducial placed). Disposable scope thrown away, minimal radiation dose to the patient and my team, and no additional expensive mobile cone beam required to make my robot actually work.
