Because discharge is its own operational and clinical entity that interfaces with all other aspects of inpatient care, it requires its own definitions and improvement focus. In this article, we use the Institute of Medicine’s (IOM) Crossing the Quality Chasm quality domains to define high quality discharge processes. We then provide a framework to align aspects of the discharge process with each of the quality domains and synthesize methods of achieving high quality discharge processes.
