15% Off Pediatric Care: MCOs' 'Special' Way Of Saying Thanks For All The Checkups!   Pediatric practices continue to grapple with Managed Care Organizations (MCOs) and several Medicaid programs aiming to decrease payments for services provided by "Advanced Practice Health Care Providers," like nurse practitioners and physician assistants. Amid predictions of an increase in the number of non-physician providers in pediatric care in the coming years, there's an increasing fear that such payment policies will jeopardize the financial health of pediatric practices nationwide. A prime example is United Healthcare's published national policy to cut 15% off the contracted allowed amounts for non-physician services. Conceptually, this reduction is often inaccurately applied across all components of a CPT code, ignoring the distinctions between provider's education & training (wRVU), practice operational costs (peRVU), and medical malpractice costs mRVU). By not confining such discounts solely to the work component of the individual CPT code (wRVU), MCOs and Medicaid plans effectively undermine their own rationale for payment cuts. The repercussions of such over-reaching reductions could endanger the financial viability of pediatric practices, potentially jeopardizing the quality of healthcare for children for many years to come.
