How do you improve your SDLC team's learning culture?
I worked at a company where the CEO wanted to "make a note in their personnel file" anytime someone made a mistake. It completely killed the teams' motivation to try new ideas or even speak up. You have to break a few eggs to make a cake. In startups especially if you are afraid to make a mistake you can't build anything truly disruptive and innovative. Embrace the failures, learn from them, and try again.

How can you manage expectations when using Agile Methodologies?
The important thing to remember is agile won't fix a culture of poor communication. Everyone on the team needs to be diligent about communicating their current status, any unforeseen issues, and potential upcoming roadblocks. Daily standups can start to feel less productive with everyone going through the motions. It's critical that we keep those focused.
