    The Hospitalist

    As pediatric hospitalists, our organ is the hospital. We care for a wide variety of pathology in hospitalized children and work side-by-side with nearly every subspecialty, in both inpatient and outpatient settings. Through our clinical work and care coordination efforts, we directly feel the effects of our hospital systems and the societal powers that surround them.     As pediatric hospitalists, our organ is the hospital. We care for a wide variety of pathology in hospitalized children and work side-by-side with nearly every subspecialty, in both inpatient and outpatient settings. Through our clinical work and care coordination efforts, we directly feel the effects of our hospital systems and the societal powers that surround them. Thus, we are uniquely positioned to have strong connections to individual patients, the situations that brought them to us, and the care continuum they travel through.

    In this article, we identify the top 10 publications of 2021, as presented at the Pediatric Update at SHM Converge 2022 in Nashville, Tenn., with summaries for five of those publications. We include clinical topics that may change how we practice as individuals, but also larger societal issues we can influence as hospital-based physicians.

  • In Search of The Perfect Discharge: A Framework for High-Quality Hospital Discharges

    Pediatric hospital discharges have broad reaching effects throughout the healthcare system, and inadequate discharge processes are detrimental to patients, families, and hospitals. Because discharge is its own operational and clinical entity that interfaces with all other aspects of inpatient care, it requires its own definitions and improvement focus.     Pediatric hospital discharges have broad reaching effects throughout the healthcare system, and inadequate discharge processes are detrimental to patients, families, and hospitals. Because discharge is its own operational and clinical entity that interfaces with all other aspects of inpatient care, it requires its own definitions and improvement focus. In this article, we use the Institute of Medicine’s (IOM) Crossing the Quality Chasm quality domains to define high quality discharge processes. We then provide a framework to align aspects of the discharge process with each of the quality domains and synthesize methods of achieving high quality discharge processes.
