Our team partnered with national researchers to provide the first Quality Improvement web application to help CAHs manage, track, and benchmark post-acute measures for swing bed programs. After collecting three years of quality data, we have found that CAH swing-bed programs outperform Skilled Nursing Facilities (SNFs) in quality performance. Specifically, we found that: - 73.6% of CAH swing bed patients were discharged to their community, significantly exceeding SNFs’ rate of 49.7%. - 8% of CAH swing-bed patients experienced unplanned returns to acute care, compared to SNFs' 11.4%. - 9.2% of CAH swing-bed patients had returned to acute care within 30 days post-discharge, whereas SNFs' rate stood at 10.5% Read more in our latest white paper: https://hubs.li/Q02LNXnk0 #StroudwaterAssociates #RuralHealthcare
