    For twenty years Hospital Focus 5 has worked with eight different organizations in the Hartford Hospital System. During this time a transformational process now called How Hartford HealthCare Works or H3W.

  • Transfer Trauma

    PARTICIPANT In a study of Transfer Trauma with a team from the University of Florida, the University of Kansas, the University of Missouri-Kansas City and the Philadelphia Geriatric Center.

    Moving 300 patients from an older 312 bed SNF to a new, replacement 420-bed SNF without any move related set backs or deaths.
