Our hub and spoke model of care reached over 23,000 individuals, resulting in 78,000+ substance-related visits. ✨What is the Specialized Co-Enrollment Program (Co-Enrollment)? This is an innovative collaboration between the Public Health Institute at Denver Health, Denver Health (DH) and Denver WIC to streamline WIC education, enrollment, and benefits for DH patients and families. The program is designed to boost the number of eligible but not currently enrolled (EBNE) individuals in the WIC program. It also aims to ensure that those already enrolled continue to receive WIC benefits by coordinating medical visits.
