Did you know that most types of dementia are *not* passed down genetically? In fact, genetic factors are rarely the primary reason a person experiences dementia. Most cases of dementia are influenced by a combination of factors, such as lifestyle choices or chronic health conditions. A year-long case study of the age 60+ population in San Francisco totaled 80k ER visits — 60% of those visits were for cognitive impairment and 70% of those patients had no medical reason for being there. If you do the math, that's over 30k unnecessary emergency room visits for vulnerable patients with dementia — in one city, in one year. Now zoom out nationally: Medicare members with dementia account for 1 in 5 ER visits and 1 in 3 hospitalizations. Needless to say, it’s a massive problem. Remo’s dementia experts share three preventative steps that caregivers can take to avoid hospitalization of their loved one. Taking on the role of caregiver demands extraordinary emotional and physical commitment – 74% of dementia caregivers say they are concerned about maintaining their own health, and approximately 60% of family caregivers report a very high rate of emotional stress. CMS' new GUIDE model offers a promising solution by incentivizing comprehensive dementia care and providing financial support to organizations meeting specific criteria for both patients and their caregivers. As a provider in the GUIDE program, Remo takes a data-driven approach to serving patients and caregivers. Read more about how our team is partnering with organizations like HDAI to leverage data analytics to identify areas of greatest need and tailor care delivery for optimal impact. 👉 https://lnkd.in/gXKnCWrJ cc Health Data Analytics Institute (HDAI) Holli White #dementia #dementiadiagnosis #dementiacare #GUIDEmodel #CMS #data #caregiversupport #dementiacaregivers #alzheimers

People with Alzheimer's disease and other forms of dementia heavily depend on family and friends for daily assistance, but there is limited understanding of care transitions over time. Data from the Health and Retirement Study reveals that 1/3 of people with dementia received help with daily activities two years before their first positive dementia screen, increasing to 60% during the initial positive screen (primarily from family care). These findings highlight gaps in dementia care and suggest targeted interventions for subpopulations with inadequate caregiving resources.
