“The key to the healthcare problem is we can engage the payers and we can engage the providers, but at the end of the day, if you don’t have an engaged and activated patient, we can’t get ahead of the problem that we’re facing.” Abi Sundaramoorthy, MD, MBA, Chief Medical Officer at Wellinks, sat down with Phreesia to discuss the crucial role patient activation plays in improving health outcomes. Learn how the Patient Activation Measure® (PAM®), a tool that measures a patient’s knowledge, skills and confidence to self-manage their health, helps healthcare organizations empower patients: https://lnkd.in/gvRNTc2p

When patients provide information about their sexual orientation and gender identity (SOGI), clinicians can provide better-informed care. But #LGBTQ patients are not always comfortable sharing these details with healthcare providers and staff due to fear of discrimination or mistreatment. To help ease patient discomfort in disclosing personal information, the CARE Center partnered with Phreesia to digitally collect important data directly from patients. Read about the impact this has had on patient outcomes in Xtelligent Healthcare’s recent article: https://bit.ly/3A8nu0Q

Did you know 1 in 5 people speak a language other than English at home, but most practicing physicians are not fluent in any language other than English? If patients can’t communicate with providers in their language of preference, it could jeopardize their safety, care quality and health outcomes over the long term. Swipe below to see the four benefits of communicating with patients in their language of preference. Want to learn more about how preferred-language communication improves care and how technology can bridge the communication gap? Visit: https://lnkd.in/gVrFdz3v

The Patient Activation Measure® (PAM®) surveys a patient’s knowledge, skills and confidence necessary to self-manage their health and segments them into 4 PAM levels ranging from “overwhelmed and disengaged” (level 1) to “proactive and goal oriented” (level 4). Christina Suh MD MPH, Director of Clinical Content at Phreesia, explains how the PAM® is predictive of most health behaviors, helping organizations and providers gather insight needed to personalize care for high-risk and high-utilization patients.
